MRI volumetry of medial temporal lobe structures in amnesia following herpes simplex encephalitis.

Using a magnetic resonance imaging-based volumetry, we quantified the volumes of the hippocampal formation (HF), parahippocampal gyrus (PHG), amygdaloid body (AMB), and anterior temporal neocortex (ATN) in 5 post-herpes simplex encephalitic (post-HSE) patients with temporal lobe damage and memory impairment at 12-52 months after the onset, and in 10 age-matched control subjects. In the post-HSE patients, the HF (p < 0.001) and PHG (p < 0.005) were significantly atrophic, while the AMB (p = 0.155) and ATN (p = 0.102) were smaller but not significantly. Performances on the verbal learning memory…

Follow-up of pregnant women at high risk of transmitting herpes simplex virus.

Prospects for neonatal herpes transmission were studied in a group of pregnant Chilean women at high herpetic risk--including 59 with a history of genital herpes, 11 with a first genital herpes episode during the observed pregnancy, and 16 whose sexual partners had a history of genital herpes. Each woman completed a survey questionnaire, provided serologic samples for detection of herpes simplex virus (HSV), and provided periodic samples taken with cotton swabs for HSV isolation. The 86 women who completed the study had an average age of 28 years; most (58.8%) were primiparas. Only 21 of the 8…

Herpes simplex dissemination following glucocorticoids for upper airway obstruction in an adolescent girl.

A previously healthy adolescent female suddenly developed a severe febrile pharyngitis and impending upper airway obstruction. Her Monospot test was positive, and her throat culture grew group A beta-hemolytic streptococcus. After treatment with antibiotics and glucocorticoids, she developed a fulminant course dominated by disseminated intravascular coagulation and hepatic necrosis. At autopsy, herpes simplex virus type-1 was cultured from lung, liver, and spleen tissue. We believe this is the first report of disseminated herpes simplex infection after steroid use for upper airway obstruction.

[Chronic mucocutaneous herpes simplex infection. Occurrence within the scope of liver-induced immunodeficiency].

Herpes simplex virus (HSV) infections causing severe disease are reported frequently in patients suffering from human immunodeficiency virus (HIV) infection. This disease pattern may also be seen in an immunocompromised disease state with other causes, however, as in the case presented in this paper. An 84-year-old woman had hepatic cirrhosis resulting from chronic hepatitis C virus infection. The woman developed ulcerative lesions in and around her mouth and in the genito-anal region, and these persisted for some months. Diagnosis of HSV infection was not obtained until after extensive labora…

Category-specific form-knowledge deficit in a patient with herpes simplex virus encephalitis.

In-depth case study of a herpes simplex virus encephalitis patient who presents with a relatively clear knowledge disorder and anterograde amnesia in the absence of any other major cognitive deficit. The main neuropsychological feature is a category-specific impairment restricted to living things. The patient misnamed pictures of animals and vegetables, could not accurately draw animals from memory or verbally describe their visual appearance, and was not accurate in sorting pictures of real animals from pictures of unreal animals; conversely, in the same tasks her performance with artifactual…

Use of polymerase chain reaction for laboratory diagnosis of herpes simplex virus encephalitis.

Polymerase chain reaction was used to detect herpes simplex virus (HSV) specific deoxyribonucleic acid (DNA) sequences in acute and convalescent cerebrospinal fluid (CSF) and brain tissue of a 78-year-old man and in CSF of a neonate who died of complications owing to herpes simplex virus encephalitis (HSVE). Polymerase chain reaction (PCR) was carried out for 35 cycles with a set of primers that bracketed a 92 base pair segment unique to the HSV DNA polymerase gene. Amplified DNA was electrophoresed on 3 percent agarose gel, blotted onto a nylon membrane, and probed with 32p-labeled oligonucle…

The 'other' sexually transmitted diseases. Chlamydial, herpes simplex virus, and human papillomavirus infections.

The incidence of sexually transmitted diseases continues to rise across the country. Because many cases are subclinical and asymptomatic, chlamydial, herpes simplex virus, and human papillomavirus (HPV) infections continue to spread at an alarming rate. These infections can lead to serious sequelae, such as pelvic inflammatory disease, infertility, and cervical dysplasia or carcinoma, so improved disease-control strategies are needed. Preventive efforts should include use of the latest diagnostic and therapeutic methods to uncover and eradicate subclinical chlamydial and HPV infections. In add…

Targets of herpes simplex virus type 1 infection in a mouse corneal model.

In animal models, spread of herpes simplex virus type 1 (HSV-1) from epithelial replication sites to the peripheral and central nervous system is known from analysis of individually dissected tissues. To examine virus spread in undissociated tissues, corneas of adult mice were inoculated with HSV-1. After 1 to 13 days groups of mice were perfused with formalin, and decalcified blocks of head and neck were embedded in paraffin. At intervals, serial sections were screened for HSV antigen. On days 1 and 2, viral antigen was restricted to cornea and conjunctiva but by days 3 and 4 was also seen in…

Early loss of astrocytes in herpes simplex virus-induced central nervous system demyelination.

