Ophthalmic injuries in patients with zygomaticomaxillary complex fractures requiring surgical repair.

PURPOSE: Injuries to the middle third of the face commonly destroy the integrity of the orbital skeleton, and are frequently complicated by injury to the eye, ranging between 2.7% and 90.6% in reported series. This article is a retrospecitve, descriptive case study assessing the spectrum and incidence of ophthalmic involvement in patients presenting with zygomaticomaxillary complex (ZMC) fractures. PATIENTS AND METHODS: Ninety-six patients with ZMC fractures who were surgically treated in 1 academic institution between 1996 and 2006 were assessed pre- and postoperatively by the same oculoplas

Blockage of vibrissae afferents: I. Motor effects.

In the past, it has been proposed that the rat vibrissae play an important role in other hand, postural abnormalities, muscle tone decreases and hypomotility after sensory organ destructions were proposed as evidence supporting the "level setting" or "tonic" hypothesis. This hypothesis postulates that afferent activity, besides its well know transductive functions, sets the excitability state of the central nervous system. We thought the vibrissal system to be a good model to dissect these two postulated roles because vibrissae trimming would annul the transductive function without affecting t

Results of transantral orbital decompression in 428 patients with severe Graves' ophthalmopathy.

We reviewed records from 428 consecutive patients with severe Graves' ophthalmopathy to determine early and late results after transantral orbital decompression. Optic neuropathy was present in 217 (50.7%) patients. Post-operatively, 402 (89%) of 453 eyes with preoperative visual acuity worse than 20/20 improved or remained the same. Visual field scotomas improved or resolved in 245 (91%) of 269 eyes tested pre- and postoperatively. Preoperative papilledema resolved or improved in 99 (94%) of 105 eyes, and preoperative exposure keratitis improved or resolved in 178 (92%) of 195 eyes. Average p

[Complications during and after third molar extraction].

A retrospective analysis of intra- and postoperative complications with special regard to infections should give information about the success rate of our antiseptic treatment procedure when removing third molars. 3,980 third molars were surgically removed between July 1, 1992 and June 30, 1999. Dates were sampled from the patient medical histories (963 men, 935 women) and statistically analyzed. Intraoral complications arose in 188 cases: 153 perforations (3.8%) of the maxillary sinus and additional 35 complications (0.9%) arose from ruptured tuber maxillae, intraoperative bleedings, dislocat

Primary yolk sac tumor of pterygopalatine fossa with loss of vision: A case report.

INTRODUCTION: Primary yolk sac tumor (YST) is an infrequently-diagnosed malignant extragonadal germ cell tumors. It is likely to recur locally and may present with widespread metastases once diagnosed. Primary YST of the head is uncommon but can cause severe complications, such as loss of vision once the tumor mass invades the optic nerve. PATIENT CONCERNS: A 20-month-old boy presented to the general clinic of the local children's hospital with a complaint of swelling of left face for 1 year and proptosis of the left eye for over 2 weeks as stated by his parents. Initially, he did have some v

Long-term CBCT evaluation of bone volume stability post dental implant placement: 1. Exploring sex as a risk factor.

OBJECTIVES: This retrospective study assesses the long-term effects of implants on alveolar and basal bone to determine if a patient's sex affects dimensional changes in the jaw and evaluate whether dental implants halt the atrophy of bone when compared to no post-extraction intervention. METHOD AND MATERIALS: Institutional Review Board approval was obtained for this study. Of the 125 patients (432 sites) in the study, 53% were women. All had two CBCTs taken, an average of 4.7 years apart. The scans were divided into three groups. In the control group, the extraction site (first CBCT, T1) was

[Nevus of Ota. Presentation of a case associated with a cellular blue nevus with suspected malignant degeneration and review of the literature].

