The kinematics of five fresh frozen wrist specimens were studied before and after a simulated scaphoid waist fracture. To determine the change in wrist motion and fracture site characteristics associated with an unstable wrist, the relative motion of each carpal bone was determined from the movement of implanted carpal markers on biplanar radiographs obtained in neutral and the four extreme wrist positions. A questionnaire survey of motion sickness occurrence on board passenger ferries has been conducted. Data were collected from 20,029 passengers on 114 voyages on 9 vessels: 6 ships, 2 hovercraft, and 1 jetfoil. Information was obtained about feelings of illness, the occurrence of vomiting, the taking of anti-seasickness tablets, the consumption of alcoholic drinks, regularity of travel by sea, age, and sex. Although the trend in management of tibial plateau fractures has been toward early motion, the period of immobilization that can be tolerated safely is open to question. Investigations were performed with 19 healthy male volunteers to specify a possible role of endogenous opioid peptides in the pathogenesis of motion sickness. For this purpose the test subjects were administered naloxone, a specific antagonist of opiates and opioids, before rotation and during rotation in a BU-4 armchair at a rate of 30 rpm. The appropriateness of kaolin consumption, one form of pica, as an index of motion sickness in the rat was examined. Unlike other motion sickness indices, the use of kaolin consumption results in a bitonic function across daily rotation sessions. Flexion-extension motion and compression-distraction forces across the cervical spine were evaluated in 6 patients in halo vests and in 1 patient in a halo cast. Motion in the vest was evaluated in supine and upright positions and averaged 31% of normal motion at the levels tested. Compression-distraction forces showed great individual variation and frequent compression. The average distraction force varied in different positions by nearly 20 pounds in the vest and over 30 pounds in the cast. Methods of improving cervical spine stability in halo orthoses are discussed.
