Refeeding syndrome in hematological cancer patients - current approach.

Introduction: According to estimates based on the GLOBOCAN database of the International Agency for Research on Cancer, in 2018 alone, 18 100 000 cancers were globally diagnosed. Importantly, the majority of cancer patients experience unintended weight loss that leads to many adverse clinical consequences, including malnutrition and cancer cachexia. At the same time, each nutritional intervention must be carried out individually, as it can lead to critical complications, resulting in a threat to the health and life of the cachectic patient. An example of this type of risk is refeeding syndrome

[Malignant or benign hyperleukocytosis ?].

Leukocytosis or an increase of white blood cells in the peripheral blood is a frequent anomaly. It is important to immediately distinguish if it is a benign or malignant leukocytosis and the analysis of the blood formula is the first step. The presence of abnormal cells such as blast cells is an alarm which must immediately consider the diagnosis of leukemia, and medullogram has to be performed. The presence of lymphoma cells will lead to the conclusion of lymphoproliferative disorder and the presence of myeloid precursors to a myeloproliferative disorder. However, a benign leukemoid reaction

Notch signaling: its roles and therapeutic potential in hematological malignancies.

OBJECTIVES: Behçet's disease (BD) is a chronic multisystem inflammatory disorder associated to uveitis that may represent a serious sight-threatening condition. The purpose of the present study is to assess the effectiveness of adalimumab as new strategic therapeutic approach in patients affected by severe Behçet's uveitis. METHODS: Clinical data from twelve selected patients (22 eyes) were retrospectively analysed. All patients received 40 mg of adalimumab subcutaneously, once every 2 weeks, in addition to traditional immunosuppressive on-going therapy and eight of them were switched to adal

The role of miR-29 family members in malignant hematopoiesis.

AIMS: MicroRNAs of the miR-29 family members were one of the first microRNAs identified as possible therapeutic agents in malignant hematopoiesis. The aim of our review is to summarize the current state of knowledge on miR-29 family members. METHODS: We performed literature searches involving miR-29 family members and their relationship to individual hematological malignancies, namely acute myeloid leukemia (AML), chronic lymphoblastic leukemia (CLL) and chronic myeloid leukemia (CML). We also searched for subgroups of hematological malignancies, e.g. multiple myeloma, that are regarded as me

Drug-related thrombosis in hematologic malignancies.

Cancer patients are at increased risk for thrombosis. Among the predisposing factors for the hemostatic imbalance, drugs have a definite role. Induction of thrombosis by drugs involves a variety of mechanisms: Enhancement of procoagulant activity, reduction in anticoagulants synthesis, stimulation of platelet aggregation and endothelial damage. L-asparaginase is associated with thrombotic events, mainly in the venous system. Supportive therapy with fresh frozen plasma is probably insufficient and heparin needs further evaluation. Venous thromboembolism has recently emerged following thalidomid

Myeloid/Lymphoid Neoplasm with FGFR1::ZMYM2 Rearrangement Presenting as T-Cell Acute Lymphoblastic Lymphoma with Concurrent Myeloproliferative Neoplasm: A Case Report.

Background: Myeloid/lymphoid neoplasms with FGFR1 rearrangement (MLN-FGFR1), also known as 8p11 myeloproliferative syndrome, are rare and aggressive hematologic malignancies arising from pluripotent stem cells. The FGFR1::ZMYM2 fusion resulting from t(8;13)(p11.2;q12) characteristically co presents as T-lymphoblastic lymphoma in lymph nodes alongside a myeloproliferative neoplasm in the bone marrow. The disease is resistant to tyrosine kinase inhibitors and conventional chemotherapy, and carries a median survival of less than 12 months without allogeneic hematopoietic stem cell transplantation

Non-Relapse Mortality With Bispecific Antibodies in Hematologic Malignancies: A Dual Systematic Review and Pharmacovigilance Analysis.

Bispecific antibodies (BsAbs) redirect T-cells to tumor antigens in an MHC-independent manner. These universally available, off-the-shelf products have demonstrated unprecedented efficacy in heavily pretreated patients with relapsed/refractory (R/R) hematological malignancies (HMs). The indefinite treatment duration associated with most BsAb products, coupled with neutropenia, B-cell aplasia, and hypogammaglobulinemia result in significant immune-related toxicities and infections. Hence, non-relapse mortality (NRM) which represents the cumulative burden of treatment-related toxicities, defined

Immune escape mechanisms and therapeutic advances in virus-associated hematological malignancies.

