Splenectomy followed by bone marrow transplantation (BMT) has been applied successfully in the treatment of neuronopathic Gaucher disease (GD). GD in combination with schizencephaly has not previously been reported. We describe a girl who presented with hemiparesis and oculomotor apraxia since infancy, and thereafter developed progressive anemia, thrombocytopenia, hepatosplenomegaly, psychomotor retardation, and skeletal abnormalities. GD caused by homozygosity of the L444P mutation was diagnosed, in combination with schizencephaly. The child received enzyme replacement therapy for 3 years, fo

We review the cardiac abnormalities in 94 patients (27 new, 67 literature) with Costello syndrome, an increasingly recognized syndrome consisting of increased birth weight, postnatal growth retardation, and distinctive facial, skin, and musculoskeletal features (MIM 218040). A cardiac abnormality was found in 59 (63%) patients, with each of three categories occurring in approximately one-third of patients. A cardiovascular malformation (CVM) was noted in 30%, typically pulmonic stenosis (46% of those with a CVM). Cardiac hypertrophy was reported in 34%, which involved the left ventricle in 50%

Hypercalcemia is a relatively common metabolic abnormality; however, it is frequently undiagnosed. It often manifests with neuropsychiatric, gastrointestinal, renal, and musculoskeletal symptoms. We report the case of a 75-year-old woman with a history of syncope, anorexia, asthenia, weight loss, polydipsia, polyuria, visual hallucinations, confusion, generalized tonic-clonic seizures, and depression. Laboratory workup revealed severe hypercalcemia (ionized calcium: 1.81 mmol/L) with normal renal function and significantly elevated parathyroid hormone (PTH) levels. A diagnosis of primary hyper

MR imaging of the hip is one of the most common musculoskeletal MR imaging studies performed today to assess for occult fractures, acetabular labral tears, hyaline cartilage loss, and musculotendinous injuries. Several developmental variations are seen in the hip, which can be mistaken for disease or potentially even contribute to the development of a pathologic condition. As in any imaging study, it is important to be cognizant of these variations as well as associated findings that help distinguish between true abnormality and developmental variation when interpreting an MR image of the hip.

The calcineurin inhibitors (CIs) cyclosporine A and tacrolimus are essential for graft-versus-host disease prophylaxis but are associated with adverse effects, including neurotoxicity. We report a case of irreversible CI-induced neuropathic pain following allogeneic hematopoietic stem cell transplantation. The patient developed dysesthesia, electric shock-like pain, and severe itching followed by intractable analgesic-resistant pain in the lower extremities. There were no abnormal radiographic findings, and there was no improvement with a reduction of CI dosage or with administration of a calc

BACKGROUND: Congenital contractures of the limbs and face, hypotonia, and developmental delay (CLIFAHDD) syndrome (OMIM #616266) is an autosomal dominant hereditary disease that can lead to the congenital contracture of the limbs and face, hypotonia, and developmental delay. In addition, it may result in growth retardation and present various clinical symptoms, such as brain atrophy, a small pituitary gland, musculoskeletal abnormalities, abnormal breathing, abdominal hernia, and abnormal facial features. Herein, we describe a novel de novo missense genetic variant in the sodium leak channel, …

US has emerged as a valuable imaging tool for evaluating musculoskeletal abnormalities, including radial wrist conditions. It offers high-resolution real-time assessment of joints, ligaments, tendons, nerves, and other soft-tissue structures in the region. Yet, the complex anatomy of the wrist, frequent anatomic variants, and often nonspecific clinical symptoms present substantial challenges for the clinician and the US practitioner alike. This comprehensive review provides a structured and practical approach to US evaluation of the radial wrist, designed to enhance diagnostic confidence and a…

We aimed to describe adverse pregnancy outcomes among women who had symptomatic, RT-PCR-confirmed ZIKV infection and early childhood outcomes among their infants. We enrolled pregnant women with symptomatic, RT-PCR-confirmed ZIKV infection in a prospective cohort study, and their infants in a prospective pediatric cohort study. We defined adverse pregnancy and early childhood outcomes based on selected neurologic, ophthalmologic, auditory, musculoskeletal, and anthropometric abnormalities. We used RT-PCR and serologic tests to determine the ZIKV infection status of the child. Between 10 March

