Combined Effects of Early Mobilization and Nutrition on ICU-Acquired Weakness.

Intensive Care Unit-Acquired Weakness (ICUAW) is a very common condition in patients admitted to intensive care units (ICUs), even after rel … n of early and minimal rehabilitation-including passive mobilization-when combined with appropriate nutritional support, can be a valuable strategy to help reduce the incidence of ICUAW. In this narrative review, we aim to summarize the current scientific knowledge on this topic, emphasizing the importance of an integrated approach that combines nutrition and early m …

Acid-base disorders: learning the basics.

Nutrition support practitioners should be confident in their ability to recognize and treat various metabolic and respiratory disorders encountered in daily practice. A clinician's comprehension of the underlying physiologic processes and/or exogenous causes that occur during acid-base disorders is essential when making therapeutic decisions regarding fluids, parenteral nutrition, and electrolyte management. This invited review will discuss basic metabolic and respiratory disorders while briefly addressing mixed acid-base disorders.

Nutrition-focused evaluation and management of dysnatremias.

Alterations in sodium and water balance are commonplace and often occur concurrently. Dysnatremias (hyponatremia and hypernatremia) merely reflect a relative excess or deficit of plasma water in relation to plasma sodium. There is no predictable relationship between plasma sodium concentration, extracellular fluid volume, and total body sodium content. Evaluation and management of dysnatremias rely on an understanding of key terminology, normal body fluid composition and distribution, and regulation of water and sodium balance, combined with a directed history and physical and appropriate labo

Necrotizing pancreatitis: diagnosis and management.

Necrotizing pancreatitis is a severe disease characterized by gland necrosis and a destructive systemic inflammatory response. Early management involves aggressive resuscitative and supportive measures. Outcomes are primarily determined by the presence of late secondary bacterial infection of the necrotic gland. Early empiric antibiotics and late surgical necrosectomy in the appropriate setting are the keys to managing these sick patients. With appropriate management, mortality can be minimized and long-term quality of life may be restored.

Refeeding syndrome in early pregnancy. Case report.

Refeeding syndrome is a very serious disorder that is not often observed today, as it is more common during times of mass starvation or war. Nowadays, it is sometimes found in patients suffering from anorexia nervosa or neoplastic diseases. A case recently treated in our Intensive Care Unit is described. The patient was pregnant and suffering from Crohn's disease. It is emphasized that although refeeding syndrome is often fatal if not treated early, it is easily prevented or treated with adequate nutritional support.

Sustained-release boluses to supply trace elements and vitamins to calves.

Cylindrical rumen boluses of a size (55 mm length x 18 mm diameter, density 2.9 g cm(-3)) appropriate for ruminating calves over 75 kg liveweight were constructed. The mean daily nutrient releases from two rumen boluses measured in rumen-cannulated cows were 62 mg Cu, 0.56 mg Se, 1.08 mg Co, 1.14 mg I, 38 mg Mn and 55 mg Zn with vitamins 3140 IU A, 628 IU D3 and 9 IU E. In a 123-day experiment with housed dairy calves given hay and a barley/soyabean meal mix, administration of two rumen boluses significantly increased plasma copper and blood glutathione peroxidase levels compared with unsupple

Copper, selenium, and zinc status and balances after major trauma.

To investigate the trace elements (TE) losses and status after trauma, 11 severely injured patients (Injury Severity Score: 29 +/- 6), admitted to the ICU were studied from the day of injury (D0) until D25. Balance studies were started within 24 hours after injury, until D7. Serum and urine samples were collected from D1 to D7, then on D10, 15, 20, and 25. Intravenous TE supplementation was initiated upon admission. SERUM: Selenium (Se) and zinc (Zn) levels were decreased until D7 and were normal thereafter. LOSSES: TE urinary excretions were higher than reference ranges until D20 in all patie

Chronic respiratory failure after lung resection: the role of pulmonary rehabilitation.

Pulmonary rehabilitation gradually has become the gold standard treatment for patients with severe lung disease, especially COPD. By definition, rehabilitation services are provided to patients with symptoms, most of whom have moderate-to-advanced lung disease. Because new therapeutic strategies, such as lung volume-reduction surgery and lung transplantation, require well-conditioned patients, pulmonary rehabilitation is becoming a crucial component of the overall treating strategy of many patients who heretofore were deemed untreatable. The positive results in several randomized trials have d

Benefits of home parenteral nutrition before lung transplantation: report of a case.

A 54-year-old woman, who was severely malnourished and awaiting lung transplantation for chronic respiratory failure, was given nutritional support by home parenteral nutrition (HPN). The patient's emaciated state was further compromised by the fact that she had undergone a distal gastrectomy 4 years earlier. We decided that HPN was indicated because her oral intake was so poor and drinking enteral formula reduced her appetite. After 1 year of HPN, her body weight increased and her nutritional, anthropometric, and biochemical values were well maintained by the time of transplantation. Moreover…

Nutritional status and the risk of malnutrition in older adults with chronic kidney disease - implications for low protein intake and nutritional care: A critical review endorsed by ERN-ERA and ESPEN.

Increased life expectancy is posing unprecedented challenges to healthcare systems worldwide. These include a sharp increase in the prevalence of chronic kidney disease (CKD) and of impaired nutritional status with malnutrition-protein-energy wasting (PEW) that portends worse clinical outcomes, including reduced survival. In older adults with CKD, a nutritional dilemma occurs when indications from geriatric nutritional guidelines to maintain the protein intake above 1.0 g/kg/day to prevent malnutrition need to be adapted to the indications from nephrology guidelines, to reduce protein intake i

Clinical nutrition for critically ill patients: a practical appraisal of guideline gaps and perspectives for future research.

Malnutrition and accelerated muscle wasting are frequent during critical illness and make nutritional support a central component of ICU care. Nutritional support is therefore central to ICU care, but its optimal timing, route, dose, and composition remain debated. Recent randomized trials have challenged the traditional objective of early full feeding and shifted practice toward progressive, phase-adapted, and tolerance-based nutrition. This narrative review compares current international guidelines for nutritional support in critically ill adults. Guidelines broadly agree that nutrition shou…

Esophageal strictures in children with recessive dystrophic epidermolysis bullosa: experience of balloon dilatation in nine cases.

BACKGROUND: Recessive dystrophic epidermolysis bullosa is a rare disorder characterized by extreme vulnerability of the squamous epithelium and mucous membranes. Minor trauma such as is caused by swallowing solid food is followed by blistering and scarring. Stricture formation at the pharyngoesophageal junction (C6 or C7) is the severest complication of this disease. METHODS: We evaluate the effectiveness of Microvasive Rigiflex balloon dilatation and extensive nutritional support as a primary treatment for this condition. Nine of 21 recessive dystrophic epidermolysis bullosa patients develop…