Virus-associated hematological malignancies have become a key focus in oncology research due to their complex pathological mechanisms and unique immune escape capabilities. In recent years, research in this area has increased with a deeper understanding of the role of viruses in tumorigenesis. However, the immune escape mechanisms in virus-associated hematological malignancies remain incompletely elucidated, posing challenges for the development and optimization of treatment strategies. This review summarizes the roles of various known viruses-particularly Epstein-Barr virus (EBV), human immun

Defining lines of therapy in haematological malignancies: a proposed systematic and comprehensive framework from the EBMT Practice Harmonisation and Guidelines Committee.

The therapeutic landscape of hematological malignancies has been transformed by targeted therapies, immunotherapy, and cellular therapies, leading to more complex and prolonged clinical courses. Currently, there is substantial variability in how "lines of therapy" (LoT) are defined and recorded, which hinders research and data harmonization. To address this, the EBMT Practice Harmonisation and Guidelines Committee has developed a standardized framework for LoT across acute leukemias, lymphomas, and multiple myeloma. A LoT is defined as a coherent therapeutic episode aimed at achieving or maint

Primary antifungal prophylaxis in hematological malignancies. Updated clinical practice guidelines by the European Conference on Infections in Leukemia (ECIL).

At the 10th European Conference on Infections in Leukaemia (ECIL), the guidelines for antifungal prophylaxis in pediatric and adult patients with hematological malignancies (HM) were updated and some changes introduced. Regarding acute myeloid leukemia (AML) and myelodysplastic syndromes (MDS) patients undergoing remission induction chemotherapy, a B-II grading has been assigned to isavuconazole, micafungin, and caspofungin, based on non-randomized studies that have shown efficacy in preventing invasive fungal diseases (IFD). Regarding high-risk MDS patients treated with azacytidine, prophylax

[Disseminated intravascular coagulation associated with hematologic malignancies].

Hematological malignancies that are frequently complicated with disseminated intravascular coagulation (DIC) include acute leukemia, high-grade malignant lymphoma, and cases undergoing certain immunotherapy or cellular therapy. In typical cases of DIC associated with hematological malignancies, bleeding symptoms are aggravated not only by the activated fibrinolytic system but also by thrombocytopenia induced by the underlying disease or anticancer therapy, and their management is of critical clinical interest. As such, antithrombotic therapy with heparinoids carries significant risk of bleedin

[Anti-rituximab antibodies in autoimmune diseases and hematologic malignancies: An update].

Rituximab, a chimeric monoclonal antibody targeting the CD20, is a key therapy for treating haematological malignancies and numerous autoimmune diseases. However, its use may be complicated by the occurrence of anti-drug antibodies (ADA), which reflect its immunogenicity. This immunogenicity varies depending on the clinical context: it remains rare in oncology, but is more common in autoimmune diseases. These ADAs may be neutralising or non-neutralising and may influence pharmacokinetics, B-cell depletion, clinical efficacy and the risk of hypersensitivity reactions. Their occurrence is influe

International Consensus Classification of myeloid and lymphoid neoplasms: myeloproliferative neoplasms.

The recently published International Consensus Classification (ICC) of myeloid neoplasms summarized the results of an in-depth effort by pathologists, oncologists, and geneticists aimed to update the 2017 World Health Organization classification system for hematopoietic tumors. Along these lines, several important modifications were implemented in the classification of myeloproliferative neoplasms (MPNs). For chronic myeloid leukemia, BCR::ABL1-positive, the definition of accelerated and blast phase was simplified, and in the BCR::ABL1-negative MPNs, the classification was slightly updated to

Alterations in the ribosomal machinery in cancer and hematologic disorders.

Ribosomes are essential components of the protein translation machinery and are composed of more than 80 unique large and small ribosomal proteins. Recent studies show that in addition to their roles in protein translation, ribosomal proteins are also involved in extra-ribosomal functions of DNA repair, apoptosis and cellular homeostasis. Consequently, alterations in the synthesis or functioning of ribosomal proteins can lead to various hematologic disorders. These include congenital anemias such as Diamond Blackfan anemia and Shwachman Diamond syndrome; both of which are associated with mutat
