Effect of cervicolumbar coupling on spinal reflexes during cycling after incomplete spinal cord injury.

Spinal networks in the cervical and lumbar cord are actively coupled during locomotion to coordinate arm and leg activity. The goals of this project were to investigate the intersegmental cervicolumbar connectivity during cycling after incomplete spinal cord injury (iSCI) and to assess the effect of rehabilitation training on improving reflex modulation mediated by cervicolumbar pathways. Two studies were conducted. In the first, 22 neurologically intact (NI) people and 10 people with chronic iSCI were recruited. The change in H-reflex amplitude in flexor carpi radialis (FCR) during leg cyclin

Risk factors associated with dairy cattle lameness and hock and knee injuries on Iranian dairy farms.

Both lameness and hock and knee injuries represent significant welfare and economic concerns on dairy farms. However, there is a notable lack of data from certain housing systems, including bedded-pack farms and a few studies that compare free-stalls with bedded-pack farms. The objective of this study was therefore to utilise data collected during a welfare assessment to identify key risk factors for lameness and leg injuries in these housing systems. A generalised linear mixed model was used to identify risk factors associated with hock and knee injuries, whereas for lameness a binomial regre…

Computational analysis of the clinical presentation of a ligamentous Lisfranc injury.

Lisfranc injuries in the midfoot disrupt key arches of the foot which, if left untreated, can progress to pain, dysfunction, and arthritis. A clinical challenge is that 30-40% of Lisfranc injuries are missed in initial evaluations. The objective of this study was to explore different conditions of limb loading that could influence the biomechanics of the Lisfranc joint in a validated computational model. A computational model was created using SolidWorks software to represent the bones and soft tissues of the lower leg and foot. The model was compared to a cadaveric study of healthy and injure

Repairing Pretibial and Foot Soft Tissue Defects with Reverse Transplantation of the Medial Crural Fasciocutaneous Flap.

Soft tissue defects of the pretibial area and the foot are among the most common complications in patients with lower extremity injuries and remain a challenge for surgeons. This study examined the clinical effects of repairing pretibial and foot soft tissue defects with a medial crural fasciocutaneous flap. Twenty-three injury cases with soft tissue defects in the middle/lower parts of the tibia and ankle/heel were treated. Of these, 8 injuries were in the lower pretibial region, 11 injuries were in the heel, and 4 injuries were in the dorsalis pedis. The dimensions of the soft tissue defects

Bilateral isokinetic peak torque of quadriceps and hamstring muscles in professional soccer players with dominance on one or both two sides.

AIM: Dominance of one leg may cause asymmetry between the contralateral muscle groups and develop predisposition for injury to the leg with the weaker muscle strength. The purpose of this study was to examine the strength balance in the extensor and flexor muscle groups, as well as the hamstring to quadriceps (H/Q) ratios of both legs in professional soccer players with dominance on one or both legs. METHODS: Forty-two professional soccer players of the first Greek division participated in this study. Participants were divided into 3 groups according to their leg use during training sessions

Orthoplastic soft tissue management in lower limb open tibial fractures based on the angiosome concept: a narrative review.

The purpose of this article is to introduce the angiosome concept in the lower legs and its consideration in severe soft tissue injuries in open tibial fractures. The main aim is to focus on the vascularity and perforators in the crura and to emphasise step-by-step time management and proper debridement for securing the maximal viability of the soft tissue before the reconstruction phase. So far, many publications have presented optimal traumatological protocols and timing in lower leg injuries and many reviews have compared fracture stabilisation and flap reconstruction options; however, no c…

Novel measures of frontal plane knee wobbling predict anterior cruciate ligament injury in Japanese female basketball players: A prospective pilot study.

