In infertile women breast secretion may be provoked in approximately 11%, although basal prolactin serum levels are only elevated (greater than 12 ng/ml) in 30% of the cases. In a young woman a strong carpal-tunnel-syndrome was observed after everey of her three deliveries, apparently at the termination of the lactation. Pain and tingling in the arms and hands was very good to be influenced by diuretics. It has be concluded from these facts, that this type of carpal-tunnel-syndrome is to trace back to a temporary generalized oedema, probable caused by the hypothalamical oxytocin-producing centre.

A young female patient with amenorrhea, galactorrhea and hyperprolactinemia developed a carcinoma of the breast a few years later. Examination of the amenorrhea--galactorrhea syndrome did not reveal any prolactinoma so that hyperprolactinemia remains unexplained. Treatment with bromoergocryptine 5 mg daily was successful. The concept that domapinergic mechanisms control LH secretion by modulation of gonadotropin-releasing hormone (GnRH) has been recently investigated in man. Fifteen patients with hyperprolactinemia, amenorrhea, and galactorrhea were studied; 10 of these patients had apparent tumors. Nine of 12 patients had a suppression of prolactin with oral L-dopa, and all of 4 patients undergoing dopamine infusion had suppression of prolactin. Using prolactin suppression (L-dopa) and provocative (thorazine) tests along with the clomid test the intactness of hypothalamico-pituitary axis was assessed in a group of eight patients with galactorrhea syndrome. The acute effect of pyridoxine (B6) on serum GH and PRL levels and its chronic effects on galactorrhea in nine subjects (group I, n=4, idiopathic galactorrhea with normal PRL levels and normal menses; Group II, n=5, galactorrhea-amenorrhea with increased PRL levels) have been studied. Pyridoxine did not acutely alter GH or PRL levels. There was no decrease in galactorrhea, no resumption of menses and no decrease in PRL following tow months of B6 therapy.
