Development of a hidradenitis suppurativa patient decision aid.

BACKGROUND: Patient decision aids (PDAs) are tools that facilitate informed shared decision-making between patients and health care providers. To address a previously identified need in treatment decision-making in hidradenitis suppurativa (HS), we developed an HS-PDA. METHODS: Development of the HS-PDA was based on International Patient Decision Aids Standards. Evidence was derived from the North American Clinical Management Guidelines for HS. RESULTS: Content from guidelines was transformed into patient-friendly language and reviewed by three physicians and two patient representatives. Fee

Patient-centered Treatment Decisions for Urethral Stricture: Conjoint Analysis Improves Surgical Decision-making.

OBJECTIVE: To determine whether the use of a choice-based conjoint analysis (CA) exercise decreased patients' decisional conflict about treatment preferences for surgical management of urethral stricture disease. Understanding patient preferences for treatment decisions assists in shared decision-making and emphasizes patient-centered outcomes. CA offers a method to understand what risks patients are willing to take for what gains. METHODS: The CA methodology was used by providing participants with case-based choices to elucidate the relative importance that individuals place on various treat

The effect of patient involvement in surgical decision making for carpal tunnel release on patient-reported outcome.

PURPOSE: To investigate whether patient-reported outcomes are different according to patients' preference or experience in surgical decision making for carpal tunnel release. METHODS: We preoperatively surveyed 85 patients who underwent carpal tunnel release regarding their preferred role in the process of surgical decision making and assessed their experienced role in the actual decision making 6 months after surgery using a Control Preference Scale. For patient-reported surgical outcomes, we used the Disabilities of the Arm, Shoulder, and Hand questionnaire. We compared these outcomes with

The impact of newer atypical antipsychotics on patient-reported outcomes in schizophrenia.

Over the past two decades there has been increasing interest in including patients' self-reports in the management of their illness. Among the many reasons for such recent interest has been a rising consumer movement over the past few decades, which has led patients, their caregivers and their families to press for more meaningful sharing with physicians in the clinical decision-making process, with the clear expectations of better therapies and improved outcomes. Patients as consumers of services, their views, attitudes towards healthcare, as well as their level of satisfaction with care, hav

From adherence to self-determination: evolution of a treatment paradigm for people with serious mental illnesses.

Treatment adherence and nonadherence is the current paradigm for understanding why people with serious mental illnesses have low rates of participation in many evidence-based practices. The authors propose the concept of self-determination as an evolution in this explanatory paradigm. A review of the research literature led them to the conclusion that notions of adherence are significantly limited, promoting a value-based perspective suggesting people who do not opt for prescribed treatments are somehow flawed or otherwise symptomatic. Consistent with a trend in public health and health psycho

Pattern of participation in a cohort aged 50-60 years at first invitation to the service-screening programme with mammography in Stockholm county, Sweden.

BACKGROUND: Mammography screening can only be effective if a high proportion of the invited women attend. The aim of the present study was to search for patterns of participation in women who were invited to five consecutive rounds in a large screening programme. METHOD: Stockholm service-screening programme started 1989 and invites women between 50 and 69 years of age to mammography with 2 years interval. 64,852 women were eligible for an invitation to all five rounds during a 10-year period and were included in the study. RESULTS: More than 50% attended at all five rounds, and more than 70

Barriers and opportunities to improve shared decision-making for patients with ulcerative colitis: a qualitative study.

BACKGROUND: Shared decision-making (SDM) for ulcerative colitis (UC) is complex and often occurs under challenging clinical circumstances. Although SDM can enhance patient-centered care, there is limited understanding of how patients experience this process, particularly across diverse populations. This study aimed to explore barriers and facilitators to SDM among patients who underwent surgery for UC. METHODS: This study was a multisite qualitative study using semistructured interviews with patients with UC who underwent surgery. Participants were recruited through inflammatory bowel disease

Improving Enhanced Recovery after Surgery (ERAS): The Effect of a Patient-Centred Mobile Application and an Activity Tracker on Patient Engagement in Colorectal Surgery.

