The relationship between ultrasonographically measured testicular volumes and cord blood inhibin B concentrations in healthy term male neonates.

Serum inhibin B (INHB) concentrations are associated with testicular volumes (TV) in all periods of childhood. The aim of the study was to investigate the relationship between TV measured by ultrasonography (US) and cord blood inhibin B and total testosterone (TT) concentrations, stratified by mode of delivery. In total 90 male infants were included. Testes of healthy, term newborns were evaluated by US on the third day after delivery. TV were calculated using two formulae: The ellipsoid formula [length (mm) × width (mm2) × π/6] and Lambert formula [length (mm) x width (mm) x height (mm) x 0.7

Selective enrichment of milk phospholipids via cryo-crystallization: application to various dairy matrices and elucidation of sphingomyelin crystallization mechanisms.

Milk sphingomyelin (SM) is a bioactive phospholipid involved in infant neurodevelopment and gut health; however, its proportion in dairy by-products is around 20% of total phospholipids, compared to over 30% in human milk, limiting its application in infant formula products. The molecular mechanisms underlying the selective enrichment of SM during cryo-crystallization were elucidated. Cryo-crystallization increased total phospholipid content by 2.12-fold, while the SM proportion increased from 27% to 41%. Lipidomics revealed preferential enrichment of very-long-chain fatty acid-containing phos…

Solely human milk diets for preterm infants.

Human milk provides not only ideal nutrition for infant development but also immunologic factors to protect from infection and inflammation. For the newborn preterm infant, the natural delivery of milk is not attainable, and instead pumped maternal milk, donor human milk, and human milk fortification are mainstays of clinical care. Current research demonstrates a decreased risk of necrotizing enterocolitis with maternal milk and donor human milk when individually compared to formula and with a complete human milk diet of maternal milk supplemented with donor human milk. The incidence of severe

Gastroesophageal reflux in infants: a primary care perspective.

This clinical paper discusses the role of the pediatric nurse in the evaluation and management of gastroesophageal reflux (GER) in infants. During an infant's first year of life, GER is a common occurrence and concern of families. The infant with uncomplicated GER may be managed conservatively with feeding schedule modifications, thickened feeds, changes in positioning, or a trial of formula change. Gastroesophageal reflux disease (GERD) is a pathological process in infants manifested by poor weight gain, signs of esophagitis, persistent respiratory symptoms or complications, and changes in ne

An unusual transmission of Neisseria meningitidis: neonatal conjunctivitis acquired at delivery from the mother's endocervical infection.

BACKGROUND: Primary meningococcal conjunctivitis is assumed to be due to the direct inoculation of Neisseria meningitidis into the conjunctival sac from an exogenous source. According to a literature review, no case of neonatal conjunctivitis infection acquired at delivery from maternal endocervicitis has been published. GOAL: To report a case of meningococcal neonatal conjunctivitis acquired at delivery because of the mother's endocervical infection and cross-transmission of the strain with her partner. STUDY DESIGN: Strains were characterized by bacteriologic and serologic methods includin

Is breast the best?

With the recent resurgence of breast-feeding, the medical profession has been facing numerous questions regarding the advantages, limitations and practicality of this mode of infant feeding. The recent identification in human milk of "growth modulators" has probably augmented its nutritional superiority. Several psychosocial advantages of breast-feeding have also become well recognized. The increasing knowledge on the immunologic quality of human milk has contributed a great deal to the explanation of the long noticed lower morbidity and mortality in breastfed infants. Several benefits have al

Association Between Infant-Feeding Practices and Early Childhood Caries: A Clinical and Epidemiological Study.

Background: Early childhood caries (ECC) remains one of the most prevalent chronic conditions among preschool children worldwide. Feeding practices during infancy play a significant role in shaping oral microbial colonization and caries risk. This study aimed to evaluate the association between breastfeeding, artificial feeding, and the occurrence of ECC among children aged 3-5 years. Methods: A cross-sectional clinical and epidemiological study was conducted involving 103 children aged 3-5 years. Parents completed a structured questionnaire. Participants were divided into three groups based o

In Support of Breast-/Chestfeeding by People With HIV in High-Income Settings.

Given that HIV can be transmitted through breastfeeding, historically, breastfeeding among women with HIV in the US and other resource-rich settings was discouraged. Formula feeding was the mandated feeding option out of concern for breast-milk transmission of HIV, which occurred in 16-24% of cases pre-antiretroviral therapy (pre-ART) use. In January 2023, the US Department of Health and Human Services' Perinatal Guidelines were revised to support shared decision-making for infant feeding choices. Updated clinical trials' data from resource-limited settings suggest the actual breastmilk HIV tr

Clinical update: understanding jaundice in the breastfed infant.

Breastfed infants are more likely to be jaundiced than infants who are formula fed. Community practitioners need to understand the physiology of jaundice and the issues associated with breastfeeding so that they can support parents. Visible jaundice is a result of hyperbilirubinaemia and, in most cases, is harmless and caused by normal physiological processes. It does, however, require detection monitoring and sometimes treatment to prevent rare but serious health complications. Although some debate remains over the association between breastfeeding and jaundice, the literature suggests that i

Infant feeding experiences and concerns among caregivers early in the COVID-19 State of Emergency in Nova Scotia, Canada.

