Consensus protocol for EEG and amplitude-integrated EEG assessment and monitoring in neonates.

The aim of this work is to establish inclusive guidelines on electroencephalography (EEG) applicable to all neonatal intensive care units (NICUs). Guidelines on ideal EEG monitoring for neonates are available, but there are significant barriers to their implementation in many centres around the world. These include barriers due to limited resources regarding the availability of equipment and technical and interpretive round-the-clock personnel. On the other hand, despite its limitations, amplitude-integrated EEG (aEEG) (previously called Cerebral Function Monitor [CFM]) is a common alternative

Altered neural oscillatory dynamics underlie reduced anticipatory schema use during event segmentation in adolescents with High-Functioning Autism Spectrum disorder.

Autism Spectrum Disorder (ASD) is characterized by diverse symptoms that may stem from atypical integration of sensory information and prediction processes. Event Segmentation Theory (EST) offers a framework to examine how individuals parse continuous experience into meaningful units. This study investigated whether altered event segmentation in adolescents with ASD arises from atypical neural oscillatory dynamics underlying prediction and updating processes. Behaviorally, adolescents with ASD showed a weaker adaption of segmentation behavior to the occurrence of situational changes than neuro

Abruptly attenuated terminal ictal pattern in pediatrics.

PURPOSE: We examined the ictal discharges at the end of pediatric seizures and categorized the various patterns. One particular pattern, termed "abruptly attenuated termination" was studied in detail. METHODS: Ictal segments captured on video-EEG monitoring during a 26-month interval were analyzed for a variety of ictal termination patterns, including one that we rigorously defined as abruptly attenuated termination pattern (AAT). We studied the associations between AAT and the other ictal EEG and clinical features. RESULTS: AAT was noted in 16 of 200 (8%) pediatric seizures. All 16 were imm

The use of VEPs for CNS monitoring during continuous cardiopulmonary bypass and circulatory arrest.

Cerebral function was monitored with the use of visual evoked potentials (VEPs) in 16 infants (mean age 9.9 +/- 4.3 months) during surgery for congenital cardiac anomalies. While hypothermia was employed in all patients, half (8) remained on continuous cardiopulmonary bypass (CCB) while the rest were cooled to lower temperatures before the induction of circulatory stasis and venous exsanguination (CA), i.e., profound hypothermic circulatory arrest (PHCA). VEPs were recorded before, during and after surgical intervention. Latency changes occurred in both the N100 and P145 components of the VEP

The Recognition, Incidence, and Management of Spinal Cord Monitoring Alerts in Pediatric Cervical Spine Surgery.

BACKGROUND: Perioperative spinal cord injury and postoperative neurological deficits are the major complications in spinal surgery. Monitoring of spinal cord function is of crucial importance. Somatosensory evoked potentials and transcranial electric motor-evoked potentials are now widely used in cervical spine surgery. Although much has been written on spinal cord monitoring in adult spinal surgery, very little has been published on the incidence and management of monitoring of cervical spine surgery in the pediatric population. The goal of this research was to review the recognition, inciden

Free-hand electrode placement for intraoperative monitoring of extraocular cranial nerves in skull base surgery: preliminary experience and feasibility assessment.

Postoperative dysfunction of the oculomotor (CN III) and abducens (CN VI) nerves remains a major determinant of disability after skull base surgery for tumors. This study assessed the feasibility, safety, and diagnostic performance of a surgeon-controlled, free-hand extraocular muscle electrode placement for corticobulbar motor evoked potentials (cb-MEPs) and direct nerve stimulation (DNS). This monocentric, observational, retrospective study enrolled 40 patients scheduled for skull base tumor surgery, with planned intraoperative monitoring of CN III and/or VI. Curved needle electrodes were pl

The value of intraoperative neuromonitoring and neurostimulation in thyroid surgery: A single-center retrospective analysis and prospective follow-up.

