Artificial intelligence-based morphological fingerprinting of megakaryocytes: a new tool for assessing disease in MPN patients.

Accurate diagnosis and classification of myeloproliferative neoplasms (MPNs) requires integration of clinical, morphological, and genetic findings. Despite major advances in our understanding of the molecular and genetic basis of MPNs, the morphological assessment of bone marrow trephines (BMT) is critical in differentiating MPN subtypes and their reactive mimics. However, morphological assessment is heavily constrained by a reliance on subjective, qualitative, and poorly reproducible criteria. To improve the morphological assessment of MPNs, we have developed a machine learning approach for t

Diagnosis of a tympanic paraganglioma with CT perfusion imaging: a technical note and case description.

Paragangliomas can be diagnosed accurately using magnetic resonance imaging and dynamic four-dimensional magnetic resonance angiography. Four-dimensional imaging uses the highly vascularised structure of these tumours, which results in a homogenous capillary blush and usually, due to the arteriovenous shunting, in an early draining vein. By these features the tumour can be differentiated from other neoplasms. The authors describe a case of a paraganglioma localised in the middle ear of an elderly patient. Magnetic resonance contraindications led to preoperative diagnostics with high resolution

Solitary Fibrous Tumors in Pediatric Patients: A Rare and Potentially Overdiagnosed Neoplasm, Confirmed by STAT6 Immunohistochemistry.

Pathological diagnosis of solitary fibrous tumor (SFT) in the pediatric population is challenging, as it occurs uncommonly in this age-group and resembles other spindle cell neoplasms. SFT contains a NAB2-STAT6 fusion gene, which can be reliably detected using STAT6 immunohistochemistry. Positive staining is highly sensitive and specific. We sought to investigate the utility of STAT6 immunohistochemistry, to show how commonly SFT was historically recognized at 3 academic pediatric institutions, to reclassify them when appropriate, and to demonstrate features of major mimics of SFT. Our series

Development of second primary malignancy in patients with non-Hodgkin lymphoma: a nationwide population-based study.

BACKGROUND: With the improved survival of non-Hodgkin lymphoma (NHL) patients, development of second primary malignancy (SPM) has become an increasingly important issue in these long-term survivors. METHODS: We conducted a retrospective study to analyze NHL patients diagnosed between January 1997 and December 2010 in Taiwan. Standardized incidence ratios (SIRs) were applied to compare the risk of SPMs in NHL patients and the general population. Multivariate analysis was performed to determine the independent predictors of SPM. RESULT: NHL patients have a significantly greater risk of develop

TDT (-), KIT (+), CD34 (+), CD99 (+) precursor T lymphoblastic leukemia/lymphoma.

Although the definition of precursor T lymphoblastic lymphoma (T-LBL) is based only on histopathology, most cases cannot be diagnosed only by HE sections. Since 95% of T-LBL expresses TdT and TdT is expressed only in lymphoblasts, immunohistochemical demonstration of TdT is mandatory for the diagnosis of TLBL. However, little is known about the expression of other precursor cell molecules. A 58-year-old woman with myelodysplastic syndrome (RAEB) became overt acute myelogenous leukemia (AML). She was treated twice with allogeneic peripheral blood stem cell transplantation from her son. Nine mon

Primary bone tumours and tumour-like lesions in children in Zaria, Nigeria.

BACKGROUND: Studies on bone neoplasms are generally scanty globally and more so in children. Primary bone tumours and tumour-like lesions in children have not been reported from Zaria. OBJECTIVE: To determine the relative frequencies, sex and age distributions, and anatomical sites of occurrence of primary bone tumours and tumour-like lesions in children in Zaria. MATERIALS AND METHODS: A retrospective review of histopathology reports of 40 children with bone tumours and tumour-like lesions in 11 years of age. RESULTS: Benign tumours accounted for 12 (30%) of the 40 tumours reviewed (osteom

MR imaging of miscellaneous disorders of the shoulder.

