Blue rubber bleb nevus syndrome: a case report with long-term follow-up.

Blue rubber bleb nevus syndrome is an uncommon condition manifested by gastrointestinal and skin hemangiomas that lead to gastrointestinal bleeding and anemia. The purpose of this report is to present a case with long-term follow-up. The patient is a 37-year-old female with a life-long history of blue rubber bleb nevus syndrome. She underwent multiple resectional operations that combined to give her a partial gastrectomy, partial small bowel resection, total abdominal colectomy, and end ileostomy. She continues to need endoscopy with sclerotherapy. In addition, she has iron-deficiency anemia, …

Chronic subcutaneous octreotide decreases gastrointestinal blood loss in blue rubber-bleb nevus syndrome.

BACKGROUND: A patient affected by blue rubber-bleb nevus syndrome had chronic gastrointestinal bleeding requiring weekly blood transfusions. Despite multiple surgical and endoscopic procedures to treat the venous malformations, the patient continued to bleed primarily from lesions in the small bowel. Therefore, this patient was treated with octreotide, a somatostatin analog known to decrease splanchnic blood flow and that is used for acute and chronic gastrointestinal bleeding. METHODS: Octreotide therapy, 5.7 microg/kg subcutaneously twice daily, was initiated, and the patient was followed u…

Giant congenital nevomelanocytic nevus with satellite lesions, vitiligo and lipoma : a rare association.

A case of giant congential nevomelanocytic nevus with satellite lesions, vitiligo and lipoma is described, owing to the rarity of the association. The constellation of finding can be explained on the basis of a defect in the neural crest, which is considered to be a common origin of melanoblasts, Schwann cells, sensory ganglia, bone, fat, muscle and blood vessels.

Treatment of a Becker's nevus using a 694-nm long-pulsed ruby laser.

Becker's nevus is an uncommon pigmented smooth muscle hamartoma that develops during adolescence and occurs primarily in young men. The nevus is characterized by hypertrichosis and hyperpigmentation and is usually located unilaterally over the shoulder, upper arm, and scapula. We describe a patient with a Becker's nevus who was treated with a long-pulsed ruby laser in order to decrease hair density and pigmentation.

[The basal cell nevus syndrome. Case report of a 14-year-old girl].

"Basal cell naevus syndrome" (Gorlin's syndrome) is a sporadic autosomal dominant hereditary precancerous condition which affects several organ systems. The dominating clinical manifestations are multiple basal cell naevi which develop into malignant basocellular carcinoma. Further abnormalities include abnormalities of the vertebrae and ribs, odontogenic keratocysts, calcification of the falx cerebri, a special facial appearance with progeny and macrocephaly. Affected patients may develop also ovarian fibromas, fibrosarcomas, cardiac fibromas, medulloblastomas and meningiomas. Lymphatic and c

Cellular blue nevus ("melanocytoma") of the spinal meninges: electron microscopic and immunohistochemical features.

A primary cellular blue nevus (melanocytoma) of the spinal canal in a 21-year-old woman is reported. Light microscopic examination revealed a melanotic neoplasm with histological patterns resembling schwannoma, dermal nevi, and neuroblastic-like tumor. The ultrastructural features of the neoplastic cells were similar to those in dermal blue nevi and melanomas. There was no evidence of arachnoidal cell differentiation. Immunohistochemistry revealed positive reactions for S-100 protein and neuron-specific enolase in many cells and no reactions for glial fibrillary acidic protein, cytokeratins, e…

Prevalence of congenital-nevus-like nevi, nevi spili, and café au lait spots.

To determine the clinical prevalence of medium-sized (1.5- to 19.9-cm-diameter) congenital-nevus-like nevi (CNLN), a consecutive series of 601 patients (mostly adults) had total cutaneous examinations. In this series, 15 (2.5%) were found to have such lesions. In addition, 14 (2.3%) had nevi spili and 83 (13.8%) had café au lait spots. All three types of lesions were equally represented in both sexes and tended to spare the head, neck, and upper extremities. Compared with CNLN, nevi spili were found to have significantly larger diameters and lower mean age, suggesting that these are different …

Woolly hair nevus. Two case reports and a discussion of unruly hair forms.

Woolly hair is very curly hair that is characteristically unruly and will not form naturally into locks. Woolly hair nevus first appears when the scalp hair begins to grow. It is seen as a patch of unruly, very curly hair, arising in a field of otherwise unremarkable scalp hair. Woolly hair can also be seen as a hereditary trait involving the entire scalp and in acquired forms. It may be distinguished from two other types of unruly hair, pili torti and spun glass hair.

