Axillary Artery Injury and By-Pass Restoration After Open Reduction of a Chronic Shoulder Dislocation: A Case Report.

Injury of the axillary artery after open reduction of a chronic shoulder dislocation is a rare and life-threatening condition. We present a case of an elderly woman suffering from a chronic shoulder dislocation which was addressed initially with close reduction and secondarily, after re-dislocation, with open reduction. Intraoperatively axillary artery rupture was established. By-pass restoration with a saphenous vein graft successfully managed the complication. The humeral head was immobilized in the glenoid with temporary K-wires. A CT-angiography was performed on the first and second days p…

Shoulder Hemiarthroplasty for Proximal Humerus Fracture.

There are a variety of treatment options available for proximal humerus fractures, including nonoperative management, open reduction internal fixation with screws, locking plates, intramedullary nailing, or suture fixation, and arthroplasty, including hemiarthroplasty and total shoulder replacements. Fracture characteristics, including the number of fracture parts and involvement of the humeral head and glenoid and the patient's functional status and postoperative goals help dictate the optimal choice. Although the indications for hemiarthroplasty as treatment for severe proximal humerus fract…

[Subcutaneous rupture of tendons and muscles of the extremities].

Under study there were 278 patients with 285 subcutaneous ruptures of tendons and muscles of the upper and lower extremities. The tendon of the long head of the humeral biceps muscle and Achilles tendon are most liable to subcutaneous ruptures (123 patients--43.2% and 97 patients--34% respectively). Pathohistological examinations have shown that subcutaneous ruptures of tendons and muscles usually take place against the background of dystrophic alterations reducing their mechanical strength. The authors find it expedient to use polymer materials (kapron, lavsan) for the restoration of injured

What Is the Optimal Graft and Transfer Location in Lower Trapezius Tendon Transfer for Posterosuperior Massive Rotator Cuff Tears? A Cadaveric Biomechanical Simulation Study.

BACKGROUND: Lower trapezius tendon (LTT) transfer can restore shoulder function, particularly active external rotation (ER) following massiv … transferred graft. Under each condition (intact rotator cuff, MRCT, LTT-Achilles, ST-S, and ST-M), humeral head translation and functional abduction force (FAF) were evaluated at 0°, 30°, and 60° of glenohumeral elevation. ER torque was assessed across 5 angles (60° internal rotation [60IR], 30° internal rotation [30IR], neutral, 30° ER [30ER], and 60° ER [60 …

[Proximal interlocking of humeral intramedullary nails and risk of axillary nerve injury].

QUESTION: Possible injuries to the axillary nerve are criticised in antegrade and retrograde endomedullary nailing of the humeral shaft during proximal interlocking. Clinical experience seems not to support the theory of a high risk of nerve injury. The real risk although remains unknown under anatomical-morphological aspects. MATERIAL AND METHODS: Eight complete human cadaveric shoulder-arm regions with original soft-tissue coverage had been used. Four times the unreamed humeral nail (UHN) had been inserted retrograde, and four times antegrade, distributed in each pair. Antegrade interlockin

Anesthesia mumps and morbid obesity.

Parotid gland swelling during/after surgery is a rare but known complication of general anesthesia. It has been found to be associated with patients of all age groups and various surgical procedures. We have found this rare complication to be associated with morbid obesity as well. A forty five year old morbid obese lady (BMI 50.5 kgm-2) was operated for fracture shaft of left humerous after positioning in right lateral position. Intraoperatively, her head was positioned using soft bandage head ring used commonly for such positioning. Postoperatively unilateral painful parotid gland swelling w

Reverse Hill-Sachs and Hill-Sachs Lesion Repair with Fresh Talus Osteochondral Allograft: A Series of 3 Patients.