During the course of routine MR imaging of the shoulder, a wide variety of abnormalities may be encountered owing to the technique's broad soft-tissue contrast resolution and multiplanar tomographic capability. In this article, the authors provide an overview of the lesions that might be encountered in this setting.

Incidence and survival of European adolescents and young adults diagnosed with sarcomas: EUROCARE-6 results.

BACKGROUND: Epidemiological data for sarcoma in adolescents and young adults (AYAs) and across age groups are limited. We aim to: 1) update sarcoma incidence, survival, and changes over time in European AYAs; 2) provide an updated comparison of sarcoma survival in AYAs versus children and mature adults. METHODS: We calculated crude incidence rates (IR) per 100,000 European population per year from 2006 to 2013. Using the period approach, we calculated 5-year relative survival (RS) for the follow-up period 2010-2014. We estimated changes in incidence and survival for bone sarcoma (BS) and soft

Management of the Hand Tumors.

Tumors in the hand are relatively uncommon but 95% are benign. Tumors occurring in the hand, forearm and arm often have unique growth patterns and potential for metastasis that may be different from those seen elsewhere in the body. Secondary metastatic tumors in the hand are very rare (0.1%). Diagnosis is mainly clinical, but X-ray, USG and MRI help as a diagnostic aid. The aim of the study was to early diagnosis, see the pattern and proper management of the hand tumor and ensure good hand function. This prospective study was done from January 2004 to July 2019. We found 220 hand tumors in th

Novel MAP4::RAF1 Fusion in a Primary Bone Sarcoma: Expanding the spectrum of RAF1 Fusion Sarcoma.

Soft tissue tumors with RAF1 fusion had been emerging as a group of tumors with peculiar histology and immunoprofile. While a case series and rare case reports of RAF1 translocated sarcoma had been reported, to our knowledge a primary bone tumor with RAF1 translocation and fusion partner with MAP4 had not been described in the literature. The patient was a 60-year-old lady, with strong family history of breast cancer, who presented with pathological fracture of right humerus. X-ray revealed a 9.7 cm juxta-articular lesion of the proximal humerus, which was expansile and lytic with a non-sclero

Safe Use of Radium-223 Dichloride (223RaCl2) Across a Wide Range of Clinical Scenarios, Incorporating a 10-year Single-Institution Radiation Safety Experience.

Radium 223Ra dichloride (223RaCl2) is an effective therapeutic radiopharmaceutical presently approved for the treatment of prostate cancer metastatic to bone. It is unique by virtue of being the first alpha-emitting radiopharmaceutical to achieve approval for use in the clinic, reaching this status both in the United States and Europe in 2013. In over ten years of research and approved clinical usage, the authors have encountered very few radiation-safety incidents of concern with 223RaCl2; in this review, they relate their first-hand experience with this radiopharmaceutical and share some les

Oncogenic Osteomalacia From a Primary Phosphaturic Mesenchymal Tumor of the Toe: A Case Report.

Oncogenic osteomalacia is an acquired, rare paraneoplastic syndrome characterized by renal phosphate wasting and subsequent hypophosphatemic osteomalacia. The condition is usually associated with a phosphaturic mesenchymal tumor, which produces fibroblast growth factor 23, the primary circulating factor responsible for reduced tubular phosphate reabsorption. Clinically, adult patients typically present with bone pain, myalgia, recurrent and/or multiple stress fractures, and fatigue, with serum levels typified by low 1,25-(OH)2 vitamin D3, increased alkaline phosphatase, and normal calcium, par

Radiographic features of osseous metastases of soft-tissue sarcomas.

In contrast to most other neoplastic processes of bone, in our study osseous metastases from soft-tissue sarcomas (STS) were more reliably detected by radiography than by scintigraphy. The radiographic manifestations of 65 skeletal metastases in 23 patients with STS were reviewed. Although most metastases (88%) were osteolytic, eight (12%) lesions were osteoblastic. The destructive patterns were geographic in 29%, moth-eaten in 55%, and permeative in 12% of the lesions. The axial skeleton was most commonly involved. In the long bones the diaphyses were the predominant sites of involvement. The
