Prognostic factors in thymic epithelial neoplasms.

OBJECTIVES: The primary thymic epithelial neoplasms (PTENs) are uncommon tumours with a broad spectrum of both biological and morphological features. The aim of this study is to analyse the prognostic factors that influence survival. METHODS: Forty-four patients with a complete follow-up were analysed. Nine patients (20.5%) were asymptomatic, the most common symptoms in the rest being myasthenia gravis and dyspnoea. All the patients underwent surgery, 30 cases (68.2%) receiving total thymectomy and the rest a partial resection or biopsy. Marino-Müller's histological classification showed the

Hodgkin's disease in childhood.

A retrospective study of 116 children with Hodgkin's disease diagnosed in the period 1935-1970 was undertaken to assess the prognostic role of histopathologic classiciation and clinical extent of the disease. The ages of the 80 boys and 36 girls ranged from 2.5 years to 15.0 years (mean, 10.0 years). The histopathologic diagnosis by lymph node biopsy revealed lymphocyte predominance in 22, nodular sclerosis in 67, mixed cellularity in 24, and lymphocyte depletion in 3. Within the subgroup of nodular sclerosis, 47 biopsies had classic well-developed collagenous bands, whereas 20 were in the cel

Primary syphilis of the oropharynx mimicking a neoplasm.

Syphilis is surging in Japan and worldwide among both homosexual and heterosexual individuals. Diagnosis can be challenging because syphilis is "the great imitator" and clinical manifestations are highly variable. Oral manifestations of syphilis are usually seen in the second stage and the primary syphilis in the oral cavity is rare. We describe a heterosexual man with isolated tonsillar lesions initially misdiagnosed as pharyngeal lymphoma and subsequently diagnosed as primary syphilis. Clinicians should be aware that isolated oropharyngeal involvement can occur in the primary syphilis and ca

Role of positron emission tomography in primary carcinoma ex pleomorphic adenoma of the bronchus: A case report.

BACKGROUND: Primary carcinoma ex pleomorphic adenoma arising from the tracheobronchial system is rarely reported. CASE SUMMARY: We present a patient with primary carcinoma ex pleomorphic adenoma of the bronchus and review the associated literature for further comparison, including age, clinical manifestations, and diagnostic process. This patient had no history of neoplasms of the salivary gland. CONCLUSION: Positron emission tomography played an important role in the staging work-up of primary carcinoma of ex pleomorphic adenoma. Long-term follow-up was necessary for further prognosis analy

Disappearance of recurrent thymic epithelial tumor following corticosteroid and cyclosporine treatment: A case report.

Thymic epithelial tumors (TETs) are considered orphan neoplasms, and treatment options for recurrent or metastatic stages are limited. Here, we have reported a case of recurrent TET that showed complete remission after receiving high-dose corticosteroids followed by low-dose corticosteroids and cyclosporine. No recurrence was observed for the next 2 years. The effects of corticosteroids on the TET and the associated pure red cell aplasia led to adjustment of the diagnosis from thymic carcinoma to thymoma. Low-dose corticosteroids and cyclosporine might be the reason for remission maintenance.

Cytological Profile of a Case of Malignant Mesothelioma of Pleura with Supraclavicular Lymph Node Metastasis.

Malignant mesothelioma of pleura is a locally aggressive rare neoplasm of mesothelial cells, which produces distant metastasis in advanced stages of its course. It is exceptional for patients to present with metastasis prior to the detection of primary tumor in the pleura. Here, we report a case of malignant mesothelioma of pleura in a 65-year-old quarry worker where the initial diagnosis was offered by fine needle aspiration of an enlarged supraclavicular lymph node. Subsequent pleural fine needle aspiration cytology (FNAC), tumor biopsy, and immunohistochemical analysis confirmed the diagnos

Primary cutaneous lymphomas: diagnosis and treatment.

Primary cutaneous lymphomas (CLs) are a heterogeneous group of lymphoproliferative neoplasms, with lymphatic proliferation limited to the skin with no involvement of lymph nodes, bone marrow or viscera at the diagnosis. Cutaneous lymphomas originate from mature T-lymphocytes (65% of all cases), mature B-lymphocytes (25%) or NK cells. Histopathological evaluation including immunophenotyping of the skin biopsy specimen is the basis of the diagnosis, which must be complemented with a precise staging of the disease and identification of prognostic factors, to allow for the choice of the best treat

A case of primary diffuse large B-cell non-Hodgkin's lymphoma misdiagnosed as chronic periapical periodontitis.

