INTRODUCTION: Venous thromboembolism (VTE) is a frequent and serious emergency department (ED) diagnosis. When clinical risk stratification tools do not adequately explain the patient's presentation, physicians must consider less common underlying causes.
CASE: A 76-year-old male with chronic venous stasis presented to the emergency department with progressive bilateral leg swelling (left greater than right) and shortness of breath over one month, with acute worsening over 3 days. He was found to have pronounced asymmetric lower extremity edema and a new oxygen requirement. CT pulmonary angio

The left inferior phrenic vein (LIPV) is a major drainage vessel of gastric varices and serves as an important conduit in endovascular treatment for gastric varices. The narrowing of LIPV has been empirically demonstrated and sometimes hinders catheter insertion for the treatment of gastric varices. We herein investigated the morphology of narrowed LIPV in patients with portal hypertension. Venograms of LIPV on 25 patients with gastric varices (15 males; 10 females; age range, 45-79 years with a mean of 67 years) were retrospectively reviewed, the following four parameters were measured: the d

BACKGROUND: Evaluation of the transverse sinus stenosis (TSS) is essential for TSS-related diseases.
OBJECTIVE: To investigate a new method for the quantitative assessment of TSS based on the correlation between TSS and trans-stenotic pressure gradient (TPG).
METHODS: Patients with unilateral pulsatile tinnitus with or without idiopathic intracranial hypertension were retrospectively included. All patients underwent CT venography and venous manometry and were confirmed to have TSS. The cross-sectional diameter/area of TSS, the poststenotic and prestenotic segments, and the superior sagittal

Transvenous approaches for pacemaker and defibrillator lead insertion offer numerous advantages over epicardial techniques. Although the cephalic, axillary, and subclavian veins are most commonly used in clinical practice, they each offer their own set of advantages and disadvantages that leave their usage dependent on patient anatomy and physician preference. Alternative methods using the upper and lower venous circulation have been described when these veins are not available or practical for lead insertion. Until current technology is superseded by leadless pacing systems, the search for th

We report a case in which a novel three-dimensional (3D) electromagnetic cardiovascular navigation system (MediGude™, St. Jude Medical, St. Paul, MN, USA) was used to create a 3D reconstruction of the axillary vein and its anatomic course to guide its successful puncture and cannulation. The MediGuide system's projection accuracy has proved to offer a level of precision that is sufficient to achieve axillary vein access. This highlights its enormous possibilities to obtain 3D venous system reconstruction with minimal fluoroscopic use when performing cardiac or even non-cardiac procedures.

Mesenteric vein thrombosis (MVT) accounts for 5%-15% of all mesenteric ischemic events and is classified as either primary or secondary. Primary MVT is idiopathic, whereas secondary MVT can result from a variety of underlying diseases and risk factors, including primary hypercoagulable states or prothrombotic disorders, myeloproliferative neoplasms, cancer (most frequently of the pancreas or liver), diverse inflammatory conditions, recent surgery, portal hypertension, and miscellaneous causes such as oral contraceptives or pregnancy. Clinical symptoms of MVT are rather nonspecific and are main

Gonadal vein thrombosis is a rare but well recognized entity which predominantly occurs in the post partum period. It is also associated with gynecological malignancies, cesarean deliveries, abortions, hypercoagulability and pelvic inflammatory disease. Prompt diagnosis and treatment is warranted to avoid serious complications. We report the rare case of idiopathic, unprovoked gonadal vein thrombosis.

Ovarian vein thrombosis (OVT) is an uncommon entity typically seen in the post-partum, patients with pelvic surgery, infection, or inflammation, and hypercoagulabilty. Concurrent pulmonary embolism (PE) may occur in these patients; however, is an uncommon complication. Treatment commonly involves anti-coagulation and antibiotics in the setting of pelvic inflammatory disease. Presented is a case report of ovarian vein thrombosis leading to pulmonary embolism in the setting of malignancy, underscoring the importance of inspecting the gonadal vein during interpretation, particularly in the emerge

Two cases of gastric varices were treated by balloon-occluded retrograde transvenous obliteration via the pericardiophrenic vein at our hospital, and both were successful. One case developed left hydrothorax. Gastric varices did not bled and esophageal varices were not aggravated in both cases for 24-30 months thereafter. These outcomes indicate the feasibility of balloon-occluded retrograde transvenous obliteration via the pericardiophrenic vein.

Membranous obstruction of the inferior vena cava is a frequent cause of Budd--Chiari syndrome in Eastern countries. The study reports the case of a young male patient with Budd--Chiari syndrome caused by extremely rare multiple membranous obstruction of inferior vena cava. A percutaneous transluminal angioplasty with stent placement was performed successfully, accompanied with satisfactory resolution of clinical symptoms and signs. The graft was reported to be patent 2 years postoperatively.

Persistent left superior vena cava is a common congenital anomaly. We present angiographic pictures and hemodynamic tracings from a case of superior vena cava syndrome in a patient with this anomaly. The images depict a challenging percutaneous intervention for the amelioration of the hemodynamic problem, made even more interesting as the patient was a recipient of orthotopic heart transplant.

Patients with existing internal cardioverter defibrillators (ICDs) often require upgrading to a biventricular ICD for treatment of congestive heart failure. Placement of a left ventricular (LV) lead can be technically challenging in the best of circumstances. A subclavian vein stenosis or occlusion related to previously placed leads adds a major obstacle to a successful implant. We report a technique to implant an LV lead from the same side as the existing ICD system despite failed microdissection of a complete occlusion of the subclavian vein.

AIM: Vena cava filters are used for the prevention of pulmonary embolism in cases of contraindication, failure or complications of the anticoagulant therapy. There are no certain data in the literature concerning the effectiveness of vena cava filters and, above all, the possible long-term complications are not known. For this reason increasing attention is paid to permanent/removable filters that permit exploitation of the short-term advantages of vena cava interruption, eliminating the possible long-term complications. We have reported the results of a multicentre experience concerning ALN p

BACKGROUND AND OBJECTIVES: Biventricular pacing is a new modality of treatment for patients with drug-refractory heart failure and ECG evidence of dyssynchronous ventricular contraction. A third lead is required to be placed in a coronary vein through the coronary sinus for left ventricular pacing, in addition to the atrial and right ventricular leads. Subclavian crush phenomenon is associated with lead placement using subclavian puncture. Cephalic venous cutdown, free of this complication, provides vascular access which is probably too small for multiple leads placement in biventricular pacin

PURPOSE: The purpose of this study was the comparison of intravascular ultrasound scanning (IVUS) with transfemoral venography in the assessment of chronic iliac vein obstruction.
METHODS: IVUS and standard, single-plane, transfemoral venography were performed in 304 consecutive limbs during balloon dilation and stenting of an obstructed iliac venous segment. The appearance of the obstruction was described, and the degree of stenosis (maximal diameter reduction) was estimated with venography and IVUS. The stenotic area was derived with diameter calculations (pir(2)) and also was measured with

The presence of variant venous anatomy may increase the technical difficulty of percutaneous varicocele embolization. The authors review their experience performing venography of the left spermatic vein and varicocele embolization in 18 men with circumaortic left renal veins. Selective catheterization of the left spermatic vein was achieved in all but one patient, and all patients with positive venographic results underwent successful embolization. The site of the confluence of the left spermatic vein and the renal vein was variable and it was necessary to use jugular and femoral venous approa