This study aims to determine the accuracy of neurostimulation with laryngeal palpation (NSLP) and intermittent intraoperative neuromonitoring (I-IONM) to predict the postoperative function of the recurrent laryngeal nerve during thyroid surgery and to follow up patients with vocal fold paralysis (VFP). This was a historical single-center retrospective cohort study with a prospective follow-up. A total of 1471 patients (2408 nerves at risk) comparing I-IONM and NSLP for patients undergoing hemi-, total, or completion thyroidectomy from January 2009 to January 2024. The I-IONM group included 810

Early-Phase Quadriceps Activation After Knee Surgery: A Narrative Review of Current Rehabilitation Interventions and Identification of an Unmet Clinical Need.

Arthrogenic muscle inhibition (AMI), neurophysiological suppression of voluntary quadriceps activation triggered by joint effusion and inflammation, is consistently initiated within hours of any form of knee surgery. If not actively counteracted during the first two postoperative weeks, AMI may drive a cascade of neuromuscular, morphological, and biomechanical deficits that can persist for years, substantially increasing the risk of post-traumatic osteoarthritis, reinjury, and long-term functional disability. Emerging evidence indicates that preoperative patient-related factors, including base

The Use of Magnetoencephalography in the Diagnosis and Monitoring of Mild Traumatic Brain Injuries and Post-Concussion Syndrome.

Background/Objectives: The main objective of this systematic review was to explore the role of magnetoencephalography (MEG) in the diagnosis, assessment, and monitoring of mild traumatic brain injury (mTBI) and post-concussion syndrome (PCS). We aimed to evaluate the potential of some MEG biomarkers in detecting subtle brain abnormalities often missed by conventional imaging techniques. Methods: A systematic review was conducted using 25 studies that administered MEG to examine mTBI and PCS patients. The quality of the studies was assessed based on selection, comparability, and outcomes. Studi

Electrophysiological correlates of feedback processing in subarachnoid hemorrhage patients.

Patients with subarachnoid hemorrhage (SAH) secondary to anterior communicating artery (AComA) aneurysm rupture often experience deficits in executive functioning and decision-making. Effective decision-making is based on the subjects' ability to adjust their performance based on feedback processing, ascribing either positive or negative value to the actions performed reinforcing the most adaptive behavior in an appropriate temporal framework. A crucial brain structure associated to feedback processing is the medial prefrontal cortex (mPFC), a brain region frequently damaged after AComA aneury

A machine learning approach to predicting dyspnea with noninvasive biomarkers.

Dyspnea is the subjective sensation of breathing discomfort. This symptom is highly prevalent in patients with chronic and critical illness, and its presence is associated with poor clinical outcomes and long-term psychological trauma. The multidimensional nature of the neurophysiological mechanisms underlying dyspnea, paired with individual variation in its presentation, makes identifying and monitoring this symptom difficult, particularly in non-communicative patients. Undetected and untreated dyspnea in critically ill patients is a significant problem contributing to patient suffering. Ther

Distinct neural circuits for control of movement vs. holding still.

In generating a point-to-point movement, the brain does more than produce the transient commands needed to move the body part; it also produces the sustained commands that are needed to hold the body part at its destination. In the oculomotor system, these functions are mapped onto two distinct circuits: a premotor circuit that specializes in generating the transient activity that displaces the eyes and a "neural integrator" that transforms that transient input into sustained activity that holds the eyes. Different parts of the cerebellum adaptively control the motor commands during these two

Posterior Approach for Microsurgical Resection of a Ventrolateral Cervical Ganglioneuroma: an Illustrative Case and Technical Report.

Ganglioneuromas of the cervical spine are exceptionally rare, well-differentiated neuroblastic tumors. We present the case of a 4-year-old female with a several-month history of progressive fatigue, right upper extremity weakness, and gait instability, in whom MRI demonstrated a homogeneously enhancing, cervical intradural extramedullary mass causing significant ventral spinal cord compression. A posterior midline approach with cervical two to cervical six laminoplasty was performed under continuous neurophysiological monitoring, providing wide dorsal access to the lesion.