Lymphoma is a malignant neoplasm of component cells of the lymphoid system which is very rare in the jaws. Here we report a case of primary diffuse large B-cell lymphoma located in the periapical region of a mandibular molar which was misdiagnosed as chronic periapical periodontitis. The present case was diagnosed at an early stage and effectively managed by chemotherapy. Although lymphoma of the mandible is rare, it must be considered in the differential diagnosis of radiolucent lesions in this region. Lack of knowledge of this rare presentation may lead to delays in diagnosis and poor progno

Endoscopic groin lymph node dissection as a preferable technique for malignant skin neoplasms.

Groin lymph node dissection (GLND) remains an effective treatment for malignant neoplasms of the skin arising on the lower extremities and perineum. However, complications such as seroma, flap necrosis, and infections have been encountered. It is thought that a conventional operation using a long inguinal incision can result in those complications. To minimise the risk of such complications, endoscopic GLND (EGLND) was performed in five patients. No severe complications were found. Although EGLND is still in the developmental stage, this technique is suggested as a possible surgical option for

[Computerized tomography of malignancies of the oral cavity, the oropharynx and hypopharynx and invasiveness].

Carcinomas of the oral cavity, oropharynx and hypopharynx represents 5% of all malignant neoplasms. The vast majority, over 90%, are squamous cell carcinomas; less common are lymphomas and tumors of the minor salivary glands. In general, the clinical findings already suggest the presence of a tumor. Therefore, the role of imaging is precise tumor localisation and staging of the malignancy. CT is still the gold standard, although MRI is gaining more importance. We present the CT appearance of most frequent malignancies of the oral cavity, oro- and hypopharynx, discussing briefly the pertinent a

The pathology of head and neck tumors: squamous and mucous-gland carcinomas of the nasal cavity, paranasal sinuses, and larynx, part 6.

Carcinomas of the nasal cavity and paranasal sinuses are statistically dominated by squamous-cell lesions, which account for between 85% and 90% of the epithelial malignancies in these sites. Neoplasms of glandular derivation are far less frequent, accounting for between 4% and 8% of carcinomas in this region. Both classes of malignancy, however, indicate an ominous prognosis for their hosts, and for both, it is the extent of the cancer, and hence the clinical stage, that is the major determinant. The clinicopathologic characteristics of squamous-cell and glandular carcinomas of the nasal cavi

Intracholecystic papillary neoplasm: A case report.

INTRODUCTION AND IMPORTANCE: More needs to be understood concerning the natural progression and visual attributes of intracholecystic papillary neoplasm. Its longevity, especially the rate at which it transitions from benign to malignant growths, remains ambiguous. Consequently, it is imperative to elucidate the intrinsic progression of this precancerous lesion in the gallbladder. CASE PRESENTATION: A 70-year-old North African woman arrived at the emergency department with abdominal pain, anorexia, and weight loss. The patient's physical examination showed favourable results for abdominal pai

NTRK-rearranged spindle cell neoplasm of the female genital tract: case report and literature review.

NTRK-rearranged spindle cell neoplasm is a rare subtype of soft tissue sarcoma that occasionally arises in the lower female genital tract. Accurate diagnosis is clinically important, as these tumors generally display low-grade malignant behavior and may respond to TRK inhibitor therapy. We report a rare case of cervical NTRK-rearranged spindle cell neoplasm in a 40-year-old woman who presented with abnormal vaginal bleeding. An initial biopsy of the cervical mass suggested a diagnosis of sarcoma. The patient subsequently underwent a total hysterectomy with bilateral salpingo-oophorectomy. Hist

Comprehensive analysis of the HCK gene in myeloid neoplasms: Insights into biological functions, prognosis, and response to antineoplastic agents.

