INTRODUCTION: Pancreatic neuroendocrine tumors (pNETs), a group of endocrine tumors that arise in the pancreas from the hormonal cells of the islets of Langerhans, are among the most common gastroenteropancreatic neuroendocrine tumors. The incidence of pNETs has increased in recent years to about 1.5 per 100,000 population and prognosis correlates with histologic grade.This review aims to provide an overview of the newly approved or currently developing drugs over the past five years that have shown a significant improvement in prognosis for patients affected by this rare disease.

Human islet amyloid polypeptide (hIAPP) is the primary component of the amyloid deposits found in the pancreatic islets of patients with type 2 diabetes mellitus. However, it is unknown how amyloid fibrils are formed in vivo. In this study, we demonstrate that gangliosides play an essential role in the formation of amyloid deposits by hIAPP on plasma membranes. Amyloid fibrils accumulated in ganglioside- and cholesterol-rich microscopic domains ('lipid rafts'). Islet amyloid is a recognized characteristic finding in insulinoma and secondary islet hyperplasia resulting from severe insulin resistance. Little information is available about the presence of islet amyloid in primary islet hyperplasia (nesidioblastosis) of childhood. Here we report that islet amyloid was not present in 12 children with primary islet hyperplasia associated with hyperinsulinemic hypoglycemia.

Using a synthetic C- terminal tetradecapeptide of human islet amyloid polypeptide (IAPP), we prepared an antiserum for human IAPP [24-37] and established a highly sensitive radioimmunoassay (RIA) for human IAPP.
