Addressing barriers and identifying facilitators to support informed consent and recruitment in the Cavernous malformations A Randomised Effectiveness (CARE) pilot phase trial: insights from the integrated QuinteT recruitment intervention (QRI).

Facilitators identified included the preparedness of some neurosurgeons' to offer surgery to people otherwise offered medical management alone, multi-disciplinary team equipoise, and effective information provision presenting participation as a solution to equipoise regarding management. Hydrocephalus surveillance following CSF diversion: a modified Delphi study.

OBJECTIVE: Long-term follow-up is often recommended for patients with hydrocephalus, but the frequency of clinical follow-up, timing and modality of imaging, and duration of surveillance have not been clearly defined. Here, the authors used the modified Delphi method to identify areas of consensus regarding the modality, frequency, and duration of hydrocephalus surveillance following surgical treatment. METHODS: Pediatric neurosurgeons serving as institutional liaisons to the Hydrocephalus Clinical Research Network (HCRN), or its implementation/quality improvement arm (HCRNq), were invited to…

METHODS: We reviewed all cases of caudal appendage encountered by pediatric neurosurgeons at Children's Hospital Colorado since 2000 in which the appendage would be classified as a true tail by the most commonly used system mentioned above. Current Use of Intraoperative Neurophysiology in Neurosurgery: Supratentorial Part 1.

The general aim of the neurosurgical practice is to both anatomically and physiologically preserve functional neurological structures to ensure a higher quality of life. Intraoperative neuromonitorization (IONM) helps the neurosurgeon physiologically identify and assess the functional integrity of said neurological structures. The uses of IONM in neurosurgery practice are categorized into three areas; brain (supratentorial and infratentorial), brain stem, and spinal. For every anatomical region and surgical procedure, characteristic differences in electrophysiological methods exist for both re

Role of the neurologist in the evaluation and treatment of patients with trigeminal neuralgia.

The neurologist, although not usually the first healthcare provider to evaluate a patient with trigeminal neuralgia (TN), is often involved in confirming the diagnosis and managing the patient's pain with medications. The neurologist has several other important roles for patients with TN: assessing and reducing the individual's pain, patient and family education, and encouraging referral to a neurosurgeon for pain-reducing procedures when appropriate. In general, surgical procedures for TN should be considered when the patient does not attain pain relief after adequate trials of two or three m

Unusual penetrating cranio-orbital injury by a cut-off wheel.

The rare case of a penetrating cranio-orbital injury and the surgical treatment is presented. A 38-year-old woman was brought to the Emergency Unit of the University of Leipzig Hospital after suffering a severe craniocerebral injury from a broken cut-off wheel. A computed tomography (CT) scan demonstrated the entrance of the cut-off wheel with extension from the left sinus maxillaris and frontalis through the median part of the left-sided orbit to the anterior skull base. After removing the cut-off wheel and metal splinters, the neurosurgeon performed an osteoplastic bifrontobasal trepanation

[A double abscess of the cerebellum of otogenic origin].

Brain abscess is a rare, extremely aggressive lethal infection. It is especially important in case of otogenic abscesses. It is known that the problem of otitis existed already thousands of years before Christ, and it is still a current problem. Otogenic brain abscess is the most common otogenic complication encompassing 40-80% of all brain abscesses in adults. In 50-80% of these cases COM (chronic otitis media) with cholesteatoma is found. In the last 20 years CT is of the first rank in diagnosis. Brain abscess can be treated conservatively and surgically; but the ear has to be treated operat

Spinal markers: A new method for increasing accuracy in spinal navigation.

Spinal navigation opens up a completely new dimension in the planning and realization of neurosurgical and orthopedic procedures, and offers the possibility of simulating the operation preoperatively. There is currently only limited experience with spinal navigation, and despite the development of advanced software, intraoperative difficulties include identification of characteristic and reproducible anatomical landmarks, localization of these points in the surgical field, referencing, and intraoperative control. We report the use of a new kind of implantable fiducial marker in a case of a 58-

Transsphenoidal surgery of parasellar pituitary adenomas.

A series of 147 transsphenoidal operations for parasellar pituitary adenomas performed in a 4.5 year period is presented. The clinical and radiological features of the tumours are described and a new practical classification for the neurosurgeon is introduced. The results of surgery are analyzed with special attention to normalization rates in endocrinologically active adenomas and to the recovery rate of optomotoric nerve lesions in 10 of these cases. While in intra- and parasellar microadenomas the surgical normalization rates are comparable to those not extending beyond the confines of the

Management of orbital-cranial trauma.

Orbital-cranial injuries have the greatest potential for death and disability of any condition treated by the ophthalmologist. An object that penetrates through the orbit into the brain may leave only a small entrance wound. Patients can have normal vision, neurologic exam, and plain x-rays despite trauma that may lead to meningitis, brain abscess, or pneumocephalus. The CT scan greatly aids in both the early and late management of blunt and penetrating orbital-cranial trauma. The detection of pneumocephalus may be the only clue that intracranial penetration has occurred. Blunt trauma can caus

Practical MRI interpretation of intracranial metastases before and after multidisciplinary management: a pictorial review.

Intracranial metastases represent the most common form of brain tumors in adults. Their diagnosis and post-treatment evaluation are routine tasks in neuroradiology. In this setting, radiologists collaborate with neuro-oncologists, neurosurgeons, radiation oncologists, and other specialists to guide multidisciplinary treatment strategies and assess their response. A well-appointed radiologist should therefore be familiar with current treatment paradigms and their respective manifestations on imaging. This pictorial review begins by reviewing imaging findings of intracranial metastases, emphasiz…

[Expert consensus on the assessment of brain injury and determination of brain death during extracorporeal membrane oxygenation support (2025 edition)].

Extracorporeal membrane oxygenation (ECMO) serves as a pivotal life-support modality. However, the associated increased brain injury and its prognosis and outcome have raised significant concern. To address these challenges, an interdisciplinary task force comprising neurologists, neurosurgeons, intensivists, emergency physicians, pediatricians, anesthesiologists, pharmacists, and medical ethics physicians collaboratively developed the "Expert consensus on the assessment of brain injury and determination of brain death during extracorporeal membrane oxygenation support". What should otolaryngologists know about dural venous sinus stenting?

Dural venous sinus stenting is an emerging and exciting area in otolaryngology in collaboration with neurosurgeons and neuroradiologists. The first cases were reported 20 years ago. It is now considered part of the routine treatment of increased intracranial pressure due to transverse sinus stenosis. ENT doctors are the first to see these patients in their clinics, as sinus headaches, pulsating tinnitus, and dizziness are the most common symptoms. Previously, with limited success, high-dose diuretics and intracranial shunts had been the only options for treating these patients. Other methods, …
