Kidney involvement in a Wegener granulomatosis case.

Wegener Granulomatosis is a systemic Anti-Neutrophil Cytoplasmic Autoantibody-associated Vasculitis, affecting small-to-medium vessels. Clinical presentation with simultaneous involvement of kidney and upper and lower respiratory tract is unusual. We report an instructive case of WG, analyzing clinical course, laboratory, and radiological features, kidney, lung, and larynx histological pictures. Besides renal biopsy, nephrology team performed larynx and lung biopsies because of unusual clinical presentation, computed tomography chest examination, and relevant malignancy risk regarding followin…

Acute kidney injury and dialysis in children: illustrative cases.

Pediatric nephrologists and critical care physicians are faced with a heterogeneous patient population with varied epidemiology caring for children with acute kidney injury or other diseases that may require renal replacement therapy provision. We have composed 4 detailed case scenarios to highlight the challenges and interdisciplinary approach required for optimal care provision to children, and that serve to direct the different articles contained in this special issue of Seminars of Nephrology devoted to acute kidney injury in children.

Wound care management in the end-stage renal disease population.

As the end-stage renal disease patient population ages, the management of pressure ulcers and vascular ulcers becomes an increasingly common problem. The practice of wound care management and the devices used to manage wounds has changed dramatically. Basic knowledge of the healing process and new techniques in wound care is paramount for the nephrology team. Identification of patients at risk for wounds and the development of wound care models is one step in the goal towards preventative care.

Renal cystic disease in tuberous sclerosis complex.

Tuberous sclerosis complex (TSC) is associated with TSC1 or TSC2 gene mutations resulting in hyperactivation of the mTORC1 pathway. This mTORC1 activation is associated with abnormal tissue development and proliferation such that in the kidney there are both solid tumors and cystic lesions. This review summarizes recent advances in tuberous sclerosis complex nephrology and focuses on the genetics and cell biology of tuberous sclerosis complex renal disease, highlighting a role of extracellular vesicles and the innate immune system in disease pathogenesis.

[Division-related function and organ-related therapy in Fabry's disease. An interdisciplinary challenge].

Fabry's disease is a rare X-linked lysosomal storage disorder leading to an accumulation of globotriaosylceramides in the lysosomes of all tissues. As the accumulation occurs in all organs, different medical faculties are involved in the diagnostics and therapy of Fabry's disease. With this review the three main faculties (cardiology, nephrology and neurology) as well as the adjacent faculties (ophthalmology and dermatology) want to discuss the division-related function and also to suggest an organ-related additional therapy besides enzyme replacement therapy.

Toward personalized medicine in childhood nephrotic syndrome.

Childhood idiopathic nephrotic syndrome remains treated empirically despite profound pathophysiological heterogeneity. Tu et al. develop a polygenic risk score incorporating clinical data and human leukocyte antigen class II variants to stratify steroid responsiveness at diagnosis. Although predictive performance remains modest and several limitations preclude immediate clinical translation, this work represents a pivotal step toward precision nephrology, envisioning a future where clinical, serologic, and genetic markers converge to guide personalized therapy, minimize steroid exposure, and i…

Immunosuppression in children with primary mitochondrial diseases after kidney transplantation: insight from case series of RMND1-related disease.

While successful kidney transplantation has been reported in pediatric patients with primary mitochondrial diseases, immunosuppression regimen and its effect on systemic disease were not described. We present four pediatric patients with genetically confirmed RMND1 disease in a quaternary nephrology center. They presented at a very young age and progressed rapidly to stage 5 chronic kidney disease. All underwent successful kidney transplantation. Their allograft function remained stable throughout the follow-up period, and they did not manifest any major systemic deterioration.

[Possibilities of orthopaedic treatment of patients with renal failure and its meaning in the continuation of haemodialysis and quality of life improvement].

Good cooperation between the fields of orthopaedic surgery and nephrology creates a possibility of multidirectional surgical treatment of the patients with terminal renal failure despite massive general burden and existing risk. Taking a very difficult decision concerning the multidirectional surgical treatment of these patients may to a very great extent improve their locomotor functions and their life comfort. An example of such a situation is the described case of elbow replacement, vascular surgery procedure, total hip replacement and carpal tunnel release surgery.

Acute renal failure owing to paraphenylene diamine hair dye poisoning in Sudanese children.

INTRODUCTION: Paraphenylene diamine (PPD) has traditionally been used as a dark-coloured hair dye. In Sudan, it is used by women to colour their hair and as a body dye when added to henna (Lawasonia alba). Accidental or deliberate ingestion causes severe systemic toxicity. Although a wide variety of complications has been described, there are few reports in children. AIM: To describe the clinical features, management and outcome of PPD intoxication in Sudanese children. METHODS: Data for a 3-year period (2006-2008) were extracted from the medical records of the Paediatric Nephrology Unit, So…

Progression to End-Stage Renal Disease Due to IgG4-Related Nephritis Refractory to Rituximab.

An 81-year-old woman was referred to nephrology for a follow-up on progressive chronic kidney disease. She has a past medical history of hypertension, T2DM, breast cancer, and secondary hyperparathyroidism related to renal disease. A renal biopsy showed patchy interstitial fibrosis and tubular atrophy with an increased number of IgG4-positive plasma cells. A diagnosis of IgG4-related kidney disease was made based on clinical presentation and pathology. The patient ultimately required the initiation of hemodialysis, despite the administration of steroids and rituximab.

[Subacute kidney injury in a 40-year-old female Northern African patient].

A 40-year old woman from Eritrea was admitted due to worsening renal function of unknown origin. The basic nephrologic workup provided no further information. Renal biopsy was performed and revealed acute interstitial nephritis (AIN) while no offending medication could be identified. Further investigations showed a recurrence of the urogenitary tuberculosis that had already been in 2015. The finding of AIN in the absence of a typical medical history should raise suspicion of infection-related forms or AIN associated with systemic diseases.
