The immune microenviroment in somatotropinomas: from biology to personalized and target therapy.

Pituitary tumors are rare neoplasms, with a heterogeneous biological and clinical behavior, due to their clinical course, local invasive growth, resistance to conventional therapies and the risk of disease progression. Recent studies on tumor microenvironment (TME) provided new knowledge on the biology of these neoplasia, that may explain the different phenotypes of these tumors and suggest new biomarkers able to predict the prognosis and the treatment outcome. The identification of molecular markers that act as targets for biological therapies may open new perspectives in the medical treatmen

Granular cell tumor of the bronchus: A case report.

INTRODUCTION AND IMPORTANCE: Pulmonary granular cell tumor (GCT) is a very rare neoplasm that originates from Schwann cells. CASE PRESENTATION: Our case report describes a symptomatic benign pulmonary GCT found during follow-up CT imaging and was eventually excised with a good outcome. CLINICAL DISCUSSION: GCT are mostly benign tumors. Pulmonary GCTs mostly occur in the endobronchial region and can be symptomatic or discovered incidentally. CONCLUSION: Treatment options for pulmonary GCT are either conservative treatment or surgical resection depending on the size of the tumor and the prese

An intra-abdominal desmoid tumor.

Desmoid tumor is a very rare neoplasm which develops from fibroblasts. These tumors do not have the ability to metastasize, but they can cause significant morbidity and mortality by local invasion and they are prone to local recurrence. We present a case of an aggressive fibromatosis in a 28-year-old male patient with no previous medical history. The tumor was in the retroperitoneum and eventually caused perforation of the coecum. During the operation, no metastasis was found; however, local lymphadenopathy was seen. After the surgical resection, no adjuvant therapy (radio or chemotherapy) was

Pathologist-performed palpation-guided fine needle aspiration cytology of lingual actinomycosis: A case report and review of literature.

"Lingual actinomycosis" is an uncommon, suppurative inflammation of lingual mucosa, caused by Actinomyces. Infectious oral lesions can mimic benign or malignant neoplasms and fine needle aspiration cytology (FNAC) is a simple, minimally invasive procedure for the assessment of patients with such lesions. Here, we describe the case of a 28-year old patient presented with an asymptomatic, submucosal nodular mass of the tongue. Then, FNAC was performed by an experienced fine needle aspiration (FNA) pathologist (* ) in our pathology department and the lesion diagnosed with actinomycosis. We also r

Advances in thymic carcinoma diagnosis and treatment: a review of literature.

Thymomas account for up to 50 % of anterior mediastinal neoplasms with an incidence of 0.13 per 100,000 person-years in the USA. Thymic carcinoma is a rare malignancy of the thymus gland distinguished from thymomas as it has a more invasive and metastasizing potential conferring poor prognosis. Due to the rarity of thymic carcinoma and the great variety of its histological subtypes, there is no solid evidence on optimal staging, imaging and treatment guidelines. Herein, we systematically review the literature on current clinical practice with regard to diagnostic evaluation, histopathological

A rare cutaneous presentation of metastatic parotid adenocarcinoma.

Primary adenocarcinomas of the parotid gland are rare, accounting for < 5% of all head and neck malignant neoplasms. The biological behaviour of these tumours varies considerably. Low-grade tumours are minimally invasive, whereas high-grade tumours show a high incidence of local recurrence and distant metastases. We report a case of metastatic parotid adenocarcinoma which presented with cutaneous features. This case illustrates that such salivary gland malignancies can very rarely present to the dermatologist. These potentially aggressive tumours require prompt diagnosis and management with mu

Unusual presentation of adenoid cystic carcinoma of the maxillary antrum.

Unusual presentation of adenoid cystic carcinoma of the maxillary antrum. Adenoid cystic carcinoma (ACC) is a rare tumour which affects mainly the major and accessory salivary glands. It is an aggressive neoplasm characterized by early neural invasion and a high incidence of local recurrence and distant metastases, which may develop years after the initial resection. Surgery followed by radiotherapy seems to be the best treatment. Based on a well-illustrated case of extensive maxillary ACC involving the palate, orbit floor, and pterygo-palatine fossa, we discuss the prognosis and the importanc

Small cell carcinoma of the uterine corpus: MR imaging and pathological correlation.

Small cell carcinoma of the uterine corpus is a rare but aggressive neoplasm showing neuroendocrine differentiation; its radiological findings are not well described. We report the magnetic resonance features of 3 cases with pathological correlation. Small cell carcinoma is shown as a bulky endometrial tumor of heterogeneous appearance on T2-weighted images, frequently associated with diffuse myometrium invasion and early extrauterine spread. Irregular enhancement with multifocal unenhanced areas of the thickened myometrium on contrast-enhanced T1-weighted images may represent the infiltrative

[Ductal carcinoma of the parotid gland with the clinical picture of obstructive sialadenitis].

Salivary duct carcinoma is a highly malignant tumor. Pain, facial nerve palsy, perineural and lymphatic invasion are common findings, as well as an extensive cervical lymph node involvement and distant metastasis. However, occasionally, as in the case presented, the clinical features of this tumor may resemble obstructive sialadenitis and thus cause a delay in appropriate treatment. Although salivary duct carcinoma exhibits an unpredictable clinical course and a poor overall prognosis, total parotidectomy with neck dissection and adjunctive radiation therapy appear to be appropriate for local

Cauda equina syndrome caused by primary and metastatic neoplasms.

