Chronic myeloid leukemia (CML) is a myeloproliferative neoplasm characterized by overproduction of myeloid white blood cells. Philadelphia chromosome is an essential finding for CML diagnosis. Generally, a clinical diagnosis of essential thrombocythemia (ET) can be established from isolated marked thrombocytosis in peripheral blood. The emergence of clonal chromosomal abnormalities in Philadelphia-negative cells during treatment with imatinib in patients with Philadelphia-positive chronic myeloid leukemia has been reported. Malignancy occurs in 2-4% of children infected with human immunodeficiency virus (HIV). The most commonly reported type is non-Hodgkin's lymphoma. We describe the first reported case of chronic myelogenous leukemia (CML) in a child infected with HIV. The initial diagnosis was consistent with Philadelphia chromosome positive acute lymphocyte leukemia. However, on review of the patient's history and the persistence of the Philadelphia chromosome, the diagnosis of CML was made. CML needs to be added to the list of malignancies seen in patients with HIV.

In the United States in the late 19th and early 20th century, large cities had extremely high death rates from infectious disease. Within major cities such as New York City and Philadelphia, there was significant variation at any point in time in the mortality rate across neighborhoods. Between 1900 and 1930 neighborhood mortality convergence took place in New York City and Philadelphia. We document these trends and discuss their consequences for neighborhood quality of life dynamics and the economic incidence of who gains from effective public health interventions.

We report the case of a 19-year-old girl in whom a Ph1-positive chronic myeloid leukemia was diagnosed 10 months after an episode of infectious mononucleosis. Clinical and cytogenetical follow-up demonstrated the appearance of a variant Philadelphia chromosome translocation t(7;9;22) during a blastic crisis characterized by early B-lymphoid elements. The association between the t(7;9;22) variant Philadelphia and the lymphoid blast cells is emphasized, and the importance of Epstein-Barr virus (EBV) infection in the pathogenesis of the disease in this case is discussed.
