[Heart rate disorders in potassium and magnesium deficiency].

Potassium and magnesium deficiencies are common in patients with heart disease. These are often coexistent and pathophysiologically related. Potassium deficiency cannot be treated without correction of concomitant magnesium deficiency. Correlations between serum levels and body stores are very poor for both ions. Therefore diagnosis and treatment of these conditions based on serum levels alone are erroneous. There is some evidence that it is primarily the intracellular depletion of these ions which is arrhythmogenic. Magnesium infusion has been proved effective in treatment of torsade de point…

Treatment of status asthmaticus with intravenous magnesium sulfate.

The bronchodilating effect of magnesium sulfate (MgSO4) was studied in two patients with status asthmaticus, who were intubated and mechanically ventilated by a respirator. Airway resistance was continuously monitored by the respiration-controlled interruption technique. After administration of 0.5 mmol/min MgSO4 intravenously, airway resistance decreased from 17 to 9, and from 13 to 8 mmHg/L/s in the two patients, respectively, and piping rales diminished or disappeared. We conclude that while corticosteroid therapy requires several hours to demonstrate significant effects in status asthmatic…

Influence of age and magnesium on calcium metabolism in rats.

This study evaluates the effect of dietary magnesium concentration on calcium metabolism in rats of differing ages. Young (3 wk) and old (18 mo) Fischer 344 rats were fed the AIN-76A diet modified to contain either low (218 mg/kg) or adequate (419 mg/kg) Mg for 4 wk. Some rats subsequently underwent a metabolic balance study (12 d duration). Other rats were gavaged with approximately 220 KBq (6 microCi) of 47Ca; daily fecal and urine collections were made and periodic whole body radioactivity determined. Femurs were removed and analyzed. Calcium retention and balance were not affected by Mg in…

Magnesium metabolism: a case report and literature review.

Magnesium has been shown to play an important role in the body. In the critical care setting of particular concern is the role of Mg++ in the treatment of malignant tachydysrhythmias such as ventricular tachycardia and ventricular fibrillation. Research is being conducted to determine appropriate methods of assessing Mg++ status, the relationship of altered Mg++ states to clinical manifestations, and the treatment of the abnormalities associated with Mg++ deficiency. Safe and effective forms of parenteral and oral Mg++ supplements are currently available.

Calcium, magnesium, and zinc supplementation and perinatal outcome.

The overall importance of nutrition to favorable perinatal outcome is only beginning to be fully appreciated. Although nutritional status can be linked to such things as socioeconomic class and education, it is nutrition directly that exerts a biologic effect. This review has attempted to look at three elements and their relationship to maternal and fetal outcome. At the present time, there does not seem to be a role for routine magnesium supplementation during pregnancy. Magnesium deficiency, as an isolated nutritional deficiency, is rare, and the evidence is, at best, weak that magnesium sup…

Serum magnesium levels in pregnancy and preterm labor.

Pregnancy is marked by a state of hypomagnesemia. The serum magnesium level shows no gestational dependence (mean, 1.79 +/- 0.44 mg/dl) until 33 weeks, at which point it continuously declines. Serum magnesium is not depressed further with the onset of labor at term. Patients in preterm labor have a significantly depressed serum magnesium level (mean, 1.60 +/- 0.46 mg/dl; 21 to 33 weeks; p less than 0.0005). This level was not dependent on whether the etiology for the preterm labor was premature rupture of the membranes (PROM), twin gestation, abruption, placenta previa with bleeding, or chorio…

Erythrocyte and plasma magnesium in sickle-cell anaemia.

Erythrocyte and plasma magnesium concentrations were measured in 168 subjects. Eighty three of the subjects were sickle-cell anaemia patients (sicklers) with haemoglobin genotype HbSS, and in the steady state; 30 were subjects who had the sickle-cell trait (HbAS), and 55 were subjects with the normal haemoglobin genotype HbAA. All the subjects were aged between 10 and 16 years. The sicklers had significantly lower erythrocyte (P less than 0.01) concentrations when compared with the normal subjects or subjects with the sickle-cell trait. Erythrocyte and plasma magnesium concentrations in subjec…

Magnesium sulfate is the ideal anticonvulsant in preeclampsia-eclampsia.