Myeloid neoplasms result from molecular alterations in hematopoietic stem cells, with acute myeloid leukemia (AML) being one of the most aggressive and with a poor prognosis. Hematopoietic cell kinase (HCK) is a proto-oncogene that encodes a protein-tyrosine kinase of the Scr family, and it is highly expressed in AML. The present study investigated HCK expression in normal hematopoietic cells across myeloid differentiation stages and myeloid neoplasm patients. Within the AML cohort, we explored the impact of HCK expression on clinical outcomes and its correlation with clinical, genetic, and la

TP53 as a Diagnostic Aid in the Distinction of Ovarian Mucinous Borderline Tumors From Mucinous Carcinoma.

Ovarian mucinous borderline tumors (MBTs) are clinically managed as benign neoplasms while the management of ovarian mucinous carcinomas (MC) is dependent on tumor stage. Despite the standardization of sampling of ovarian mucinous neoplasms, limited interobserver reproducibility between MBT and MC persists. Based on our recent finding that abnormal TP53 expression is associated with unfavorable outcome in MBT, we hypothesized that TP53 status might improve the reproducible distinction of MBT from MC. A virtual slide set of 85 consecutive ovarian mucinous neoplasms received at a single institut

The Natural Disease Course of Pancreatic Cyst-Associated Neoplasia, Dysplasia, and Ductal Adenocarcinoma: Results of a Microsimulation Model.

BACKGROUND & AIMS: Estimates on the progression of precursor lesions to pancreatic cancer (PC) are scarce. We used microsimulation modeling to gain insight into the natural disease course of PC and its precursors. This information is pivotal to explore the efficacy of PC screening. METHODS: A Microsimulation Screening Analysis model was developed in which pancreatic intraepithelial neoplasms and cysts can evolve from low-grade dysplasia (LGD) to high-grade dysplasia (HGD) to PC. The model was calibrated to Dutch PC incidence data and Japanese precursor prevalence data (autopsy cases without P

Photodynamic therapy for treatment of recurrent adenocarcinoma of the lung with tracheal oligometastasis.

INTRODUCTION: Tracheal neoplasms account for less than 0.1% of all malignancies. The majority of tracheal malignancies are secondary neoplasms from direct tracheal invasion from adjacent structures and less commonly from hematogenous or lymphatic spread from distal malignancies. Evidence-based guidelines for management are lacking. Less invasive bronchoscopic ablation modalities are an option for non-operable patients. We report a case of endotracheal oligometastatic adenocarcinoma successfully treated with photodynamic therapy. CASE: A 59-year-old man with past medical history of stage 1 rig

Rare presentation of low-grade appendiceal mucinous neoplasms (LAMN) as an appendicular lump: A case report.

INTRODUCTION: The prevalence of appendiceal mucinous neoplasms (AMN) is about 0.2%-0.3% in the specimens of the appendix. The LAMN may appear unremarkable or can present as mucin-filled, crystally dilated tissues. The diagnosis of early-stage AMN is mostly incidental. It is of vital importance to know the features of LAMN for a timely diagnosis. CASE PRESENTATION: A 46-year-old male came with the complaint of right iliac fossa swelling along with severe intensity pain and a single episode of vomiting. A 4 × 4cm tender, soft, cystic, mobile swelling was found upon the examination. Contrast-enh

Managing misdiagnosed gingival squamous cell carcinoma (GSCC) with occult symptoms: A report of two cases.

INTRODUCTION: importance: Gingival squamous cell carcinoma (GSCC) is a rare neoplasm. GSCC has nonspecific features which commonly misdiagnosed with a simple toothache. CASE PRESENTATION: These are two cases; the first one describes a 62-year-old female who presented with pain in her right posterior mandibular teeth, and she was misdiagnosed with periodontal disease, but later histopathological tests confirmed gingival squamous cell carcinoma (GSCC). Consequently, the patient underwent surgery, and the gingival tumor was totally resected. Afterward, the patient was receiving chemotherapy, and

The cytologic and immunohistochemical findings of pancreatic mixed acinar-endocrine carcinoma.

BACKGROUND: The classification of epithelioid pancreatic neoplasms based on fine-needle aspiration (FNA) is important for proper management, as distinction of pancreatic neuroendocrine neoplasms from other similar appearing lesions can result in significantly different treatment. Mixed acinar-endocrine carcinomas (MAEC) are genetically related to acinar carcinomas and are treated as such. We reviewed cases of MAEC to better characterize their cytologic and immunohistochemical features. METHODS: Eight FNAs of MAECs were identified and reviewed. A chart review for each case was conducted. RESU
