External validation of dementia risk models in a United Kingdom population-based prospective cohort among stroke survivors.

OBJECTIVES: Stroke-survivors are at a significantly elevated risk of post-stroke cognitive impairment and dementia, yet early identification of high-risk cases remains a challenge. Dementia risk models developed for the general population may not translate well to stroke populations due to unique pathophysiological mechanisms, risk factor profiles, and accelerated cognitive decline often observed after cerebrovascular events. The aim of this study was to assess the accuracy of existing dementia models in a population-based stroke cohort. STUDY DESIGN AND SETTING: This study externally validat

Evaluation of Cognitive Impairment in Refractory Temporal Lobe Epilepsy Patients Concerning Structural Brain Lesions.

INTRODUCTION: Temporal lobe epilepsy (TLE) is the most prevalent form of drug-resistant epilepsy with concurrent cognitive impairment. Prevention, earlier diagnosis, and personalized management of cognitive deficits in TLE require more understanding of underlying structural and functional brain Ialterations. No study has evaluated the performance of TLE patients in different cognitive domains based on their structural brain lesions. METHODS: In this study, 69 refractory TLE patients underwent magnetic resonance imaging (MRI) epilepsy protocol and several neuropsychological tests, consisting o

Causal inference in oral health research: Beyond statistical significance toward a causal framework.

BACKGROUND: Oral health research has adopted sophisticated statistical methods, yet studies routinely present adjusted associations as causal effects. The problem is not statistical competence but the absence of explicit causal reasoning. This commentary proposes a principled framework for causal reasoning in oral health research. METHODS: We conducted a narrative synthesis of foundational and recent literature on causal inference, drawing on texts describing the potential outcomes and counterfactual frameworks, the ICH E9(R1) regulatory addendum on estimands, study design, directed acyclic g

Expert consensus on the clinical and organizational use of VNS therapy for drug-resistant epilepsy in the United Arab Emirates: a single-round modified Delphi study.

BACKGROUND: Epilepsy affects over 50 million people worldwide, and approximately one third of patients develop drug-resistant epilepsy (DRE), a major driver of morbidity, mortality, and healthcare costs. Vagus nerve stimulation (VNS) is an established neuromodulatory treatment option for DRE, but local guidance on its use in the United Arab Emirates (UAE) remains limited. This study aimed to establish an expert consensus to define best practices for the effective and efficient use of the VNS in the UAE setting. METHODS: A multidisciplinary Task Force conducted a literature review and develope

Mertk drives alveolar bone loss in experimental periodontitis associated with altered neutrophil function and osteoclast recruitment.

BACKGROUND: Alveolar bone loss is the hallmark of periodontitis and often remains refractory to conventional mechanical and antimicrobial therapies in chronic cases. Mer receptor tyrosine kinase (Mertk) has been implicated in bone homeostasis, yet the immune mechanisms by which Mertk modulates periodontal bone resorption remain insufficiently characterized. The aim of this study is to investigate the role of Mertk deficiency in modulating the inflammatory response during ligature-induced periodontitis and its impact on alveolar bone loss. METHODS: C57BL/6J wild-type (WT) and Mertk-∖- mice at

METTL3-mediated m6A methylation of lipopolysaccharide-binding protein exacerbates periodontitis by promoting M1 macrophage polarization.

BACKGROUND: Periodontitis is a chronic inflammatory disease driven by dysregulated immune responses and is closely associated with macrophage polarization. N6-methyladenosine (m6A) methylation represents the most prevalent RNA modification; however, its role in periodontitis remains unclear. This study aimed to investigate the mechanism by which m6A modification regulates macrophage polarization during the progression of periodontitis. METHODS: A periodontitis mouse model was established by ligature induction. RAW264.7 macrophages were treated with Porphyromonas gingivalis-derived lipopolysac

The ROBUST-RCT tool had at least moderate agreement in an inter-rater reliability study of assessments by junior researchers.

BACKGROUND AND OBJECTIVE: The recently introduced ROBUST-RCT tool aims to balance ease of application and methodological rigor in risk-of-bias assessment for systematic reviews. The tool is structured in two steps: first, evaluating what happened; second, judging the risk of bias related to the assessed aspect of the study. Its straightforward design aims to avoid overly complex workflows. Thus, its usability testing included junior reviewers to ensure accessibility and ease of use. No data regarding its inter-rater reliability are currently available. This study aims to assess the inter-rater

A novel split flap-roll technique for peri-implant soft tissue augmentation in the esthetic zone: A clinical case report.

BACKGROUND: Peri-implant soft tissue thickness is a critical determinant of esthetic outcomes and long-term implant stability, particularly in the anterior maxilla. METHODS: A 28-year-old systemically healthy male presented with a missing maxillary central incisor and a thin gingival biotype. Following implant placement, a modified split flap-roll technique was performed. The surgical approach involved splitting the flap into epithelial and connective tissue components, followed by mobilization and rolling of the connective tissue into a labial pouch to enhance tissue thickness. RESULTS: Sof

Allocation of Kidney Allografts From Donors With Kidney Donor Profile Index <35% and the Impact of Kidney Donor Profile Index Revisions on Access to Transplantation for Children.

RATIONALE & OBJECTIVE: Children in the United States receive priority for kidneys with a kidney donor profile index (KDPI) < 35%, although their priority falls below that of certain adult recipients. Recently, the KDPI was revised to include 8 donor characteristics instead of 10. We sought to (1) describe the allocation of these high-quality kidneys and (2) compare the characteristics of donors with a KDPI < 35% using the original KDPI (KDPI-10) versus the revised KDPI (KDPI-8) calculation. STUDY DESIGN: Retrospective cohort study. SETTING & PARTICIPANTS: 60,587 deceased donors and their kid

Operational reliability assessment of radiotherapy equipment failures through integrated Pareto and FMECA analysis.

INTRODUCTION/BACKGROUND: Unplanned equipment and infrastructure interruptions are a persistent source of radiotherapy service loss, yet prioritization is often based on either downtime burden or risk criticality alone. This study analyzed five years of interruption logs to integrate a standardized failure taxonomy with downtime-based Pareto analysis and FMECA criticality scoring. METHODS: A harmonized event log captured asset class, subsystem, cause category, operational severity, downtime, and maintainability indicators, including response and repair times. Event burden and downtime distribu

Sleep enhances gamma oscillations in the seizure onset zone and broadband activity in the irritative zone of focal cortical dysplasia.

OBJECTIVE: Focal cortical dysplasia (FCD) is a leading cause of drug-resistant epilepsy and is associated with sleep-related seizures, yet the underlying electrophysiological mechanisms during different brain states remain poorly understood. We investigated whether fast oscillations (FOs) within the seizure onset zone (SOZ) and irritative zone (IZ) change significantly during sleep compared with wake in FCD patients. We analyzed multiple frequency bands-beta (14-20 Hz), gamma (40-80 Hz), ripple (80-250 Hz), and broadband BGR (14-250 Hz) to provide comprehensive information about sleep-related
