Saccharomyces boulardii CNCM I-745 mitigates antibiotic-induced gut microbiome functional alterations independently of the host.

The probiotic Saccharomyces boulardii CNCM I-745 (Sb) is widely prescribed to alleviate antibiotic-induced diarrhea, yet its mode of action, particularly its potential direct effects on the gut microbiome, remains incompletely defined. This study aimed to evaluate whether Sb can directly mitigate antibiotic-induced gut microbiota dysbiosis and influence downstream host immune response. Using both static (MiPro) and dynamic (SHIME®) in vitro gut microbiota models, we assessed the effects of Sb supplementation under antibiotic treatment with amoxicillin/clavulanic acid (AMC) or vancomycin (Van).

Inflammatory markers in the cerebrospinal fluid linked to mortality in tuberculous meningitis.

This study examines the role of host inflammation in the high mortality of tuberculous meningitis (TBM) and identifies potential biomarkers associated with improved survival. We conducted a case-control study involving 131 patients in a discovery cohort, 81 TBM patients in a validation cohort, and 43 non-infected controls from a referral hospital in Indonesia. We measured 94 inflammation-related proteins in cerebrospinal fluid (CSF) and performed genome-wide quantitative trait loci (QTL) mapping. Sixty-seven proteins were found to be differentially expressed between TBM patients and controls,

Recent advances in the genetic engineering of the Leishmania parasite and anti-cancer properties.

Leishmaniasis is a tropical disease caused by Leishmania species, affecting millions of people worldwide and contributing to substantial morbidity and mortality. Advances in genetic engineering technologies, particularly CRISPR/Cas9, plasmid shuffling, and DiCre-based systems, have significantly enhanced our understanding of Leishmania biology. These approaches have enabled precise gene editing, functional analysis of essential genes, and the development of genetically attenuated strains with potential applications in vaccine design and drug discovery. Gene editing tools have also allowed the

The circadian control of tryptophan metabolism regulates the host response to pulmonary fungal infections.

The environmental light/dark cycle has left its mark on the body's physiological functions to condition not only our inner biology, but also the interaction with external cues. In this scenario, the circadian regulation of the immune response has emerged as a critical factor in defining the host-pathogen interaction and the identification of the underlying circuitry represents a prerequisite for the development of circadian-based therapeutic strategies. The possibility to track down the circadian regulation of the immune response to a metabolic pathway would represent a unique opportunity in t

Microbiota and IPF: hidden and detected relationships.

Lung microbiota (LM) is an interesting new way to consider and redesign pathogenesis and possible therapeutic approach to many lung diseases, such as idiopathic pulmonary fibrosis (IPF), which is an interstitial pneumonia with bad prognosis. Chronic inflammation is the basis but probably not the only cause of lung fibrosis and although the risk factors are not completely clear, endogenous factors (e.g. gastroesophageal reflux) and environmental factors like cigarette smoking, industrial dusts, and precisely microbial agents could contribute to the IPF development. It is well demonstrated that

Novel targets for hepatitis B virus therapy.

Treatment with either pegylated interferon-alpha (pegIFN-α) or last generation nucleos(t)ide analogues (NAs) successfully leads to serum viral load suppression in most chronically infected hepatitis B (CHB) patients, but HBsAg loss is only achieved in 10% of the cases after a 5-year follow-up. Thus, therapy must be administered long-term and it will not completely eliminate infection because of the persistent hepatitis B virus (HBV) minichromosome in infected cells, and cannot completely abolish the risk of developing severe sequelae such as cirrhosis and hepatocellular carcinoma. Recent progr

Targeting Human-Cytomegalovirus-Infected Cells by Redirecting T Cells Using an Anti-CD3/Anti-Glycoprotein B Bispecific Antibody.

The host immune response to human cytomegalovirus (HCMV) is effective against HCMV reactivation from latency, though not sufficient to clear the virus. T cells are primarily responsible for the control of viral reactivation. When the host immune system is compromised, as in transplant recipients with immunosuppression, HCMV reactivation and progressive infection can cause serious morbidity and mortality. Adoptive T cell therapy is effective for the control of HCMV infection in transplant recipients. However, it is a highly personalized therapeutic regimen and is difficult to implement in routi

Mucosal immunology of HIV infection.

Recent advances in the immunology, pathogenesis, and prevention of human immunodeficiency virus (HIV) infection continue to reveal clues to the mechanisms involved in the progressive immunodeficiency attributed to infection, but more importantly have shed light on the correlates of immunity to infection and disease progression. HIV selectively infects, eliminates, and/or dysregulates several key cells of the human immune system, thwarting multiple arms of the host immune response, and inflicting severe damage to mucosal barriers, resulting in tissue infiltration of 'symbiotic' intestinal bacte

Review article: angiogenesis, neovascular proliferation and vascular pathophysiology as targets for cancer therapy.

