Sinus lift procedures: an overview of current techniques.

For more than 30 years the maxillary sinus augmentation graft has been a mainstay of implant-directed maxillary reconstruction. The purpose of this article is to review the fundamentals of maxillary sinus reconstruction including anatomy and physiology of the sinus, indications for surgery, preoperative evaluation, surgical techniques, and management of complications. While there are some relative contraindications for the procedure, there are almost no absolute contraindications. With preparation, education, and experience, the maxillary sinus augmentation/elevation graft is a procedure that

Use of piezosurgery device in management of surgical dental implant complication: a case report.

An implant displacement into the maxillary sinus could be a complication of implant surgery in the upper jaw. In such cases, implant removal is needed to avoid the occurrence of sinus pathologies. Piezosurgery techniques could assure safer management of such complications because of clear surgical visibility and a selective ability to cut. The aim of this report is to present a case of an implant-related oral surgery complication that was resolved by means of a piezosurgery technique.

1Unilateral Le Fort I Advancement Versus Dentoalveolar Transport Distraction in Patients With Large Alveolar Clefts: Protocol for a Prospective Observational Study.

BACKGROUND: Cleft lip and palate is a complex congenital defect that can present functional and esthetic challenges, particularly in patients with large alveolar clefts for which surgical management is indicated. In contrast to traditional treatment techniques such as autologous bone grafting, newer techniques include unilateral Le Fort I osteotomy advancement and dentoalveolar transport distraction osteogenesis and have been shown to be useful in the treatment of maxillary hypoplasia and large alveolar clefts. These techniques may help improve both esthetic and function in patients with subst

Does preoperative joint space affect accuracy in splint-based Le Fort I osteotomy?

In splint-based Le Fort I osteotomy (LFI), variations in condyle-fossa relationship can influence surgical accuracy. The aim of this study was to investigate the impact of the preoperative joint space (JS) on surgical errors. This retrospective study included patients status post splint-based maxilla-first LFI at a single institution in a 7-month period. Preoperative JS was measured on preoperative cone-beam computed tomography (CBCT), while angular and linear surgical movements at the maxillary dental midline (U1) were measured using regional voxel-based registration. In 26 eligible subjects,

A Modified Intrabony Defect Model of Periodontitis in Rat.

OBJECTIVE: The aim of this study was to compare the radiographic depth of the periodontal bone defect, as well as clinical and histological outcomes, between the proposed ligated defect model and the current non-ligated defect model, which often heals spontaneously and fails to replicate the complexities of periodontitis. METHODS: The proposed model combined osteotomy to create controlled periodontal bone defects (depth of 1.5 mm) and ligature placement around the cervical region of the maxillary first molar to induce inflammation. Micro-CT analysis, clinical examinations, histological evalua

Successful management of recurrent adenoid cystic carcinoma in the deep infratemporal fossa by maxillo-orbito-zygomatic approach.

OBJECTIVE: In the case of deep invasion of an infratemporal fossa (ITF) tumor, surgeons find it difficult to gain sufficient visualization and working space by conventional surgical approaches. To overcome these limitations, we have developed a novel surgical technique, maxillo-orbito-zygomatic (MOZ) approach, by combining partial lateral maxillectomy with the conventional orbito-zygomatic approach. METHODS: A 63-year-old male presented with the fifth recurrent adenoid-cystic carcinoma in the right deep ITF. Using a Weber-Ferguson-type incision and partial dismasking, we elevated the skin and

Inferior subapical osteotomy for dentoalveolar decompensation of class III malocclusion in 'surgery-first' and 'surgery-early' orthognathic treatment.

Increasing experience with alternative timing protocols in orthognathic surgery has given way to new surgical and orthodontic techniques to shorten treatment times, reduce biological costs, and improve the final outcome. A prospective evaluation of class III patients who received an inferior segmental osteotomy (ISO) for decompensation of significantly retroclined lower incisors in the context of 'surgery-first' (SF) or 'surgery-early' (SE) timing protocols was performed. Treatment was planned virtually. A thorough periodontal assessment was performed at baseline and periodically until debondi

Human histologic evaluation of the use of the dental putty for bone formation in the maxillary sinus: case series.

