Subacute hematogenous osteomyelitis: are biopsy and surgery always indicated?

Forty-four consecutive cases of subacute osteomyelitis admitted at our institution over a 12-year period were retrospectively reviewed to assess the effectiveness of conservative versus surgical treatment of this condition and to determine the indications for open biopsy and surgical debridement. Twenty-four cases were treated with antibiotics only, and 20 had surgical debridement followed by antibiotics. Except for one case that received inadequate antibiotic therapy, all patients responded well to this treatment, whether conservative or surgical. At an average follow-up of 18 months, there w…

Sclerosing osteomyelitis and Actinomyces naeslundii infection of surrounding tissues.

We present the case of a young man with chronic Actinomyces naeslundii infection in the lower leg who underwent a below-the-knee amputation after unsuccessful attempts at diagnosing and eradicating the infection. Actinomycetic organisms from environmental sources can engender a dense fibrotic tissue response without fistulae. Such a response led to a painful, sclerosing, nonsuppurative infection of the bone and surrounding tissues in our patient. This clinical picture, at least the bony and periosteal reaction, was compatible with Garrè's osteomyelitis.

Maxillary osteomyelitis and spontaneous tooth exfoliation after herpes zoster.

Reports of spontaneous tooth exfoliation and osteonecrosis trigeminal herpes zoster are extremely rare and have been sporadic. This article reports a pertinent case of a 50-year-old man who exhibited prodromal odontalgia before the appearance of vesicular mucocutaneous lesions, together with severe destruction of the maxillary bone and exfoliation of multiple teeth. This patient was successfully treated using a unique closed nasal-vestibular drainage system for the ultimate control of maxillary bone viability. A review and analysis of the clinical aspects and the pathogenesis of herpes zoster

Vertebral osteomyelitis and aortic lesions: case report and review.

Coexistence of vertebral osteomyelitis and lesions of the aorta is rare but may be lethal if not diagnosed promptly and treated effectively. We describe a patient who was treated at the Cleveland Clinic Hospital, and we review 69 additional cases reported in the literature. The native aorta was involved in 66 cases; four patients developed infection of prosthetic aortic grafts. The most common aortic lesions associated with vertebral osteomyelitis were mycotic aneurysms, infected aneurysms, and pseudoaneurysms. The wide variety of pathogens involved included salmonellae and other gram-negative…

Chronic progressive osteoblastic osteomyelitis: a new approach to treatment.

The clinical, histologic and radiographic picture in 16 cases in 15 patients with nonfistulating chronic progressive osteoblastic osteomyelitis of a long tubular bone which started in childhood is presented. All patients had severe pain at rest or weightbearing. Histology showed an increase of active osteoblasts. Low virulent bacteria were found in one-fourth of the patients. To reduce the osteoblastic activity, nonsteroidal antiinflammatory treatment was combined with surgical and antibiotic treatment. Eleven patients were pain-free at follow-up (average 47 months), 3 had reduced pain, and 1 …

[Complex treatment of patients with chronic osteomyelitis using osteoplasty].

The work presents results of free bone autoplasty in 312 patients with chronic osteomyelitis of long bones. Complications were observed in 26.9% of the patients in the postoperative period. With regard for reoperations performed in patients with a complicated course of the disease, sanitation of pyonecrotic foci was achieved in 94.4% of the observations. Labour rehabilitation after operations of bone plasty was obtained in 65% of the patients.

CT detection of sacral osteomyelitis associated with pelvic abscesses.

In three patients the diagnosis of sacral osteomyelitis was made when CT demonstrated intraosseous (two) and intraforaminal (one) gas. Two of the three patients also had radionuclide bone scans, one of which was unremarkable. In the other case, radionuclide scintigraphy greatly underestimated the extent of the disease process when compared with CT. All three patients had contiguous pelvic abscesses as a cause of the osteomyelitis. Although there was a high clinical suspicion for an intraabdominal process, the diagnosis of superimposed osteomyelitis of the sacrum was unsuspected. The detection …

Acute osteomyelitis of index finger caused by dog bite.