The global emergency caused by the novel coronavirus (COVID-19) pandemic has impacted access to goods and services such as health care and s … ection (both caregiver and infant), and a lack of access to goods, namely, human milk substitutes ('infant formula'), and services, including health care, lactation support, and social supports. Most COVID-19-related information was sought from the internet and social media, so for broad reach, future evidence-based information should be shared via online platfo …

Effects of opaque, weighted bottles on maternal sensitivity and infant intake.

Caregivers' abilities to assess how much is in the bottle may lead to encouragement of infant bottle emptying and overfeeding. The present study assessed whether use of opaque, weighted bottles (as compared with conventional, clear bottles) improves feeding outcomes. Mothers with infants <32 weeks of age (n = 76) were assessed on two separate days. Mothers fed their infants from an opaque, weighted bottle on 1 day and a clear bottle on the other; conditions were counterbalanced. Blinded raters certified in the Nursing Child Assessment Feeding Scale scored all videos to determine maternal sensi

Diagnosis and treatment of gastro-oesophageal reflux disease in infants and children.

Gastro-oesophageal reflux is a frequent, aspecific phenomenon in infants and children. The recommended approach in infants with uncomplicated regurgitation consists of reassurance of the parents and, if this fails, dietary recommendations in formula-fed infants. If, despite these efforts, symptoms persist, administration of prokinetics, such as cisapride, is recommended prior to investigations such as oesophageal pH monitoring. Oesophageal pH monitoring is also recommended to document gastro-oesophageal reflux disease in children with unusual presentations such as chronic respiratory disease.

Using Birth Cohort Data to Estimate Prenatal Chemical Exposures for All Births around the New Bedford Harbor Superfund Site in Massachusetts.

BACKGROUND: Children born near New Bedford, Massachusetts, have been prenatally exposed to multiple environmental chemicals, in part due to an older housing stock, maternal diet, and proximity to the New Bedford Harbor (NBH) Superfund site. Chemical exposure measures are not available for all births, limiting epidemiologic investigations and potential interventions. OBJECTIVE: We linked biomonitoring data from the New Bedford Cohort (NBC) and birth record data to predict prenatal exposures for all contemporaneous area births. METHODS: We used prenatal exposure biomarker data from the NBC, a

Low-protein diet supplemented with isomalto-oligosaccharide reduces diarrhea rate of weaned piglets by regulating intestinal function.

Excess protein in infant formula can harm the health of infants. Isomalto-oligosaccharide (IMO), a prebiotic, can be incorporated into infant formula. This study employed weaned piglets as a model to establish varying protein levels, investigating the impact of IMO in low-protein diets on the intestinal health. A total of 192 weaned piglets were randomly assigned to four groups, with each group containing six pens of eight piglets each. The control group was fed a diet containing 19% crude protein (CP), while the other three experimental groups were supplemented with 0.2% IMO, with CP levels o…

Correlations Among Parental and Neonatal Anthropometric Parameters, Feeding Practices and Infant Obesity.

BACKGROUND AND AIMS: Infant and adult obesity is becoming a real public health concern in Romania, similar to other countries of the European Union. Maternal obesity and excessive weight gain during pregnancy are proven risk factors for the obesity of the child. The protective role of the breastfeeding against obesity has also been demonstrated. The most important issue is whether the choice of a milk formula with the right protein composition could or not protect the newborn from becoming a future obese infant and child. Our study aims to describe the characteristics of a group of macrosomic

Prediction of the ideal length of insertion to monitor left atrial pressures in pediatric open-heart surgery: a retrospective cohort study.

BACKGROUND: Monitoring the left atrial pressures (LAP) is an important perioperative management tool for maintaining adequate cardiac output. However, incorrect placement of the LAP-monitoring catheter tip often leads to inherent risk. This study aimed to evaluate the length of the LAP catheter in pediatric open-heart surgery and devise a simple formula to predict the ideal length of insertion based on correlated demographic and clinical characteristics. METHODS: We conducted a retrospective clinical cohort study involving 79 infant patients aged ≤ 1 year who underwent open-heart surgery and

Transplacental oxygen transfer during physiological-based cord clamping in preterm lambs.

Physiological-based cord clamping (PBCC) involves aeration of the newborn lung before umbilical cord clamping. As the infant can continue to receive oxygen from the mother during PBCC, the dynamics of transplacental oxygen transfer are unknown, particularly following the onset of pulmonary gas exchange. We have investigated the effects of pulmonary ventilation with supplemental oxygen during PBCC on transplacental oxygen transfer in preterm lambs. Pregnant ewes and their fetuses (n = 8; 127 days of gestation; term 147 days) were instrumented with catheters and flow probes under general anesthe

Insensible water loss in extremely preterm infants cared for in incubators with 70% and 90% relative humidity.

OBJECTIVE: To determine the insensible water losses (IWL) in preterm infants cared for in incubators with 70% or 90% relative humidity (RH). STUDY DESIGN: This was a single center retrospective study of extremely preterm infants between 22- and 28-weeks gestational ages nursed in 70% and 90% RH. IWL was calculated using a formula that utilized infants' fluid intake, urine output and body weight changes. IWL between the two groups were compared using independent t-test. Multivariate linear regression was used to assess the effects of humidity on IWL after adjusting for confounding variables.