A proof-of-principle study was conducted to assess the safety and efficacy of dental putty as an alternative sinus augmentation biomaterial. Six healthy patients requiring a total of 10 sinus augmentations received sinus augmentations. All patients volunteered and signed an informed consent based on the Helsinki declaration of 1975, as revised in 2000. The sinus augmentation was performed under local anesthesia with a mucoperiosteal flap elevated to expose the buccal wall of the maxillary sinus. The space was then filled with the dental putty in several increments, and the window was covered w

Clinical and Radiographic Outcomes of Modified Multi-Tooth Posterior Maxillary Alveolar Segment Infracture Without a Lateral Window for Staged Implant Placement: A Prospective Case Series.

Background/Objectives: This prospective case series evaluated whether modified multi-tooth posterior maxillary alveolar segment infracture without a lateral window permitted staged implant placement. Methods: Of 53 patients assessed, 8 were excluded and 25 declined; 20 eligible consenting non-smokers were enrolled consecutively and treated at 26 sites. Following three adjacent extractions, a single bone block containing the extraction sockets was mobilized through an oral-side oval osteotomy, displaced superiorly, and augmented. Cone-beam computed tomography was obtained before augmentation, a

Does simultaneous alloplastic temporomandibular joint reconstruction affect the accuracy of Le Fort I osteotomy?

Simultaneous alloplastic temporomandibular joint reconstruction (ATMJR) with Le Fort I osteotomy (LFI) may be indicated when significant temporomandibular joint (TMJ) pathology accompanies dentofacial deformities. Currently, the impact of concomitant ATMJR on LFI accuracy is not known. The purpose of this study was to assess the influence of patient-fitted ATMJR on the accuracy of simultaneous LFI. In this retrospective cohort study, patients undergoing LFI with and without ATMJR were enrolled. Preoperative and postoperative cone-beam computed tomography was analyzed using regional voxel-based

Evaluation of Osteogenic Effect Following Maxillary Sinus Floor Elevation by Analyzing Trabecular Bone Parameters: A Retrospective Study.

BACKGROUND: Maxillary sinus floor elevation (MSFE) requires reliable graft materials to ensure adequate bone regeneration. Bio-Oss Collagen and Bio-Oss granules are widely used xenografts, but evidence comparing their osteogenic performance remains limited. OBJECTIVE: This study evaluated whether collagen addition influences early bone height gain or trabecular bone quality after MSFE. METHODS: Twenty-four patients undergoing conventional lateral window osteotomy for sinus floor elevation with simultaneous implant placement were retrospectively analyzed. According to graft type, cases were a

Expansion of donor-unrestricted MAIT cells with enhanced cytolytic function suitable for TCR redirection.

Objectives Intraoperative vagus nerve monitoring during skull base surgery has been reported with the use of an oral nerve monitoring endotracheal tube. However, the intraoral presence of an endotracheal tube can limit exposure by its location in the operative field during transfacial approaches and by limiting superior mobilization of the mandible during transcervical approaches. We describe a transnasal vagus nerve monitoring technique. Design and Participants Ten patients underwent open skull base surgery. Surgical approaches included transcervical (five), transfacial/maxillary swing (three

Surface electromyography before and after orthognathic surgery and condylectomy in active laterognathia: a case report.

BACKGROUND: Condylar hyperplasia is a rare bone disease characterised by excessive development of mandibular condyle, which can lead to the development of asymmetric facial deformity together with malocclusion, mandibular deviation, TMJ and masticatory musculoskeletal system dysfunction. There is not a treatment protocol universally accepted. In order to determine the correct management, treatment and intervention timing of these patients, morphological examinations should be coupled with functional assessments. CASE REPORT: In the present case report, morphological (bone scintigraphy; orthop

Simultaneous sinus lifting and alveolar distraction of a severely atrophic posterior maxilla for oral rehabilitation with dental implants.

We retrospectively reviewed a new preimplantation regenerative augmentation technique for a severely atrophic posterior maxilla using sinus lifting with simultaneous alveolar distraction, together with long-term oral rehabilitation with implants. We also analyzed the regenerated bone histomorphologically. This study included 25 maxillary sinus sites in 17 patients. The technique consisted of alveolar osteotomy combined with simultaneous sinus lifting. After sufficient sinus lifting, a track-type vertical alveolar distractor was placed. Following a latent period, patient self-distraction was st