BACKGROUND: Nevus of Ota is a melanotic pigmentary disorder ("dermal melanocytosis"), mostly congenital or acquired, involving the skin innervated by the first and the second branch of the trigeminal nerve, hence its descriptive label of "nevus fuscus coerulaeus ophthalmic and maxillaris". In more than half of patients this condition is associated with "ocular melanocytosis" ("melanosis oculi") involving the conjunctiva, the sclera, the uveal tract and possibly the optic nerve. In some cases a condition of "orbital melanocytosis" with involvement of orbital fat and periosteum by dendritic mela

Anatomic study of the infraorbital foramen for radiofrequency neurotomy of the infraorbital nerve.

OBJECTIVE: To examine the anatomy of the infraorbital canal and foramen and the angles at which a radiofrequency probe must be directed to enter the infraorbital foramen and canal, as a guide to performing radiofrequency ablation of the infraorbital nerve in patients with relative or absolute contraindications to lesions of the trigeminal ganglion or posterior root. METHODS: Eleven cadaveric skulls were studied. The infraorbital nerve, after passing through the infraorbital foramen, enters the infraorbital canal and groove in the floor of the orbit before reaching the foramen rotundum. Small

Does Dexamethasone Facilitate Neurosensory Function Regeneration After Zygomatic Fracture? A Randomized Controlled Trial.

PURPOSE: This study sought to clarify the rate of neurosensory disturbance (NSD) after zygomatic complex fractures in general, as well as the effect of perioperatively administered dexamethasone on neurosensory recovery. PATIENTS AND METHODS: This was a single-blinded randomized study aiming to clarify the benefits of perioperative dexamethasone after surgery. The patients were randomly assigned either to receive dexamethasone (up to a total dose of 10 or 30 mg) or to act as control patients (no glucocorticoid treatment). The outcome variable was NSD, the presence of which was established whe

Craniofacial fibrous dysplasia of orbit.

Fibrous dysplasia is a benign, rare, and idiopathic skeletal disorder characterized by replacement and expansion of medullary bone by disorganized fibro-osseous tissue. Treatment for these lesions is usually sought for cosmetic or functional reasons. The surgical approaches vary from watchful expectancy for the lesion to stop growing after puberty to conservative resection to radical resection. In this case reports, 3 patients with craniofacial fibrous dysplasia involving orbital region underwent surgery. In these primary cases, resection and immediate reconstruction were performed to regain f

[Wegener's granulomatosis with meningeal involvement: clinic-radiological correlations].

INTRODUCTION: Wegener's granulomatosis is a systemic vasculitis which, in its classical form, is characterized by involvement of the superior and inferior respiratory tract and the kidneys. The vasculitis may be multisystemic. Ophthalmic and neurological involvement are common (22% and 54% of those affected respectively). When considering involvement of the nervous system, the commonest finding is peripheral neuropathy, particularly in the form of multiple mononeuritis. Meningeal involvement is exceptional. CLINICAL CASE AND CONCLUSIONS: We present a case of Wegener's granulomatosis with meni

Effect of a platform switch and a laser-microgrooved coronal design on short dental implants: A multicenter retrospective study.

BACKGROUND: The presence of severe atrophy in proximity to the maxillary sinus or the inferior alveolar nerve is a primary indication for the use of short dental implants. The placement of standard-length implants in such cases often requires complex regenerative procedures, leading to prolonged healing time and an increased invasiveness of the surgical procedure. OBJECTIVES: The aim of the study was to evaluate crestal bone remodeling, implant success, complications, and implantand patient-related risk factors associated with the use of short, platform-switched dental implants with a laser-m

Trigeminocardiac reflex in bimaxillary orthognathic surgery: case review.

BACKGROUND: The trigeminocardiac reflex (TCR) is a rare but clinically significant phenomenon characterized by bradycardia, hypotension, or asystole triggered by trigeminal nerve stimulation during maxillofacial surgery. It necessitates prompt recognition and management to ensure patient safety. TCR has been reported in orthognathic surgery, particularly during specific surgical maneuvers. MATERIAL AND METHODS: We report the case of a 36-year-old male who experienced TCR during bimaxillary orthognathic surgery. Detailed documentation of the patient's clinical characteristics, intraoperative e