A body of evidence that vascular-mediated damage occurs in murine tumours after many existing forms of anti-tumour therapy is rapidly accumulating (see Gray Conference Proceedings edited by Moore & West, 1991). Rapid conventional screens of cells in vitro or using leukaemias of lymphomas will not detect this mode of action and such screens will therefore miss effective agents. A change in the approach to experimental cancer therapy is needed to ensure that this important new avenue is fully investigated. Solid tumours will need to be studied and the importance of specific tumour cell biochemis

Identification of mitochondrial metabolism-associated tuberculosis key genes and construction of a diagnostic model based on WGCNA.

Despite recent advances, tuberculosis (TB) remains a complex global health challenge, with its causative bacterium, Mtb, actively manipulating host cellular metabolism to survive. Therefore, studying mitochondrial metabolism-related genes is crucial, as they offer a promising avenue for identifying diagnostic biomarkers and developing new host-directed therapies to combat the disease. This study integrated transcriptomic datasets from three TB cohorts in the Gene Expression Omnibus (GEO) database (training set: GSE83456; validation sets: GSE42826 and GSE19444). Differential analysis was perfor

Extracorporeal Photopheresis in Heart Transplantation: From Salvage Therapy to Immunosuppression-Sparing Strategy.

Heart transplantation remains the definitive therapy for end-stage heart failure, yet graft survival continues to be limited by acute and chronic rejection and the cumulative toxicity of lifelong immunosuppression. Extracorporeal photopheresis (ECP), an immunomodulatory therapy with established efficacy in graft-versus-host disease and lung transplantation, has been investigated for more than 3 decades as an adjunctive therapy in heart transplantation. While prior reviews have summarized historical outcomes, the role of ECP in contemporary transplant practice remains incompletely defined. This

Eradication of HCV by direct antiviral agents restores mitochondrial function and energy homeostasis in peripheral blood mononuclear cells.

Hepatitis C virus (HCV) adopts several immune evasion mechanisms such as interfering with innate immunity or promoting T-cell exhaustion. However, the recent direct-antiviral agents (DAAs) rapidly eliminate the virus, and the repercussions in terms of immune system balance are unknown. Here we compared the PBMCs transcriptomic profile of patients with HCV chronic infection at baseline (T0) and 12 weeks after the end of the therapy (SVR12) with DAAs. 3862 genes were differently modulated, identifying oxidative phosphorylation as the top canonical pathway differentially activated. Therefore, we

Targeted genetic and molecular therapies in neurofibromatosis - A review of present therapeutic options and a glimpse into the future.

Neurofibromatosis type 1, the most common phakomatoses, can present with a host of signs and symptoms, usually involving the skin and the peripheral nervous system. It is characterized by a mutation in the neurofibromatosis type 1 gene on chromosome 17q11.2 that codes for the protein neurofibromin. Neurofibromin acts as a tumor suppressor gene by inhibiting rat sarcoma (Ras) activity and its deficiency leads to increased Ras activity, cellular proliferation and tumor formation. This review was conducted to analyze the various targeted therapies at the genetic and molecular level employed to ma

SOHO State of the Art Updates and Next Questions | Best of Stem Cell Transplant: Recent Advances in Allogeneic Hematopoietic Cell Transplantation.

Allogeneic hematopoietic cell transplantation is undergoing rapid transformation, propelled by a wave of practice-changing studies that are reshaping how donors are selected, how graft-versus-host disease (GVHD) is prevented, and how post-transplant relapse is managed. Here we review the major advances of the past several years, focusing on practice-changing studies from 2024 to 2026, and the questions they raise. The adoption of post-transplant cyclophosphamide (PTCy) as a near-universal GVHD prophylaxis backbone has diminished the long-standing primacy of human leukocyte antigen matching, en

Induction of β-Defensin 1 (DEFB1) via synthetic mRNA reduces Cryptosporidium parvum infection in human intestinal cells.

BACKGROUND: Cryptosporidiosis, caused by the protozoan parasite Cryptosporidium, is a leading cause of severe diarrhea, morbidity, and mortality in immunocompromised patients and malnourished children. Because the only FDA-approved treatment, nitazoxanide, shows limited efficacy in malnourished children and is ineffective in the immunodeficient, novel therapeutics for vulnerable populations are urgently needed. Host-directed therapy (HDT) is a promising strategy that utilizes the induction of endogenous protective molecules. For example, intestinal epithelial cells defend the mucosa by upregul…

A novel potent autophagy inhibitor HM-013 limits production of infectious dengue virus particles.

BACKGROUND: Dengue virus (DENV) infects millions of individuals annually, yet no specific antiviral therapy exists. Autophagy, a conserved cellular degradation pathway, is activated during DENV infection and supports the production of infectious virions. Although autophagy modulation has emerged as a potential antiviral strategy, few small-molecule autophagy inhibitors with both potent anti-dengue activity and low cytotoxicity have been reported. Thus, identifying novel, safe, and effective autophagy-targeting compounds remains an important unmet need for host-directed antiviral development.
