Comparison of Ultrasound and MRI for Measuring Foramen Magnum Diameter in Neonates and Infants: A Preliminary Study.

BACKGROUND: The foramen magnum is the largest opening at the skull base. Congenital skeletal disorders such as achondroplasia can cause foramen magnum stenosis, leading to neurological problems. In high-risk infants, repeated magnetic resonance imaging (MRI) is often used to screen for or monitor stenosis. Ultrasound (US), already used for neonatal spinal assessment, may offer a non-invasive method for evaluating the craniovertebral junction. However, to our knowledge, no studies have directly compared foramen magnum assessment between US and MRI. METHODS: We retrospectively reviewed neonates

Imaging and Non-Imaging Approaches for the Diagnosis and Monitoring of Necrotizing Enterocolitis-What Lies Ahead?

Necrotizing enterocolitis (NEC) remains one of the most serious gastrointestinal emergencies in preterm infants, and imaging plays a central role in diagnosis and clinical management. Historically, evaluation has relied primarily on abdominal radiography, which remains widely available and embedded in established diagnostic frameworks. However, the hallmark radiographic signs of NEC (i.e., pneumatosis intestinalis, portal venous gas, and free air) reflect relatively advanced manifestations of intestinal injury that indicate established mucosal disruption or transmural necrosis. Bowel ultrasoun

Case Report: Holistic approach to management of an infant with severe osteogenesis imperfecta in the neonatal intensive care unit.

BACKGROUND: Osteogenesis Imperfecta (OI) Type II is the most severe and often lethal form of OI, characterized by profound skeletal fragility and perinatal complications, most notably respiratory failure secondary to thoracic deformities and pulmonary hypoplasia. Effective NICU management requires a holistic, family-centered approach combined with comprehensive medical care, and can be particularly challenging in centers with limited experience managing similar conditions. This is further complicated by ethical dilemmas, including decisions regarding the extent of interventions requiring nuanc

Umbilical venous catheter- and peripherally inserted central catheter-associated complications in preterm infants with birth weight < 1250 g : Results from a survey in Austria and Germany.

BACKGROUND AND OBJECTIVE: Umbilical venous catheters (UVC) and peripherally inserted central catheters (PICC) are commonly used in preterm infants but have been associated with a number of serious complications. We performed a survey in Austria and Germany to assess the use of UVCs and PICCs in preterm infants with a birth weight < 1250 g and associated rates of catheter-related adverse events. METHODS: Electronic survey of participating centers of the NeoVitaA trial. Main outcome parameter was the reported rates of UVC- and PICC-associated complications (infection, thrombosis, emboli, organ

[Medical termination of pregnancy for isolated intrauterine growth retardation beyond 24 weeks and more than 450g].

INTRODUCTION: Fetal growth restriction (FGR) is an obstetric complication responsible for increased perinatal morbidity and mortality. In some severe and early FGR situations, termination of pregnancy (TOP) may be considered. The main objective of our study was to describe the population of fetuses for whom a TOP was performed for isolated FGR beyond 24 days' gestation and for a birth weight>450g and to analyze the immediate outcome, at 2 and 5 years, of term- and weight-matched neonates born in a context of severe FGR after 24 weeks' gestation and over 450g. MATERIAL AND METHODS: We conducte

[Pathologic ophthalmologic changes in prematurity].

The personal author's experiences has been presented, with regards to the early discovering and adequate preventing of ophthalmology diseases at prematurely born children. Thanks to the development of perinatology, almost every organ can be monitored and functionally examined even before the child is born. Despite all problems, the work of ophthalmo-paediatritians is extremely challenging, because that is the only situation in which embryology of the eye can be seen "In Vitro" and in which the physiological development of the eye's function can been monitored. During the period from 1999 to 20

Emerging opportunities in ROP therapy: from inhibiting pathological vessel growth to promoting physiological vascularization.

Despite continuous advances in the care of preterm infants, therapeutic management of ROP has made limited progress in recent years and remains a source of frustration for neonatologists. The current approach largely relies on the spontaneous resolution of the disease, limiting clinical intervention to observation and monitoring, with little capacity to significantly intervene on the pathological progression of retinal vascularization. Similarly, ophthalmologists often adopt a watchful waiting strategy, with invasive treatments reserved only for preventing imminent retinal detachment. As a res…

Infant flow biphasic nasal continuous positive airway pressure (BP- NCPAP) vs. infant flow NCPAP for the facilitation of extubation in infants' ≤ 1,250 grams: a randomized controlled trial.

BACKGROUND: The use of mechanical ventilation is associated with lung injury in preterm infants and therefore the goal is to avoid or minimize its use. To date there is very little consensus on what is considered the "best non-invasive ventilation mode" to be used post-extubation. The objective of this study was to compare the effectiveness of biphasic nasal continuous positive airway pressure (BP-NCPAP) vs. NCPAP in facilitating sustained extubation in infants ≤ 1,250 grams. METHODS: We performed a randomized controlled trial of BP-NCPAP vs. NCPAP in infants ≤ 1,250 grams extubated for the f

An analysis of birth weight by gestational age using a computerized perinatal data base, 1975-1992.

OBJECTIVE: To develop birth weight-for-gestational age nomograms based on a computerized perinatal data base collected prospectively from 1975-1992. METHODS: Using information from over 60,000 singleton deliveries (January 1975 through October 1992) at the MetroHealth Medical Center in Cleveland, Ohio, standard curves for normal birth weights were computed. Nomograms were developed for the overall population and for subgroups determined by factors known to affect fetal growth, including sex, race, smoking status, and gestational diabetes. The nomograms included the tenth, 50th, and 90th perce

Outcomes of patent foramen ovale greater than 3 mm at birth in extremely low birthweight infants.

BACKGROUND: Foramen ovale (FO) is an obligate fetal shunt that typically resolves after birth, although patency throughout life is not uncommon. The natural history of patent FO (PFO) is known in term infants, but less is known about its course in extremely preterm infants. We describe the echocardiographic changes in FO size from birth to discharge in extremely low birth weight (ELBW) infants in this retrospective study. METHODS: Cohort was divided based on size of FO at birth. Size of FO at discharge was measured and evaluated relative to postnatal weight gain. Demographics and clinical out

A primary lesion of advanced melanoma in pregnancy: case report and review of literature of the advanced cases in the last ten years.

INTRODUCTION: Pregnancy- associated melanoma (PAM) is reported between 2.8 and 5.0 per 100,000 pregnancies and approximately 35% of women with melanoma are of childbearing age. The diagnosis and treatment of melanoma during pregnancy needs a balance of risks and benefits for both maternal and fetal well-being. It is a type of malignancy, which requires a multidisciplinary approach, not limited to the obstetrician, but also to oncologists, neonatologists, pharmacists and psychologists. MATERIAL AND METHODS: We present a case of a 36-year-old pregnant woman, who was admitted to our 3rd level Un…
