A case of bacteremic pneumococcal pneumonia associated with rhabdomyolysis and myoglobinuric renal failure is presented. This may be the first report of the association of these two disorders, and may influence the choice of antibiotic selection in patients with rhabdomyolysis and a pulmonary infiltrate.

Carnitine palmitoyltransferase (EC 2.3.1.21) was studied in sonicated muscle homogenates of seven patients who had recurrent attacks of myoglobinuria and marked deficiency of carnitine palmitoyltransferase in the isotope exchange assay, and in control subjects. When L-palmitoylcarnitine was reduced from 0.5 mM to 0.05 mM in the isotope exchange assay, enzyme activity returned to normal in the patients but was not significantly altered in the controls. When the forward assay was performed in the presence of 80 microM palmitoyl-CoA and 0.1% albumin, all patients showed normal carnitine palmitoyl…

1 Two cases of drug-induced rhabdomyolysis, one of them presenting with acute myoglobinuric renal failure are described. 2 The pathogenesis of rhabdomyolysis and renal failure in our cases is discussed with reference to work published by other authors. 3 Whereas there seems to be obvious evidence for direct toxic drug-muscle interaction, the mechanism for the development of myoglobinuric renal failure remains unclear.

Three cases of arterial hypertension and severe hypokaliemia are reported; in two cases an acute rhabdomyolisis was also observed. The syndrome was iatrogenic in two cases depending on the licorice eating and the combined use of diuretics and a 9-alpha-fluoroprednisolone-containing nasal spray in one patient, and on a long-term use of the same nasal decongestant in the other. In the patient 2 the persistence of low PRA and aldosterone levels seems to indicate an excess of mineralcorticoid activity other than aldosterone. The syndrome of factitious mineralcorticoid excess should, therefore, be

Rhabdomyolysis occurred in 25 of 1,000 patients (2.5%) with phencyclidine (PCP) intoxication. 10 of these 25 patients (40%) developed acute renal failure and another 7 had mild impairment in renal function. Marked hyperuricemia was present in all 17 patients, and marked hyperphosphatemia and hypocalcemia were noted in the 10 patients with acute renal failure; 3 of the latter developed hypercalcemia during the diuretic phase of the illness. Fever, tachycardia and hypertension were frequent findings among the 25 patients with rhabdomyolysis and all had leukocytosis. The data show that rhabdomyol

Four school children, aged 6 to 9 years, had acute postinfectious myositis. The prodromal illness usually involved the upper respiratory tract, but gastrointestinal symptoms were also seen. Fever and nonspecific malaise were characteristic. After cessation of the illness, myalgia involved the calves and thighs. Arm and neck muscles were less frequently affected. Weakness was less marked than muscle pain. Serum creatine phosphokinase (CPK) was markedly increased in all cases. Myalgia and CPK levels subsided in less than a week, although one child was not back to normal for 4 weeks. Two children

A column flow-through radioimmune assay for serum myoglobin has been developed. The assay uses antibodies to human or rhesus monkey myoglobin coupled to Sepharose by cyanogen bromide activation and myoglobin labeled by the Chloramine-T method. The labeled myoglobin is stable over two to three half-lives. Serum myoglobin levels were elevated when serum was taken up to 12 hours after myocardial infarction and generally elevated up to 24 hours. A continued or repeated elevation of serum myoglobin probably indicates continued myocardial damage or new attacks. In the cases of myocardial infarction,

Rhabdomyolysis is a potentially life-threatening condition caused by acute skeletal muscle injury leading to myocyte destruction and release … and acute kidney injury. Urinalysis showed heme positivity without red blood cells, consistent with myoglobinuria. MRI of the thighs demonstrated muscle edema. Common precipitants, including trauma, exertion, infection, toxins, thyroid disease, and statin-associated antibody-mediated myopathy, were excluded by history, serology, and laboratory workup. Despite n …

BACKGROUND: Renal involvement is rarely reported in juvenile dermatomyositis and may be caused by the toxic effects of myoglobinuria or an autoimmune reaction. We report a case of dermatomyositis and nephrotic syndrome in a child to explore the association between juvenile dermatomyositis and renal involvement.
CASE PRESENTATION: An 8-year-old girl with skin rash, edema, proximal muscle weakness predominantly involving the lower extremities, low-grade fever, and foamy urine was admitted to our hospital. Her laboratory tests met the criteria of nephrotic syndrome. She had elevated creatine kin…