Benign Proliferative Nodules in a Giant Congenital Melanocytic Nevus: Diagnostic Challenge and Management.

Congenital melanocytic nevi are cutaneous malformations present at birth, resulting from a benign proliferation of melanocytes. Although generally benign, they can have significant psychological impacts and carry a non-negligible risk of progression to malignant melanoma. We report a case of a giant congenital melanocytic nevus associated with a perianal mass in an infant, two and half-month-old infant, with no notable medical history, born at term. He was referred for a pigmented perianal swelling located on a large hyperpigmented plaque covering the lower back, buttocks, perineum, external g…

Nevus Unius Lateris With Epidermolytic Hyperkeratosis Managed With Radiofrequency Ablation: A Case Report.

Nevus unius lateris is a rare, systematized clinical variant of verrucous epidermal nevus, characterized by extensive unilateral verrucous lesions following the lines of Blaschko. While most epidermal nevi are localized, the systematized form often raises concerns for epidermal nevus syndrome, which involves associated multisystem abnormalities in the skeletal, ocular, and neurological systems. We present the case of an 18-year-old male with extensive, hyperpigmented, verrucous plaques involving the left side of his trunk, upper limb, and lower limb without associated extracutaneous abnormalit…

A Congenital Melanocytic Nevus With Halo Phenomenon and Complete Regression Following Surgical Excision.

We present a case of a 4-year-old girl who underwent two planned staged surgical resections of a benign-appearing medium-sized congenital melanocytic nevus (CMN) on the right buttock. The nevus was 4 cm in diameter, medium brown in color, and exhibited central hypertrichosis with mild cobblestoning. Ten days after the first excision, a striking depigmented halo was noted around the residual nevus, which over time progressed to complete clinical regression of the residual CMN. While spontaneous development of the halo phenomenon in CMNs has been previously reported, no cases of this immune resp…

Blue Nevi and Melanoma Arising in Blue Nevus: A Comparative Histopathological Case Series.

Background and Clinical Significance: Blue nevi are a dubious pigmented lesion. While somewhat common throughout the population, they are significantly less common than other melanocytic neoplasms, and both their morphology and development bring them closer to true hamartomas than neoplasms. An exceedingly rare occurrence is the development of melanoma from a preexisting blue nevus. This nosological unit, defined as melanoma arising in a blue nevus, also known as malignant blue nevus, blue naevus-like melanoma, melanoma ex-blue naevus, and melanoma mimicking cellular blue naevus, is required t…

Anti-BCL2 therapy eliminates giant congenital melanocytic nevus by senolytic and immune induction.

Giant congenital melanocytic nevus (GCMN) is a RAS/RAF mutation-driven syndrome characterized by extensive melanocytic lesions, posing psychological challenges and a lifelong risk of malignancy. Existing treatments like surgical resection and laser therapy fail to fully remove lesions, and MAPK inhibitors show limited efficacy. This study identified a predominant population of senescent cells and a minority of proliferative cells in GCMN, necessitating dual-targeted strategies. We found that the anti-apoptotic protein BCL2 is expressed in both senescent and proliferative cells from GCMN patien…

Nevus-Associated and De Novo Melanoma: A Cross-Sectional Study on Prognostic Differences.

Background/Objectives: Melanomas may develop de novo or in association with a pre-existing nevus (nevus-associated melanoma, NAM). Whether these subtypes differ in their clinical and biological behavior remains uncertain. We aimed to compare the clinicopathologic features and outcomes of NAM and de novo melanoma (DNM) in a large single-center cohort. Methods: We retrospectively analyzed 378 patients with invasive melanoma diagnosed between 2007 and 2021 at a tertiary referral center. Tumors were classified as NAM when histopathologic continuity with a nevus was present, and as DNM otherwise. C…

Novel Finding of PLAG1 Overexpression and COL3A1::PLAG1 Rearrangement in Nevus Lipomatosus Superficialis.

Nevus lipomatosus superficialis (NLS) is an uncommon benign skin lesion and characterized by aggregates of mature adipose tissue ectopically among the collagen bundles of the dermis. The precise etiology of NLS remains poorly understood, and it is classified as a cutaneous hamartoma in the WHO Classification of Skin Tumors, 5th edition. This is a case report of recurrent classical Hoffmann-Zurhelle form of NLS on a 3-year-old male child's hand. Immunohistochemical stains revealed diffusely PLAG1 positive expression in the lesional stellate stromal cells and adipocytes. Ancillary molecular test…