A forty-seven-year-old man was bitten by a dog on his left index finger. Despite initial antibiotic treatment, he developed acute osteomyelitis of the middle phalanx over the next three weeks. Pasteurella multocida and Bacteroides were isolated from the necrotic bone. Subsequently the infection was successfully treated by debridement and Tetracycline.

Bacterial endocarditis presenting as acute vertebral osteomyelitis: 14 cases.

Association between bacterial endocarditis (BE) and vertebral osteomyelitis (VO) has infrequently been noted. In a retrospective analysis of BE (280 cases) and VO (150 cases) 14 cases were found to have this association. There were 12 males and 2 females, ages ranging from 39 to 72 years, mean age 56.6. Blood cultures were positive for Streptococcus viridans (6 cases). Str. faecalis (4 cases), staphylococcus (2 cases), Gram negative bacteria (1 case). Organism was not isolated in one case. Fever and severe back pain antedate the diagnosis of VO 3.5 and 2.5 months. X rays films of the spine and…

Hematogenous osteomyelitis with fat-fluid level shown by CT.

Computed tomography (CT) revealed fat-fluid (pus) levels in two adult patients who had hematogenous osteomyelitis due to gram-negative organisms: in one patient within an intraosseous cavity, and in another patient within the adjacent suprapatellar bursae. Both patients were hospitalized for other chronic illnesses and had confusing clinical manifestations with non-specific radiographic changes. Observation of fluid levels, in addition to bony erosions, correctly suggested the presence of pus, and in one patient prompted a diagnostic intraosseous aspiration. To our knowledge, this CT sign of h…

An orbital fistula complicating anaerobic frontal sinusitis and osteomyelitis.

A patient is described with an orbital fistula complicating frontal sinusitis and osteomyelitis of the frontal bone. The fistula was excised, but a fortnight later an acute exacerbation occurred. From the discharging pus a Staphylococcus aureus was cultured and from mucosa obtained during surgery a microaerophilic Streptococcus. These findings led to the diagnosis: synergistic bacterial inflammation of the frontal sinus, with osteomyelitis and orbital cellulitis.

[Malignant T-cell lymphoma with osteomyelitis-like bone infiltration].

After a short review on the late literature, existing about various forms of acute lymphoblastic leucemias, it is reported on a rare case of malignant T-cell-Lymphoma with ostemyelitis-like, painfull bone infiltration. The clinical symptoms, as well as differential-diagnostic criterias to other leucemias are described.

Metastatic bronchogenic carcinoma to the finger simulating acute osteomyelitis.

A case of metastatic bronchogenic carcinoma to the distal phalanx of the ring finger was initially misdiagnosed as osteomyelitis. The presence of constitutional symptoms, past history of malignancy, the milder and relatively protracted clinical course, the negative culture and lack of response to surgical drainage and antibiotics should make the diagnosis of hand metastasis a distinct possibility. Amputation of the involved digit is an effective palliative treatment and the other foci of metastases should also be treated with systemic chemotherapy.

[Malignant degeneration of fistulas in chronic osteomyelitis (author's transl)].

The malignant degeneration of a skin ulcer has been reported as early as in 1884. In spite of this, however, the danger of malignant degeneration of sinus tracts associated with recurrences of chronic osteomyelitis is still underestimated. This development is neither restricted to case histories of years and decenniums nor to elderly patients and may also involve any part of the skeleton. Usually patients become aware of suddenly increasing tenderness and purulation, "washed out" structures of osteolytic areas are demonstrable radiographically. Diagnosis is established by mandatory histologica…

Gram-negative osteomyelitis following puncture wounds of the foot.

Seven cases of gram-negative osteomyelitis following puncture wounds of the foot are presented. Regardless of age (range, five to thirty-two years) all patients had a similar, typical clinical picture of one to two weeks of improvement after the initial post-traumatic inflammation followed by worsening of the local signs. Two patients were successfully treated with antibiotics alone and five, with antibiotics in combination with curettage.
