BACKGROUND: Occult malformations of the lateral skull base are rare anomalies, but can cause severe complications such as recurrent meningitis. Therefore, they need to be precisely delineated and sufficient surgical closure is mandatory.
PATIENTS AND METHODS: Between 1986 and 2004 twenty patients (10 children and 10 adults) with occult malformations at the lateral skull base were treated surgically at the ENT-Department of the Hospital Fulda gAG. Of these 3 Mondini-malformations, 11 defects of the tegmen tympani or the mastoidal roof, 2 dural lesions to the posterior fossa and 4 malformations…

Cholesteatoma is a mass formed by the keratinizing squamous epithelium in the tympanic cavity and/or mastoid that can lead to the destruction of surrounding structures in the ear. It can arise at any age. Symptoms are non-specific and diagnosis may be difficult on otoscopic examination. The management of cholesteatoma is surgical; mastoid obliteration has reduced the incidence of recurrent cholesteatoma in recent years. The use of diffusion-weighted MRI has proven to be a reliable technique for the detection and follow-up of cholesteatoma.

A case of chronic suppurative otitis media is described in a patient with a chronic cold and cough. The presenting symptoms were otorrhoea, perforation of the tympanic membrane and granulation in the air cells of the left mastoid cavity. X-ray showed sclerotic mastoid and partial cellular obliteration. Histologic sections revealed aggregated fungal masses, filamentous hyphae and single-celled structures of Paecilomyces variotii. The infection responded to topical ketoconazole therapy. This is the first reported case of chronic suppurative otitis media caused by P. variotii.

The author presents a boy which has repeated facial palsies since his second year of life. The clinical examination showed: conduction deafness, radiological sclerosis of the bones of the basis of the skull, normal values of chemical blood analysis. Both facial nerves are operated in the mastoid and middle ear portions. The histological examination of the mastoid bone reveals a leontiasis ossea. The histological structure of the bone are described in detail and the differential diagnosis with other bone diseases of the cranial bones is discussed.

Otology & neurotology : official publication of the American Otological Society, American Neurotology Society [and] European Academy of Otology and Neurotology2012eng
OBJECTIVE: To evaluate patterns of failure for canal wall down mastoid cavities requiring surgical revision.
STUDY DESIGN: Retrospective review.
SETTING: Academic tertiary referral center
PATIENTS: Adults and children that underwent revision of an unstable open mastoid cavity from 1995 to 2010.
INTERVENTION(S): Review of demographic data, tympanomastoid pathology, and plausible risk factors for an unstable cavity. Available computed tomography (CT) scans were reviewed for indicators of suboptimal cavity shape. Spearman's correlation analysis was undertaken. Findings were classified as Type…

Otitis media either acute or chronic, is not uncommon in childhood. Multiple cranial nerve palsies occuring as a complication of either form of otitis media is unusual. A case of a nine year old boy with chronic suppurative otitis media with associated mastoiditis complicated with ipsilateral multiple cranial nerve palsies is presented. A skull X-ray and MRI scan showed sclerotic mastoids. The outcome on antibiotic treatment was good.

OBJECTIVE: To review the clinical experience for non-shaved middle ear/mastoid surgery and evaluate the proper method of preparing the postauricular surgical field.
METHODS: This retrospective study reviewed medical records of cases where the non-shaved surgical procedure was carried out for middle ear/mastoid diseases. In all cases, middle ear and mastoid surgery was performed by one otologic surgeon without hair shaving to treat chronic perforation of tympanic membrane, as well as chronic suppurative otitis media, with or without mastoiditis during two years. The prevalence of surgical site…

OBJECTIVE: Mastoidectomy can be a very challenging procedure for many reasons. The normal anatomy can be distorted because of inflammatory processes and tumors and recurrences. Avoiding injuries to the semicircular canals (SCCs) and facial canal is mandatory, and there is need to find a way to recognize the facial nerve and SCCs for safe performance of mastoidectomy. We describe, as a proof of concept, a novel technique to drill the mastoid while allowing the surgeon to recognize and avoid injuries to vital structures, in the cadaver.
METHODS: Four fresh cadaveric heads (8 sides) were prepare…

Auditory brainstem evoked potentials (ABEP) were recorded from 50 newborns (35-43 weeks gestational age), using three orthogonal differential electrode pairs, in addition to the widely used vertex-mastoid derivation. Potentials were evoked by alternating polarity, 75 dBnHL clicks presented monaurally at a rate of 10/s. From the records of the three orthogonal electrode pairs (nasion-inion; vertex-spinous cervical process VII; left-right mastoids), Three-channel Lissajous' trajectories (3CLT) were derived and analyzed. 3CLT point-by-point, as well as segmental descriptors were compared with pea…

A man in his early 40s, with no comorbidities, presented with a swelling in the right postauricular region for 5 days, which had progressively increased in size. He also gave a history of ear pain 2 months back. He denied history of ear discharge, difficulty hearing, giddiness or tinnitus. There was no history of fever, headache or seizures. On examination, the right pinna and external auditory canal were normal. The tympanic membrane was greyish white and intact. A swelling of size 4×3 cm was noted over the right mastoid region. High-resolution CT of the temporal bone showed right otomastoidi…

Gadolinium-enhanced MRI was used to evaluate 10 patients with Bell's palsy and one patient with facial paralysis secondary to Lyme disease. Nine of the eleven patients showed increased signal intensity of their facial nerve with gadolinium-enhanced MRI. In all nine patients, the facial nerve was involved at the labyrinthine, geniculate ganglion and proximal tympanic portions of the facial nerve, while two of the nine patients also had involvement of the mastoid segment of the facial nerve. Patients whose facial nerve enhancement was limited to the labyrinthine, geniculate ganglion and proximal…

An electrical stimulation in man applied between the two mastoids could facilitate the distinction between labyrinthine and retrolabyrinthine lesions by stimulating directly the primary vestibular afferences. However, for this test to be really effective in current medical practice, the results obtained in normal subjects must be symmetrical and reproducible one day to another. The ocular responses induced by a constant electrical stimulation of 2.5 mA, applied between the two mastoids for 30 s (electrically evoked vestibulo-ocular reflex [EVOR]), in one direction and the other, were quantifie…

This study aimed to develop a novel surgery classification for an endoscopic approach to middle ear cholesteatoma. We retrospectively analyzed the surgical approaches and outcomes of patients with middle ear cholesteatoma. Middle ear cholesteatoma surgeries were divided into four types and two special types as follows: type I, attic retraction pocket, which only requires tympanostomy tube placement or retraction pocket resection and cartilage reconstruction; type II, cholesteatoma which is limited to the attic or in which endoscopy can confirm complete removal of mastoid cholesteatoma lesions,…