Immunohistochemistry was used to study herpes simplex virus type 1-induced central nervous system demyelination in the trigeminal root entry zone of mice inoculated with herpes simplex virus type 1 by the corneal route. There was no change in peripheral nervous system myelin as shown by immunostaining for P0 glycoprotein. Double immunoperoxidase staining for herpes simplex virus type 1 antigens and glial fibrillary acidic protein showed that most of the infected cells were astrocytes. Glial fibrillary acidic protein immunostaining was completely lost in the inferior medial portion of the trige…

[Diagnosis of acute hepatitis caused by herpes simplex virus using in situ hybridization].

A 50-year-old male without relevant past history was admitted because of fever lasting for 23 days. Physical examination showed hepatomegaly and splenomegaly without other findings. Laboratory studies only revealed mildly abnormal hepatic enzymes. The remaining investigations (markers, serologies, antinuclear antibodies, blood and urine cultures) were negative. Chest and abdomen X-ray films were normal. In abdominal echogram a homogeneous liver without space occupying lesions was seen, and computed tomography disclosed enlarged liver, spleen and lymph nodes. Needle hepatic biopsy was reported

Genital infections due to herpes simplex virus type 2 in male guinea pigs.

A small-animal model of genital herpes simplex virus (HSV) infection has not been available for investigating the pathogenesis of disease in males. Male Hartley guinea pigs were inoculated on the medial thigh with 10(5)-10(6) plaque-forming units of HSV type 2. Animals developed vesicular lesions in the genital area, including the penis. Transient urinary retention, hindlimb paralysis, and acute urethral shedding of HSV also occurred. During acute infection, virus was detected in neural and genital tissue homogenates, including the peripheral nerve, lumbosacral dorsal root ganglia (DRG), spina…

Herpes simplex virus causes amplification of recombinant plasmids containing simian virus 40 sequences.

Simian virus 40 (SV40) DNA, inserted into a plasmid vector, does not replicate when transfected into baby hamster kidney cells. However, when the recipient cells are superinfected with herpes simplex virus type 1 (HSV-1), extensive amplification of the introduced plasmid occurs. Deletion of the late SV40 region or part of the coding sequences of the small tumour (t) antigen has no effect on the efficiency of amplification, whereas manipulations affecting either the SV40 origin of replication or the integrity of large tumour (T) antigen substantially decrease HSV-induced amplification. Phosphon…

Effect of ketorolac on herpes simplex virus type one ocular infection in rabbits.

Corticosteroids can exacerbate viral ocular infections. Ketorolac tromethamine is an effective nonsteroidal anti-inflammatory agent that may be a useful substitute for corticosteroids following ocular surgery. In this study, rabbits ocularly infected with herpes simplex virus type 1 (HSV-1) were treated topically four times daily with 0.5 percent ketorolac or 0.1 percent dexamethasone for 7 days after infection. Severity of the infection was determined by scoring corneal opacity and HSV-1 corneal ulcerations with the Draize scale as well as iritis and conjunctivitis. Ten days after treatment e…

Herpes simplex virus-mediated human hypoxanthine-guanine phosphoribosyltransferase gene transfer into neuronal cells.

The virtually complete deficiency of the purine salvage enzyme hypoxanthine-guanine phosphoribosyltransferase (HPRT) results in a devastating neurological disease, Lesch-Nyhan syndrome. Transfer of the HPRT gene into fibroblasts and lymphoblasts in vitro and into hematopoietic cells in vivo has been accomplished by other groups with retroviral-derived vectors. It appears to be necessary, however, to transfer the HPRT gene into neuronal cells to correct the neurological dysfunction of this disorder. The neurotropic virus herpes simplex virus type 1 has features that make it suitable for use as …

[Complete human Klüver-Bucy syndrome after encephalitis caused by herpes simplex type 2].

A case of complete Klüver-Bucy syndrome is reported. A 42 year-old man developed Herpes Simplex type II (H.S.V.II) encephalitis. Good sparing of language functions allowed thorough neuropsychological testing. The troubles usually described in Klüver-Bucy syndrome as psychic blindness, colour agnosia, prosopagnosia, auditive and tactile agnosia were present. The authors theorize that these symptoms are mainly correlated with the amnestic syndrome, which is constantly reported in human Klüver-Bucy syndrome. Unfortunately, after 8 weeks, the encephalitis recurred and the patient was left demented…

Herpes simplex viral infection in human neonates: an immunohistochemical and electron microscopic study.

Specimens obtained at autopsy from six neonates with herpes simplex virus (HSV) infections were examined microscopically, electron microscopically, and immunohistochemically. Coagulative necrosis with inclusions was found in the livers and adrenal glands in all cases, as well as in various other organs, including the spleen, bone marrow, lungs, esophagus, tongue, and thymus, in some cases. Distinct hemorrhagic diathesis was found in three cases. No characteristic clinical findings, such as skin rashes or elevated titers of the antibody to HSV, were found, and clinical diagnosis was therefore d…