The pathogenesis of eclamptic convulsions is unknown. A review of the world literature indicates considerable controversy regarding the ideal anticonvulsant to prevent or control these convulsions. Parenteral magnesium sulfate is the drug of choice to control eclamptic convulsions in North America, but it is rarely used for this purpose overseas. The efficacy and safety of magnesium sulfate in the treatment of preeclampsia-eclampsia have been well documented during the past 60 years. During the same time period, numerous anticonvulsant drugs have been used overseas; however, the ideal drug is …

Magnesium deficiency in patients on home enteral nutrition.

Home enteral nutrition (HEN) is an established method of long term nutritional support. Many patients receiving HEN have Crohn's disease complicated by intestinal failure and malnutrition, including magnesium deficiency. It is unknown if HEN can correct magnesium deficiency or if patients on HEN can become magnesium deficient. We measured total magnesium intake in nine patients receiving HEN, and assessed their magnesium status. Two patients had magnesium intakes below the recommended dietary allowance of 15 mmol/day. Four patients (44%) had biochemical evidence of magnesium deficiency, althou…

Control of magnesium transport in the thick ascending limb.

The mammalian renal thick ascending limb of Henle (TAL) reabsorbs approximately 55% of the filtered magnesium; accordingly, it is the major segment involved in control of renal Mg balance. This review discusses recent evidence for passive and active transport of Mg through the paracellular and transcellular pathways of the TAL, respectively. The properties of these pathways provide a basis for understanding the factors influencing magnesium reabsorption and hormonal controls regulating Mg balance. Normally, Mg absorption is load dependent, whether delivery is altered by increasing luminal Mg c…

Clinical intervention studies on magnesium in myocardial infarction.

Patients with acute myocardial infarction (AMI) display a significant decrease in serum magnesium concentrations (S-Mg,c) during the initial 48 h after infarction. This decrease is not due to an increased renal magnesium loss, neither is it a dilution phenomenon. Consequently, a migration of Mg from the extracellular to intracellular space might take place, which probably is due to a catecholamine-induced increased lipolysis, forming insoluble intracellular Mg soaps. As the Mg ion is crucial in maintaining the electrical stability of the myocardium, we found it rational to avoid this postinfar…

[Hormonal regulation and metabolic inter-relations of magnesium].

Magnesium ion is of great importance in physiology by its intervention in 300 enzymatic systems, its role in membrane structure and its function in neuromuscular excitability. The skeleton is the first pool of magnesium in the body. Intestinal absorption, renal metabolism, bone accretion and resorption of magnesium are very similar to those of calcium. Magnesium metabolism is accurately controlled, in particular by parathyroid hormone, 25 - dihydroxy vitamin D3, calcitonin, catecholamine and estrogens. The main regulation mechanisms of magnesium metabolism are located in the kidney which is th…

Keratoconus, magnesium deficiency, type A behavior, and allergy.

The etiology of keratoconus is still unknown. This project was designed to study, in a group of keratoconic patients and a control group, the following: clinical, endocrinological, immunological, psychological, and ophthalmological factors. We found mean serum magnesium deficiency and type A behavior to be significantly more common in keratoconic patients than in a control group of patients. In addition changes in gluco-mineral corticoids, changes in glucose metabolism, edema of allergic origin, and genetic factors may collaborate in the development of keratoconus. All these factors could affe…

Magnesium supplementation in pregnancy. A double-blind study.

The effect of magnesium supplementation in pregnancy was studied in 568 women who were treated with 15 mmol magnesium-aspartate-hydrochloride per day or aspartic acid as placebo given orally during pregnancy from less than or equal to 16 weeks. Allocation to the two groups was performed according to the women's birthdates. Magnesium supplementation during pregnancy was associated with significantly fewer maternal hospitalizations, a reduction in preterm delivery, and less frequent referral of the newborn to the neonatal intensive care unit. The results suggest that magnesium supplementation du…