BACKGROUND: To prospectively examine how lower extremity movement patterns during single-leg landings predict anterior cruciate ligament (ACL) injuries in adolescent Japanese basketball players during a 3-year follow-up period. METHODS: 44 female basketball players, whose knee kinematics were measured using 2-dimensional video analysis during single-leg jump landings from a box 30 cm high. Peak knee valgus angle and knee wobbling during the landing task were primary measures of injury risk. Knee wobbling, defined as repeated knee valgus/varus movement, was calculated from relative frontal to

Imaging in the diagnosis and characterization of tennis leg.

Tennis leg is a common cause of acute posteromedial calf pain and encompasses a spectrum of injuries involving the posterior calf. Although initially described as rupture of the plantaris tendon, clinical, surgical, and imaging evidence indicates that injury to the medial head of the gastrocnemius at the distal myotendinous junction is the most frequent cause. Less commonly, the lateral gastrocnemius, the soleus, or the plantaris may be affected. Clinical presentation is often nonspecific and overlaps with important alternative diagnoses, including Achilles tendon rupture, deep vein thrombosis

Knee muscle and tendon stiffness in professional soccer players: a shear-wave elastography study.

BACKGROUND: It is well known that several risk factors such as fatigue, previous injury, muscle strength deficit or imbalance are related to a higher incidence of knee injuries in professional soccer players. Knee muscle and tendon stiffness may be another factor that is related to knee injuries in professional soccer players. Therefore, the aim of the present study was to investigate the differences between professional soccer players and healthy sedentary males in terms of the stiffness of patellar tendon (PT), quadriceps tendon (QT), and rectus femoris muscle (RF) and vastus medialis muscle

Effects of stiffness-altered sport compression garments on lower-limb biomechanics in cutting maneuvers.

Athletes commonly use compression garments (CGs) for perceived effectiveness in preventing injury occurrence. However, limited evidence is available on whether lower-limb CGs reduce the risk of injury. This study aimed at (1) evaluating the effects of CGs on mitigating the risk factors of cutting-related knee injuries; (2) identifying undesirable side-effects of CGs on other joints and cutting performance; and (3) identifying possible interactions between sex and condition. 62 healthy adults performed pre-planned 90˚ cutting tasks under four conditions: control, knee sleeves, placebo leggings

A combination of initial and follow-up physiotherapist examination predicts physician-determined time to return to play after hamstring injury, with no added value of MRI.

BACKGROUND: Previous studies investigating prediction of return to play after acute hamstring injury were limited by examining a single postinjury clinical and MRI evaluation. We evaluated the added value of including follow-up clinical evaluation when predicting return to play. METHODS: A range of clinical and MRI parameters were prospectively investigated for an association with the time to return to play in 90 athletes with MRI positive hamstring injuries undergoing a criteria-based rehabilitation programme. Clinical evaluation was performed within 5 days of injury and 7 days after this in

Immobilization with a Short Leg Cast in Acute Peroneus Brevis Tendon Tears.

BACKGROUND Acute ankle inversion sprain (AAIS) is a musculoskeletal condition commonly seen in emergency departments, often leading to peroneus brevis tendon (PBT) injuries. These injuries are usually treated with conservative management. The aim of this study was to determine the effectiveness of immobilization with a short leg cast in treatment of incomplete split PBT tears compared to when immobilization is not used. MATERIAL AND METHODS A group of 49 patients with PBT split injury occured by AAIS, as verified by magnetic resonance imaging (MRI), were evaluated from 2013 to 2021. Patients w

Early versus delayed amputation in the setting of severe lower extremity trauma.

Leg-threatening injuries present patients and clinicians with the difficult decision to pursue primary amputation or attempt limb salvage. The effects of delayed amputation after failed limb salvage on outcomes, such as prosthetic use and hospital deposition, are unclear. We evaluated the timing of amputations and its effects on outcomes. We retrospectively reviewed all trauma patients undergoing lower extremity amputation from January 1, 2000 through December 31, 2010 at a Level 2 trauma center. Patients undergoing early amputation (amputation within 48 hours of admission) were compared with