BACKGROUND: The presence of a single umbilical artery (SUA) is a fetal soft marker of congenital abnormalities. Among the most common related malformations, there are cardiological, nephrourological and digestive anomalies, most of which are considered to have a vascular etiology. There is an association between increased incidence of intrauterine growth retardation and adverse perinatal indicators, but whether this association is due to related anomalies or isolated SUA (iSUA) is controvisal.
METHODS: We reviewed 96 cases of iSUA and non-isolated SUA (niSUA), diagnosed in a period of two yea

Wild-type transthyretin amyloidosis (ATTRwt) is an age-associated protein misfolding disorder marked by extracellular deposition of transthyretin fibrils within the myocardium, peripheral nerves, lungs, blood vessels, and musculoskeletal tissues. ATTRwt typically presents as a restrictive cardiomyopathy with preserved left ventricular ejection fraction that contributes to the development of hallmark symptoms such as exercise intolerance. Amyloid infiltration severely impairs ventricular compliance and diastolic reserve, limiting stroke volume and cardiac output augmentation during exercise, an

Musculoskeletal ultrasound (MSUS) has become a cornerstone imaging modality in pediatric rheumatology, providing a safe, non-invasive, and highly sensitive method for assessing musculoskeletal pathology. Reliable interpretation of MSUS requires awareness of developmental anatomy, optimization of technical parameters, and recognition of common artifacts. These considerations are especially important in children, where physiological vascularization and age-related changes in cartilage or bone can mimic pathology. MSUS enables the detailed visualization of synovitis, tenosynovitis, enthesitis, an

BACKGROUND: Systemic lupus erythematosus (SLE) is an autoimmune disease characterized by diverse symptoms and multisystem involvement. Pediatric-onset systemic lupus erythematosus (pSLE) accounts for 10-20% of all SLE cases and tends to progress more rapidly and severely than adult-onset SLE, making diagnosis more challenging. Current biomarkers remain insufficient in predicting disease severity and organ-specific complications, highlighting the need for new biomarkers.
METHODS: This study included 36 pSLE patients and 18 age and gender matched healthy controls. Serum CCL22 levels were measur

BACKGROUND: Assessment of individuals at risk for falling entails comprehensive neurological and vestibular examinations. Chronic limitation in cervical mobility reduces gaze accuracy, potentially impairing navigation through complex visual environments. Additionally, humans with scoliosis have altered otolithic vestibular responses, causing imbalance. We sought to determine whether dynamic cervical mobility restrictions or static cervicothoracic impairments are also fall risk factors.
METHODS: We examined 435 patients referred for soft-tissue musculoskeletal complaints; 376 met criteria for

Weill-Marchesani syndrome (WMS) is a rare genetic disorder that affects the musculoskeletal system, the eye, and the cardiovascular system. Individuals with WMS present with short stature, joint contractures, thick skin, microspherophakia, small and dislocated lenses, and cardiac valve anomalies. WMS can be caused by recessive mutations in ADAMTS10 (WMS 1), ADAMTS17 (WMS 4), or LTBP2 (WMS 3), or by dominant mutations in fibrillin-1 (FBN1) (WMS 2); all genes encode secreted extracellular matrix (ECM) proteins. Individuals with WMS 4 due to ADAMTS17 mutations appear to have less severe cardiac i

Infants with limb reduction deficiencies (LRD) often have other associated congenital anomalies. The purpose of this investigation was to assess the prevalence and the types of associated anomalies in a defined population. The associated anomalies in infants with LRD were collected in all livebirths, stillbirths and terminations of pregnancy during 29 years in 387,067 consecutive births in the area covered by our population-based registry of congenital malformations. Of the 317 infants bom with LRD during this period, representing a prevalence of 8.2 per 10,000, 59.9% had associated anomalies.