Cauda equina syndrome (CES) is defined as the constellation of symptoms that includes low-back pain, sciatica, saddle anesthesia, decreased rectal tone and perineal reflexes, bowel and bladder dysfunction, and variable amounts of lower-extremity weakness. There are several causes of this syndrome including trauma, central disc protrusion, hemorrhage, and neoplastic invasion. In this manuscript the authors reviewed CES in the setting of both primary and secondary neoplasms. They examined the various primary tumor types in this region as well as those representative of metastatic spread. Both su

Squamous cell cancer of the oesophagus.

Squamous cell cancer is the most common neoplasm of the oesophagus worldwide, with an enormous variation in its global incidence. Several risk factors, such as achalasia, Plummer-Vinson syndrome, coeliac disease and nutritional factors, have been identified. The surveillance of patients, especially those with tylosis or caustic ingestion, has been recommended. Vital staining with iodine may improve the diagnosis of early cancer. The endoscopic management of early cancer and dysplasia by minimal invasive techniques such as photodynamic therapy or mucosal resection has become attractive for many

[The morphogenesis of cancer of the prostate (immunohistochemical research)].

With the purpose of establishing morphogenetic features of precancer and early cancer of the prostate gland a comparative immunomorphological evaluation was done of the prostate tumor markers (PAP, PSA, PCNA, and 34 beta E12). Due emphasis is given to the part the basal cell dysplasia plays in morphogenesis of prostatic cancer. The precancer lesions of the prostate include atypical adenomatous hyperplasia, grade I and II prostatic intraepithelial neoplasia (PIN). Grade III prostatic intraepithelial neoplasm is cancer in situ, or uninfiltrative (unpalpable) cancer of the prostate. PIN patients

Retroperitoneal cystic lymphangioma.

Two cases of retroperitoneal cystic lymphangioma (CL) are presented; the current literature on this rare, benign neoplasm of the lymphatic system is reviewed. This tumor consists of various numbers of cyst-like cavities filled with a serous, serosanguineous or chylous fluid. The histogenesis of CL is still uncertain. Most commonly CL occurs in the neck and in the axillary region, whereas it is rare in the retroperitoneum. Although retroperitoneal CL is a benign lesion, it may cause significant morbidity due to its large size, and its often invasive character with a strong tendency to secondary

[The heart: a spectator or participant in neoplasms? Neoplastic subacute cor pulmonale].

Neoplastic subacute cor pulmonale is a distinct clinical syndrome that is often not too easily diagnosed. Such rapid development of cor pulmonale is observed in a patient with lymphangitis carcinomatosa of the lung consecutive to a cancer of the stomach with splenic, pancreatic and lymph nodes invasion. Diagnosis was established on transition of the normal ECG to a typical pattern of cor pulmonale and was revealed by chest roentgenographic examination (interstitial syndrome finding) and fatal evolution of severe respiratory failure. Necroptic and pathological studies confirmed histologic lung

Clinical Implementation of Urinary Biomarkers for Surveillance of Non-muscle-invasive Bladder Cancer (NMIBC): Considerations from the European Association of Urology NMIBC Guideline Panel.

Although high-level evidence is currently lacking, noninferiority randomised controlled trials in predefined risk groups in non-muscle-invasive bladder cancer are under way to scientifically prove the safety of urinary biomarker-guided follow-up. To facilitate recommendations for clinical use, it is essential to comply with the EU certification for in vitro diagnostics and to demonstrate cost effectiveness.

Tumour microenvironment as a predictive factor for immunotherapy in non-muscle-invasive bladder cancer.

Bladder cancer (BC) can be divided into two subgroups depending on invasion of the muscular layer: non-muscle-invasive bladder cancer (NMIBC) and muscle-invasive bladder cancer (MIBC). Its aggressiveness is associated, inter alia, with genetic aberrations like losses of 1p, 6q, 9p, 9q and 13q; gain of 5p; or alterations in the p53 and p16 pathways. Moreover, there are reported metabolic disturbances connected with poor diagnosis-for example, enhanced aerobic glycolysis, gluconeogenesis or haem catabolism.Currently, the primary way of treatment method is transurethral resection of the bladder t

Disparities in the prevalence and management of high-risk non-muscle invasive bladder cancer.

OBJECTIVE: To evaluate the associations of socioeconomic characteristics with the management of non-muscle invasive bladder cancer (NMIBC). METHODS: We identified adult patients aged 18 to 89 years with Ta, T1, or Tis NMIBC in the NCDB. We then examined the associations of patient and socioeconomic characteristics with the guidelines-based management of high-risk NMIBC using multivariable logistic regression. RESULTS: 163,949 patients were included in the study cohort, including 64% with Ta, 32% with T1, and 4% with Tis disease. Among those diagnosed with bladder cancer, male (OR 1.24, 95%CI

Semen quality from patients affected by seminomatous and non-seminomatous testicular tumor.

Testicular cancer is considered a rare disease affecting approximately 1% to 2% of the male population. This neoplasm has a cure rate of over 95%; as a result, a major concern is the future of fertility of carriers from this disease. There are several histological subtypes of testicular tumors; however, the Testicular Germ Cell Tumors (TGCTs), comprising both seminoma and non-seminoma tumors, are considered the main subtypes of testicular neoplasms. TGCT are characterized by being a solid tumor that mostly affects young men aged between 15 and 40 years old. While TGCT subtypes may have an inva

Testicular epidermoid cysts: a reevaluation.

BACKGROUND: Testicular epidermoid cysts (TECs) are rare benign testicular neoplasms. As TECs are rarely associated with germ cell tumours (GCTs), the understanding of biological behaviour and clinical management of TEC is unresolved. METHODS: We retrospectively searched the files of patients treated for testicular neoplasms and germ cell cancer in the time from 2000 to 2017. Those with TEC were subjected to closer review looking to clinical and histological features, and to results from imaging with ultrasonography (US), contrast enhanced sonography (CEUS) and magnetic resonance imaging (MRI)