CASE: Hill-Sachs lesions and reverse Hill-Sachs lesions (HSLs) are common in patients with shoulder instability, and addressing these lesions effectively is crucial for patient outcomes. Our study investigates a novel technique using talus osteochondral allografts (TOA) for the treatment of these lesions. The TOA offers several advantages, including congruency to the humeral head, dense bone structure, and a reduced risk of contamination compared with other allografts. CONCLUSION: We present 3 patients with large engaging Hill-Sachs lesions or reverse HSLs, who were treated using a TOA, highl…

Treatment of Chronic Locked Posterior Dislocation of the Shoulder: Is There a Time Limit to Try a Closed Reduction?: A Report of 3 Cases.

CASE: Three patients with chronic locked posterior shoulder dislocation diagnosed 4 to 10 weeks after injury, each with a reverse Hill-Sachs lesion involving >25% of the humeral head, underwent closed reduction under general anesthesia using the "2 thumbs" technique, followed by immobilization in external rotation. CONCLUSION: At a mean 4-year follow-up, all had excellent function without recurrent instability, avascular necrosis, or osteoarthritis. In carefully selected patients, closed reduction remains a safe and effective option beyond the 3-week time frame, when surgery is traditionally …

Biomechanics and Pathoanatomy of Posterior Shoulder Instability.

Posterior glenohumeral instability represents a wide spectrum of pathoanatomic processes. A key consideration is the interplay between the posterior capsulolabral complex and the osseous anatomy of the glenoid and humeral head. Stability is dependent upon both the presence of soft tissue pathology (eg, tears to the posteroinferior labrum or posterior band of the inferior glenohumeral ligament, glenoid bone loss, reverse Hill Sachs lesions, and pathologic glenoid retroversion or dysplasia) and dynamic stabilizing forces. This review highlights unique pathoanatomic features of posterior shoulder…

Luxatio erecta humeri with neurovascular compromise: inferior glenohumeral dislocation illustrating associated injuries.

Luxatio erecta humeri (LEH) is a rare type of shoulder dislocation in which the humeral head becomes trapped beneath the glenoid. Patients present with the arm in a fixed hyperabducted position. LEH is estimated to occur in 0.5% of all shoulder dislocations, most often caused by trauma and has a significant association with local bone, ligament and less frequently neurovascular injury. We present a case with initial neurovascular compromise and sustained neurological impairment at long-term follow-up. Urgent closed reduction is advocated and subsequent treatment is dependent on associated inju…

[Bilateral posterior fracture-dislocation of the shoulder caused by an epileptic seizure - diagnostic, treatment and result].

The case of an 39-year-old man is presented, who sustained a bilateral locked fracture dislocation of the shoulders occurring during an epileptic seizure. Radiographs demonstrated a compression-fracture of the anteromedial aspect of the humeral head bilaterally (reversed Hill-Sachs-lesions). Additionally, fractures of the lesser and greater tuberosity were diagnosed at the right site. Open reduction and internal fixation was performed in both shoulders within 12 hours. 6 months later the patient has no complaints with a free range of motion. Diagnostics, treatment and result are discussed in c…

Sex differences in shoulder acromiohumeral contact surface arc length on three-dimensional computed tomography imaging.

Anatomy-based guidelines for shoulder surgery have established the routine preoperative evaluation of the humeral head. Despite recognized sex differences in humeral head size, there has been limited investigation into sex-specific variations in acromiohumeral contact surface (AHCS) arc length. This study aims to assess sex differences in the AHCS arc length within a sample of the Chinese population. We retrospectively analyzed 169 normal shoulder CT images from a single medical center, collected between 2011 and 2021. The AHCS arc length was defined as the distance from the superior glenoid t…

The acromiohumeral centre edge angle: A new radiographic measurement and its association with rotator cuff pathology.

BACKGROUND: Various radiographic measurements that describe humeral head coverage by the acromion and the effect on rotator cuff pathology have been reported. This study aimed to describe and validate a new radiographic measurement, the acromiohumeral centre edge angle (ACEA). METHODS: We compared the ACEA on computed tomography (CT) and plain X-ray to determine whether X-ray is accurate for measuring this angle. We then compared the results from this control population with 107 patients with acute rotator cuff tears. We compared functional outcomes in rotator cuff tear patients to determine …
