Measured gain in projection with the extended columellar strut-tip graft in endonasal rhinoplasty.

IMPORTANCE: The extended columellar strut-tip graft was designed to improve nasal tip projection and tip definition in patients undergoing rhinoplasty. OBJECTIVE: To determine whether the extended columellar tip graft leads to a true and measurable increase in nasal tip projection or simply gives the illusion of an increase in projection. DESIGN: Retrospective case review. The mean time of follow-up photographs was 32 months after surgery (range, 8 months to 10 years). PARTICIPANTS: The study population comprised 15 patients who underwent primary or revision rhinoplasty during the last 10 y

Seeking safety therapy for pathological gambling and PTSD: a pilot outcome study.

This pilot study evaluated Seeking Safety (SS) therapy for seven outpatients with current comorbid pathological gambling (PG) and posttraumatic stress disorder (PTSD). This represents the first treatment outcome study of this population, and included both genders and 29% minorities. We found significant improvements in: PTSD/trauma (the PTSD Checklist criterion B symptoms; the Trauma Symptom Inventory overall mean and subscales anxiety, dissociation, sexual abuse trauma index, sex problems; and the World Assumptions Scale benevolence subscale); gambling (the Gamblers Beliefs Questionnaire over

Impossible spaces: maximizing natural walking in virtual environments with self-overlapping architecture.

Walking is only possible within immersive virtual environments that fit inside the boundaries of the user's physical workspace. To reduce the severity of the restrictions imposed by limited physical area, we introduce "impossible spaces," a new design mechanic for virtual environments that wish to maximize the size of the virtual environment that can be explored with natural locomotion. Such environments make use of self-overlapping architectural layouts, effectively compressing comparatively large interior environments into smaller physical areas. We conducted two formal user studies to explo

Mirror therapy in children with hemiplegia: a pilot study.

Mirror therapy, which provides the visual illusion of a functional paretic limb by using the mirror reflection of the non-paretic arm, is used in the rehabilitation of hemiparesis after stroke in adults. We tested the effectiveness and feasibility of mirror therapy in children with hemiplegia by performing a pilot crossover study in ten participants (aged 6-14 y; five males, five females; Manual Ability Classification System levels: one at level I, two at level II, four at level III, three at level IV) randomly assigned to 15 minutes of daily bimanual training with and without a mirror for 3 w

Systematic approach needed to establish cause of vertigo.

Vertigo can be defined as an illusion or hallucination of movement. The control of balance is complicated. Vertigo can be caused by many different pathologies, some of which are potentially life threatening. An important differentiation is whether the symptoms of vertigo originate from a central or peripheral origin. Clues to a central origin are other brainstem symptoms or signs of acute onset such as headache, deafness and other neurological findings. These patients warrant urgent referral and investigation. Red flags in patients with vertigo include: headache; neurological symptoms; and neu

Supernumerary phantom limbs in spinal cord injury.

STUDY DESIGN AND OBJECTIVES: Case report and review of supernumerary phantom limbs in patients suffering from spinal cord injury (SCI). SETTING: SCI rehabilitation centre. CASE REPORT: After a ski accident, a 71-year-old man suffered an incomplete SCI (level C3; AIS C, central cord syndrome), with a C3/C4 dislocation fracture. From the first week after injury, he experienced a phantom duplication of both upper limbs that lasted for 7 months. The supernumerary limbs were only occasionally related to painful sensation, specifically when they were perceived as crossed on his trunk. Although the

Is iliotibial band syndrome really a friction syndrome?

Iliotibial band (ITB) syndrome is regarded as an overuse injury, common in runners and cyclists. It is believed to be associated with excessive friction between the tract and the lateral femoral epicondyle-friction which 'inflames' the tract or a bursa. This article highlights evidence which challenges these views. Basic anatomical principles of the ITB have been overlooked: (a) it is not a discrete structure, but a thickened part of the fascia lata which envelops the thigh, (b) it is connected to the linea aspera by an intermuscular septum and to the supracondylar region of the femur (includi

Angled aorta ("sigmoid septum") as a cause of hypertrophic subaortic stenosis.

Review of the hearts of seven patients in whom hypertrophic obstructive cardiomyopathy had been diagnosed by the usual clinical and morphologic criteria revealed diminished angles between the interventricular septa and ascending aortas in three cases. The angles in these three hearts were 90 to 110 degrees, as compared with a mean value of 145 degrees in the other four hearts with hypertrophic obstructive cardiomyopathy and 140 +/- 14 degrees in 55 control hearts. None of the patients with hypertrophic subaortic stenoses and angled aortic roots died of the heart disease, and none had either as

[Differential diagnosis of tinnitus and vertigo. A review].

Tinnitus and vertigo, two common neurological complaints, often challenge the physician's ability with respect to possible etiology. Objective tinnitus can result from an abnormally patent eustachian tube, from tetanic contractions of the muscles of the soft palate, or from vascular abnormalities within the head or neck. Subjective tinnitus refers to lesions involving the external ear canal, tympanic membrane, ossicles, cochlea, auditory nerve, brainstem, and cortex. As many as 50% of patients with tinnitus do not exhibit associated hearing loss; in these patients, the cause of the tinnitus is

Vision Impairment Provides New Insight Into Self-Motion Perception.