BACKGROUND: The Enhanced Recovery After Surgery (ERAS) protocol improved perioperative colorectal care. Although the protocol is firmly implemented across hospital settings, there are benefits to gain by actively involving patients in their recovery. The main objective of this study was to investigate whether compliance with selected items in the ERAS protocol could further improve by using a patient-centred mobile application. METHOD: This multicentre, randomised controlled trial was conducted between October 2019 and September 2022. Patients aged 18 years or older who underwent elective col

Interest in initiating an early phase clinical trial: results of a longitudinal study of advanced cancer patients.

OBJECTIVE: Enhanced recruitment of patients with advanced cancer (ACP) to early phase (EP) trials is needed. However, selective recruitment may affect the kinds of patients who are recruited. To address whether ACP who initiate EP trial enrollment differ from those who do not, we prospectively surveyed ACP well in advance of potential trial recruitment and followed them over time to identify those who initiated the recruitment process. METHODS: EP trial initiation was defined as a patient being referred for screening to an active EP trial. Depression and anxiety were assessed with the Patient

The influence of patient-related variables in the timing of cataract extraction.

Data from a prospective study of quality of life after cataract extraction with intraocular lens implantation were used to examine associations between patient factors and the timing of cataract extraction. The 495 patients were drawn from 76 ophthalmologists' offices in three cities. Patients were categorized as waiting three months or less or four months or more to schedule their operations after their physicians' recommendations. Patients who delayed scheduling their operations were more likely to be younger, have higher incomes, be working full-time, have higher life satisfaction, have low

[Participation to organized screening programs: Individual and collective stakes].

The evaluation of the risk-benefit balance in public health is an ethical commitment, based on the imperative of not harming people. Screening is a probabilistic approach, which, in order to serve the needs of a few, involves the largest number to share the potential harmful effects. Improvement of scientific knowledge and opening of the societal debate modified the relation between collective and individual stakes over time. This article traces the evolution of the discourse surrounding cancer screening, particularly following the controversy surrounding breast cancer screening. Within the fr

Measuring What Matters: Redesigning Evaluation With Consumers to Understand Engagement in Hand Therapy Using Virtual Reality.

INTRODUCTION: Consumer involvement is essential to ensure health service evaluations reflect what matters to those receiving care. This service improvement project describes how the evaluation of virtual reality (VR) in outpatient hand therapy was redesigned in partnership with consumers to better capture engagement with hand therapy. METHOD: The evaluation was undertaken in an outpatient hand therapy service. Initial evaluation relied on standardised quantitative measures, which did not address the original purpose of implementing VR in the treatment setting. Through collaboration with consu

Mechanisms of Engagement With Mobile Health Apps for Adults With Long-Term Conditions: Overview of Systematic Reviews.

BACKGROUND: Engagement is a necessary precondition for the effectiveness of mobile health (mHealth) apps for long-term physical health conditions (LTCs), particularly as health systems increasingly prioritize the deployment of scalable, self-guided digital interventions. Outside controlled research settings, where clinician involvement often drives engagement, little is known about whether, how, and why people engage with mHealth apps based on intrinsic motivation alone. Existing systematic reviews have cataloged behavioral engagement indicators but rarely assess the mechanisms underlying enga

Barriers and facilitators to hepatitis C patient engagement: Interview study with general practitioner champions in England.

There are people living with a previously diagnosed chronic hepatitis C infection who have been lost to follow up, and people who may have undiagnosed hepatitis C. General Practitioners (GPs) could help to support these patients into care, via offering testing, referring to secondary care, and facilitating prescribing. We conducted semi-structured interviews with 14 GPs, most of whom work with local Operational Delivery Networks (ODNs) with funding from the NHS England Viral Hepatitis Programme as hepatitis C GP Champions. A thematic analysis with a multi-level model (system, provider-levels)
