DeepFocus: a transnasal approach for optimized deep brain stimulation of reward circuit nodes.

Objective.Transcranial electrical stimulation (TES) is an effective technique to modulate brain activity and treat diseases. However, TES is primarily used to stimulate superficial brain regions and is unable to reach deeper targets. The spread of injected currents in the head is affected by volume conduction and the additional spreading of currents as they move through head layers with different conductivities, as is discussed in Forssellet al(2021J. Neural Eng.18046042). In this paper, we introduce DeepFocus, a technique aimed at stimulating deep brain structures in the brain's 'reward circu

Brain incoming call from glia during neuroinflammation: Roles of extracellular vesicles.

The functionality of the central nervous system (CNS) relies on the connection, integration, and the exchange of information among neural cells. The crosstalk among glial cells and neurons is pivotal for a series of neural functions, such as development of the nervous system, electric conduction, synaptic transmission, neural circuit establishment, and brain homeostasis. Glial cells are crucial players in the maintenance of brain functionality in physiological and disease conditions. Neuroinflammation is a common pathological process in various brain disorders, such as neurodegenerative diseas

Therapeutic potential of Pnmt+ primer cells for neuro/myocardial regeneration.

Phenylethanolamine n-methyltransferase (Pnmt) catalyzes the conversion of norepinephrine into epinephrine, and thus serves as a marker of adrenergic cells. In adults, adrenergic cells are present in the adrenal medullae and the central and peripheral (sympathetic) nervous systems where they play key roles in stress responses and a variety of other functions. During early embryonic development, however, Pnmt first appears in the heart where it is associated with specialized myocytes in the pacemaking and conduction system. There is a transient surge in cardiac Pnmt expression beginning when the

Neural autoantibodies and neurophysiologic abnormalities in patients exposed to molds in water-damaged buildings.

Adverse health effects of fungal bioaerosols on occupants of water-damaged homes and other buildings have been reported. Recently, it has been suggested that mold exposure causes neurological injury. The authors investigated neurological antibodies and neurophysiological abnormalities in patients exposed to molds at home who developed symptoms of peripheral neuropathy (i.e., numbness, tingling, tremors, and muscle weakness in the extremities). Serum samples were collected and analyzed with the enzyme-linked immunosorbent assay (ELISA) technique for antibodies to myelin basic protein, myelin-as

Double dissociation between conduction aphasia and conduction agraphia supports a ventro-dorsal partition of the left arcuate fasciculus.

We identified in two awake surgery cases a postoperative double dissociation between phonological and graphemic output buffer deficits. Using lesion-symptom mapping from ischaemic mini-strokes and preoperative tractography, we demonstrated that the phonological (resp. graphemic) disorder fitted with ventral (resp. dorsal) damage to the AF. Further studies are needed to confirm our hypothesis of a ventro-dorsal functional organization within the AF for phonological versus graphemic processing.

Unmasking the mimic: Leprosy neuropathy misdiagnosed as chronic inflammatory demyelinating polyneuropathy: A case report from Saudi Arabia.

Leprosy neuropathy can mimic chronic inflammatory demyelinating polyneuropathy (CIDP), especially in non-endemic areas. We report a 72-year-old Saudi woman initially misdiagnosed with CIDP based on nerve conduction studies. The patient presented with widespread pruritus, erythematous cutaneous lesions, and progressive sensorimotor symptoms. Despite treatment with intravenous immunoglobulin, her condition worsened. Subsequent sural nerve and skin biopsies revealed acid-fast bacilli, confirming leprosy. This case highlights the importance of considering leprosy in the differential diagnosis of n

Neuropathy in multiple sclerosis patients treated with teriflunomide.

OBJECTIVE: Teriflunomide is an oral medication approved for the treatment of patients with multiple sclerosis. The primary effect of teriflunomide is to reduce de novo pyrimidine synthesis by inhibiting mitochondrial dihydroorotate dehydrogenase, thereby causing cell-cycle arrest. We aimed to investigate the occurrence of peripheral neuropathy, a rare side effect of teriflunomide, in patients receiving teriflunomide. METHODS: Multiple sclerosis patients receiving teriflunomide (n=42) or other disease modifying therapies (n=18) and healthy controls (n=25) were enrolled in this cross-sectional

Chronic inflammatory demyelinating polyradiculoneuropathy-associated tremor: Phenotype and pathogenesis.

BACKGROUND AND PURPOSE: Tremor in chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) is underrecognized, and the pathophysiology remains incompletely understood. This study evaluated tremor in CIDP and tested the hypothesis, established in other demyelinating neuropathies, that tremor occurs due to mistimed peripheral inputs affecting central motor processing. Additionally, the tremor stability index (TSI) was calculated with the hypothesis that CIDP-related tremor is more variable than other tremor disorders. METHODS: Consecutive patients with typical CIDP were prospectively re

Normal Values of Central, Peripheral, and Root Motor Conduction Times in a Healthy Korean Population.

