Motion - colonoscopic surveillance is more cost effective than colectomy in patients with ulcerative colitis: arguments against the motion.

There are insufficient data upon which to base recommendations about surveillance colonoscopy and prophylactic colectomy for the prevention of colorectal cancer in patients with ulcerative colitis. Case series, analyses of intermediate results and extrapolations from other patient groups do not constitute reliable evidence. Available studies are susceptible to several biases: the 'healthy worker' effect, surveillance bias and selection bias. Patients who are enrolled in surveillance programs are more likely to be thoroughly evaluated beforehand, are more likely to be given a diagnosis of dyspl

Innovations in blood donor screening and blood collection.

Blood donor screening and blood collection are two of the donor-focused activities inherent in the process of maintaining an adequate blood supply. Donor screening represents procedures to provide safety for both donors as well as recipients. Donor medical history questions are so cumbersome that self-administered manual, computer-assisted, or on-line donor questions are under consideration in order to reduce time involved in the screening process. Blood collection procedures remain primarily based on whole blood donations, but apheresis donations are increasing steadily. The yield of multiple

Screening for apraxia: a short assessment for stroke patients.

Apraxia is a disorder that involves impaired ability to execute previously learned movements that cannot be attributed to basic sensory or motor disturbances. A thorough assessment of apraxia typically entails both pantomiming and imitation of transitive (tool-related), intransitive (communication-related), and meaningless gestures, presented in an array of different, process-dependent sensory conditions. Precise and detailed assessment tools are often time-consuming and a shorter screening tool may be desirable for efficient surveillance of this disorder in stroke patients. In the present stu

Retinopathy of prematurity.

With improving survival of very low birth infants in India, Retinopathy of Prematurity (ROP is likely to emerge as a significant problem. The most important risk factor in the pathogenesis of ROP is prematurity. Other factors like frequent blood transfusions; sepsis, apnea and problems with oxygenation have also been implicated in the causation of ROP. Essentially asymptomatic in the initial stages, a good screening program is essential for the early detection and treatment of this condition. Description of the various stages and threshold ROP has been included in the protocol. Guidelines rega

Detecting which invasive cancers at mammographic screening saves lives?

OBJECTIVES: Assessment of the features of primary operable breast carcinomas arising in women known to subsequently develop metastatic disease, to show for which invasive breast cancers earlier detection by mammographic screening is potentially beneficial. These data were applied to a separate series of screen detected cancers. METHODS: Features associated with the development of metastatic disease after a previous operable breast cancer were ascertained from examination of histological sections of the initial primary carcinoma and in particular the incidence of nodal positivity and definite

Is duplex scanning the best screening test for renal artery stenosis?

Screening for renal artery stenosis is indicated in patients with suspected renovascular hypertension or ischemic nephropathy to identify those who could benefit from renal artery interventions. The critical requirements for a clinically useful screening test include safety, low cost, and a high sensitivity or low false-negative rate. Arteriography remains the "gold standard" for the anatomic diagnosis of renal artery disease, but it is unsuitable for screening because of its high cost and invasive nature. Although renal duplex scanning technically is difficult, experienced laboratories have b

Klinefelter syndrome.

Klinefelter syndrome is the most common sex chromosome disorder. Affected males carry an additional X chromosome, which results in male hypogonadism, androgen deficiency, and impaired spermatogenesis. Some patients may exhibit all of the classic signs of this disorder, including gynecomastia, small testes, sparse body hair, tallness, and infertility, whereas others, because of the wide variability in clinical expression, lack many of these features. Treatment consists of testosterone replacement therapy to correct the androgen deficiency and to provide patients with appropriate virilization. T

Interpreting the mammogram report.

The standardization of imaging techniques, interpretation and reporting has become an important issue as the use of mammographic screening has increased. Each mammogram report should indicate whether the breast is fatty or consists of dense glandular tissue. A brief description of abnormalities should be followed by the conclusion and recommendations. Well-circumscribed lesions have a 98 percent benign rate; these lesions generally do not require biopsy but can be followed at six-month intervals for a period of time. The overall rate of malignancy for biopsies prompted by mammography is 20 to

Risk factors in breast cancer.

Risk factors are identified in less than 25 per cent of women who develop invasive breast cancer. The risk factors include a family history of breast cancer in a first degree family relative, late menopause, nulliparity, a first full term pregnancy after the age of 35 years, increasing age, P2 and DY mammographic parenchymal patterns, increasing dosage of non contraceptive oestrogens, mammary dysplasia associated with epithelial proliferation, a previous invasive breast carcinoma and carcinoma in situ.

Screening for breast cancer: opportunities, status and challenges.

Screening mammography and physical examination have made major contributions to the earlier detection of breast cancer. Mammography is the only reliable means of detecting nonpalpable cancers and can detect many minimal breast cancers when they appear to be curable. Low-cost, high-quality mass screening mammography, accurately interpreted, is feasible and could reduce overall mortality from breast cancer if it were more widely applied. Recognition of the need and desire to provide the service are essential initial motivating factors. The challenge to radiology is to obtain sufficient competent…
