Multibacillary nerve histology in clinically diagnosed borderline tuberculoid leprosy patients.

The classification of leprosy into multibacillary (MB) and paucibacillary (PB) patients in almost all clinics is entirely dependent on clinical examination. In a study of 21 patients clinically classified as borderline tuberculoid (BT) and, therefore, belonging to the PB group, skin smears and skin and nerve biopsies were examined. Four patients did not have any histopathological evidence of leprosy. Skin smears showed that 1 patient was positive for acid-fast bacilli (AFB), 2 skin biopsies belonged to the borderline lepromatous (BL) category and showed AFB in their lesions, and AFB were prese

Anatomic proximity of the peroneal nerve to the posterolateral corner of the knee determined by MR imaging.

BACKGROUND: The pie crusting technique has been extensively used to release the lateral soft tissue in total knee arthroplasty. However, it may place the peroneal nerve at direct injury risk when performed in a valgus knee. The aim of this study was to determine the anatomic proximity of the peroneal nerve to the posterolateral corner of the knee. METHODS: One hundred knees were measured on axial MR images for the proximity of peroneal nerve to the closest edge of the inner surface of joint capsule or the posterolateral corner of proximal tibia at the level of the joint line and the level of

[Lingual nerve injury during removal of the lower third molar: importance of early intervention].

INTRODUCTION: Lingual nerve injury is one of the most important complications after removal of a lower third molar. CASE REPORT: We report two cases of this complication in our experience. In the first case, lingual nerve injury was repaired immediately and the patient recovered total tongue sensitivity. In the second case, lingual nerve exploration was not undertaken and the patient developed complete tongue anesthesia. DISCUSSION: Opinions are quite different about the delay before exploration of an injured lingual nerve. However, absence of sensory improvement incited us to undertake surg

[Repair of the extracranial facial nerve defect by autogenous vein graft: an experimental study].

In this study, we repaired the extracranial facial nerve defect of rabbit using an autogenous vein graft and compared the results with conventional autogenous nerve graft in function and morphology. A 1 cm segment of the extracranial morphology. A 1 cm segment of the extracranial facial nerve was removed in 30 rabbits, the defect was repaired by a segment of vein or by a segment of nerve or without repair. The gross observation, histologic and histochemical examination, electrophysiological study and nerve fascicle graphics analysis were performed at 4, 8, 12, 20 weeks. The results of this stu

Mesenchymal differentiation in trigeminal neurilemmoma.

An unusual neurilemmoma of the trigeminal nerve in a 31-year-old woman is described. The neoplasm was roentgenographically calcified and contained various types of mesenchymal tissue, including a cavernous angioma, cartilage, bone, and adipose tissue. Angioma in combination with neurilemmoma occurs more frequently than other forms of mesenchymal tissue. Lipoma is an uncommon component of neoplasms affecting the cranial nerves. Cartilage and bone are rarely encountered in benign intracranial neurilemmomas. The histogenesis of this combined neoplasm is discussed in relation to the ectomesenchyma

Comparison of ulnar nerve repair according to injury level and type.

PURPOSE: The aim of the study was to evaluate the results of epiperineural suture repaired primary (clean transaction injury, massive soft-tissue associated injury) and secondary (delayed partial injury) ulnar nerve injuries according to lesion level and type. METHODS: Forty-two patients diagnosed with ulnar nerve injury between January 2008 and January 2012 were involved in the study. Ulnar nerve lesions were classified according to the level of injury into three types: type 1--lesion located above the flexor carpi ulnaris branch; type 2--lesion located between the flexor carpi ulnaris and t

Waterjet dissection of peripheral nerves: an experimental study of the sciatic nerve of rats.

BACKGROUND: Although waterjet dissection has been well evaluated in intracranial pathologies, little is known of its qualities in peripheral nerve surgery. Theoretically, the precise dissection qualities could support the separation of nerves from adjacent tissues and improve the preservation of nerve integrity in peripheral nerve surgery. OBJECTIVE: To evaluate the potential of the new waterjet dissector in peripheral nerve surgery. METHODS: Waterjet dissection with pressures of 20 to 80 bar was applied on the sciatic nerves of 101 rats. The effect of waterjet dissection on the sciatic nerv

Side-To-Side Nerve Bridges Support Donor Axon Regeneration Into Chronically Denervated Nerves and Are Associated With Characteristic Changes in Schwann Cell Phenotype.

BACKGROUND: Chronic denervation resulting from long nerve regeneration times and distances contributes greatly to suboptimal outcomes following nerve injuries. Recent studies showed that multiple nerve grafts inserted between an intact donor nerve and a denervated distal recipient nerve stump (termed "side-to-side nerve bridges") enhanced regeneration after delayed nerve repair. OBJECTIVE: To examine the cellular aspects of axon growth across these bridges to explore the "protective" mechanism of donor axons on chronically denervated Schwann cells. METHODS: In Sprague Dawley rats, 3 side-to-

Quantitation of the lower subscapular nerve for potential use in neurotization procedures.

OBJECT: New information regarding nerve branches of the brachial plexus can be useful to the surgeon performing neurotization procedures following patient injury. Nerves in the vicinity of the axillae have been commonly used for neural grafting procedures, with the exception of the lower subscapular nerve (LSN). METHODS: The authors dissected and measured the LSN in 47 upper extremities (left and right sides) obtained in 27 adult cadavers, and determined distances between the LSN and surrounding nerves to help quantify it for possible use in neurotization procedures. The mean diameter of the

Brain-derived neurotrophic factor and peripheral nerve regeneration: a functional evaluation.

The potential benefit of brain-derived neurotrophic factor (BDNF) on motor-nerve regeneration after transection injury in 24 adult rats was evaluated after entubulation repair. Gait analysis for ankle angle and tension transduction device (TTD) strain-gauge measurements yielded functional evaluation of regeneration. The BDNF (15 mg/mL) or phosphate buffered saline (control) was injected into the silicone elastomer (Silastic) channel. Gait analysis performed 0, 2, 4, 6, 10, and 12 weeks after injury demonstrated a significant difference between uninjured and injured legs of 23 and 43 degrees, r

Acute thallium poisoning: toxicological and morphological studies of the nervous system.

Nine days following ingestion of 5 to 10 gm of thallium nitrate, a young man died with severe cranial and peripheral neuropathy, anuria, and heart failure. Ultrastructural examination of nerves obtained on days 7 and 9 demonstrated axonal degeneration with secondary myelin loss. Axons were swollen and contained distended mitochondria and vacuoles. Thallium levels in more than twenty organs and body fluids ranged from below 1.0 to 178 microgram/gm; concentrations in twenty areas of the nervous system ranged from 29 to 140 microgram/gm. The highest brain levels of thallium were found in gray mat