PURPOSE: Central, peripheral, and root motor conduction times (CMCTs, PMCTs, and RMCTs, respectively) are valuable diagnostic tools for spinal cord and motor nerve root lesions. We investigated the normal values and the effects of age and height on each motor conduction time. METHODS: This study included 190 healthy Korean subjects who underwent magnetic stimulation of the cortex and spinous processes at the C7 and L1 levels. Recording muscles were abductor pollicis brevis and abductor digiti minimi in the unilateral upper limb and extensor digitorum brevis and abductor hallucis in the contra

Unexpected Intermediate Nerve Conduction Velocity Findings in Charcot-Marie-Tooth Syndromes Classified as Demyelinated or Axonal in a Pediatric Population.

INTRODUCTION: Among the hereditary motor and sensory neuropathies (HMSN), demyelinating forms are the best characterized, with a clear predominance of CMT1A. The axonal and intermediate forms are less described. The aim of this study is to report the genetic diagnosis of Charcot-Marie-Tooth (CMT) according to the nerve conduction velocity (NCV) findings in a pediatric population. METHODS: We retrospectively described a population of HMSN children with a confirmed genetic diagnosis of demyelinated, intermediate, or axonal forms. We compared the results of the genetic analyses with those of mot

Elucidation of peripheral nerve myelination by bimodal waveform analysis.

PURPOSE: As the myelin sheath of the peripheral nerves is immaturely developed in children, a bimodal waveform may be evoked when sensory nerve conduction studies are performed in them. However, there has been no study regarding the relationship between age and the frequency of appearance of the bimodal waveform or the inter-peak latency. Herein, we examined how the bimodal waveform changes with age. METHODS: A total of 218 sensory nerves (109 median and 109 ulnar nerves) in 86 children (0 to 15 years old) were analyzed. The frequency of the bimodal waveform and the distance-corrected inter-p

Is psychological distress associated with carpal tunnel syndrome symptoms and nerve conduction study findings? A case-control study from Syria.

BACKGROUND: Carpal tunnel syndrome (CTS) is a common entrapment neuropathy of the median nerve at the wrist which causes severe symptoms. However, psychological aspects can affect patients' perception of this pain and can cause similar pain in some instances. This study aims to determine the association between symptoms severity, functional status, and nerve conduction studies (NCS) of adult patients with CTS and their anger, anxiety, and depression status. METHODS: This case-control study was conducted in clinics in Damascus, Syria. Controls were frequency matched by gender and age from a ge

Spinal cord sarcoidosis in Japan: utility of cerebrospinal fluid examination and nerve conduction study for diagnosis and prognosis prediction.

Spinal cord sarcoidosis (SCS) is rare, and its diagnosis is challenging. We examined clinical, laboratory, and imaging features in patients with SCS to obtain useful clues for diagnosis and prognosis. Eleven consecutive patients (four males, seven females) at a single Japanese institution were investigated. Median age at onset was 66 years old. The most frequent site affected, other than the nervous system, was the respiratory system. While histological confirmation of non-caseating granulomas was often found there, no patient had respiratory symptoms. Peripheral nerve involvement was detected

Elovl5 is required for proper action potential conduction along peripheral myelinated fibers.

Elovl5 elongates fatty acids with 18 carbon atoms and in cooperation with other enzymes guarantees the normal levels of very long-chain fatty acids, which are necessary for a proper membrane structure. Action potential conduction along myelinated axons depends on structural integrity of myelin, which is maintained by a correct amount of fatty acids and a proper interaction between fatty acids and myelin proteins. We hypothesized that in Elovl5-/- mice, the lack of elongation of Elovl5 substrates might cause alterations of myelin structure. The analysis of myelin ultrastructure showed an enlarg

Inhibiting Spasticity by Blocking Nerve Signal Conduction in Rats With Spinal Cord Transection.

Spasticity is a common motor disorder following a variety of upper motor neuron lesions that seriously affects the quality of patient's life. We aimed to evaluate whether muscle spasms can be suppressed by blocking nerve signal conduction. A rat model of lower limb spasm was prepared and the conduction of pathological nerve signals were blocked to study the inhibitory effect of nerve signal block on muscle spasm. The experimental results showed that 4 weeks after the 9th segment of the rat's thoracic spinal cord was completely transacted, the H/M -ratio of the lower limbs increased, and rate-d

Radiological factors associated with the severity of corticospinal tract dysfunctions for cervical spondylotic myelopathy: An analysis of the central motor conduction time and kinematic CT myelography.

Patients with cervical spondylotic myelopathy (CSM) often exhibit symptoms in clinical practice, particularly the elderly, whose lower extremity functions are more likely to deteriorate; however, the underlying mechanisms currently remain unclear. The present study aimed to elucidate the relationship between the neurological severity of CSM based on an electrophysiological examination and radiological findings. Eighty-six patients with CSM were examined using kinematic CT myelography. The cross-sectional area of the spinal cord and dynamic changes in the spinal cord were measured at the affect