PURPOSE: Leading causes of irreversible blindness such as age-related macular degeneration (AMD) and glaucoma can, respectively, lead to central or peripheral vision loss. The ability of sufferers to process visual motion information can be impacted even during early stages of eye disease. We used head-mounted display virtual reality as a tool to better understand how vision changes caused by eye diseases directly affect the processing of visual information critical for self-motion perception. METHODS: Participants with intermediate AMD or early manifest glaucoma with near-normal visual acuit

Akinetopsia on Driving.

Akinetopsia is a quite rare symptom. Two types of akinetopsia have been reported: one is cinematographic vision, and the other is invisibility of moving objects. These symptoms are thought to occur due to dysfunction of the MT/V5 area at the occipitoparietal region. I herein describe a 54-year-old man who collided with a car parked on the left side of the road while driving. He complained that the parked car looked to be moving forwards and he could not stop his car when he noticed that it was parked. Magnetic resonance imaging disclosed the fresh infarction on the right temporoparietal region

Seurat's Dots: A Shot Heard 'Round the Art World-Fired by an Artist, Inspired by a Scientist.

The iconic phrase "a shot heard 'round the world" signifies an exceptional event. Seurat's masterpiece La Grande Jatte, painted with many thousand dots of color, came as a shot to the art world-a shot fired by the imagination of the artist and inspired by the color theories of a scientist.

Decoding early and late cortical contributions to individuation of attended and unattended objects.

To isolate a visual stimulus as a unique object with a specific spatial location and time of occurrence, it is necessary to first register (individuate) the stimulus as a distinct perceptual entity. Recent investigations into the neural substrates of object individuation have suggested it is subserved by a distributed neural network, but previous manipulations of individuation load have introduced extraneous visual confounds, which might have yielded ambiguous findings, particularly in early cortical areas. Furthermore, while it has been assumed that selective attention is required for object

Magno and parvo stimuli affect illusory directional hearing in normal and dyslexic readers.

In an experimental paradigm adapted from Hari (1995), forty observers listened via headphones to 8 binaural clicks: 4 left-ear leading followed by 4 right-ear leading with either 38 or 140 ms interstimulus intervals (ISIs). Concurrently, they viewed either foveal or peripheral visual stimuli designed to activate either the parvocellular or magnocellular pathway. They then reported the perceived location of each click-pair. Our results replicated Hari's finding that observers mistake the perceived location of short ISI click-pairs more often than long. That is, when ISIs were short, the sounds

Further considerations of visual cognitive neuroscience in aided AAC: the potential role of motion perception systems in maximizing design display.

Current augmentative and alternative communication technologies allow animation within visual symbol displays. Clinicians therefore have the option of incorporating motion-based effects into AAC displays. Yet there is no research in the field of AAC to guide this clinical decision-making, in terms of the number or types of animated symbols that would best suit specific communication needs. A great deal is known within the discipline of cognitive neuroscience about how humans perceive motion, however. In this paper we propose that the field of AAC might exploit these known principles of motion

Perceived speed differences explain apparent compression in slit viewing.

When a figure moves behind a stationary narrow slit, observers often report seeing the figure as an integrated whole, a phenomenon known as slit viewing or anorthoscopic perception. Interestingly, in slit viewing, the figure is perceived compressed along the axis of motion, e.g., a circle is perceived as an ellipse. Underestimation of the speed of the moving object was offered as an explanation for this apparent compression. We measured perceived speed and compression in anorthoscopic perception and found results that are inconsistent with this hypothesis. We found evidence for an alternative

Stroboscopic vision as a treatment for motion sickness: strobe lighting vs. shutter glasses.

INTRODUCTION: Countermeasures (e.g., drugs, training, etc.) designed to combat the problem of space motion sickness (SMS) have had only limited successes. A possible solution for SMS and terrestrial motion sickness may rest in preventing retinal slip via stroboscopic vision. METHODS: There were 32 subjects who participated in a study to investigate the effect of stroboscopic illumination on motion sickness using either a strobe light or liquid crystal display (LCD) shutter glasses. There were 19 subjects who read text and made +/- 20 degrees head movements in the horizontal plane (yaw) at 0.2

Visual hallucinations and the Charles Bonnet syndrome.

After dividing clinicians for almost 70 years, Charles Bonnet syndrome has reached an impasse. Defined by a neurologist in the 1930s, the syndrome was intended to eponymize the association of visual hallucinations with age, but evolved into one describing their association with eye disease or, more recently, an etiologically neutral phenomenologic description. Each tradition has its merits but none has defined a specific clinical entity or accounted for visual hallucinations across the spectrum of associated clinical conditions. Recent insights into the neurobiology of vision have shed new lig

[Comparison between motion sickness induced by reversed vision and that by parallel swing].

OBJECTIVE: To make clear the characteristics of motion sickness induced by reversed vision and to see whether it can be used in motion sickness experiments. METHODS: 10 healthy young men experienced walking tests wearing up-down or left-right reversing prisms, and parallel swing test on different days with intervals of 5-7d. Ataxia and motion sickness symptoms induced by walking wearing reversing prisms and those by parallel swing were observed. The reversed vision tolerance index (RVTI) and linear acceleration tolerance index (LATI) were calculated by an empirical formula. RESULTS: Both typ

A belongingness principle of motion perception.

Four experiments are reported that investigated the role of the perceived coplanarity of a moving target with respect to a frame of reference in the third dimension on the perceived path of that target. When a target dot and small moving frame appeared coplanar, the dot's perceived trajectory was governed entirely by its changing position relative to the moving frame. However, when the target and a large stationary frame appeared in a different plane than the small moving frame, the motion of the dot was seen independently of the moving frame. The results support a belongingness principle of m
