Chest Wall Reconstruction With 3D Printing: Anatomical and Functional Considerations.

Large chest wall defects, as a result of wide local excision of chest wall pathology, require skeletal and soft-tissue reconstruction to restore the anatomical shape, structure, and respiratory function of the thorax. Reconstruction is challenging and requires the surgical reconstructive team to understand the anatomic and physiologic morbidity related to the defect and the choice of reconstructive techniques available to restore form and function. Rapidly emerging 3-dimensional (3D) printing technology allows the reconstructive surgical team to customize the therapeutic process of skeletal re

Automated Torso Contour Extraction from Clinical Cardiac MR Slices for 3D Torso Reconstruction.

Whilst the electrocardiogram (ECG) is an essential tool for diagnosing cardiac electrical abnormalities, its characteristics are dependent on anatomical variability. Specifically variation in torso geometry affects relative positions of the leads with respect to the heart. We propose a novel pipeline that uses standard cardiac magnetic resonance images to reconstruct the torso and heart, and recreate the ECG considering torso and cardiac anatomy. This requires automated extraction of the torso contours. Our method combines an initial u-net segmenter with a second network that refines contours

Reconstruction of the trachea and carina: Surgical reconstruction, autologous tissue transplantation, allograft transplantation, and bioengineering.

There have been significant advancements in medical techniques in the present epoch, with the emergence of some novel operative substitutes. However, the treatment of tracheal defects still faces tremendous challenges and there is, as yet, no consensus on tracheal and carinal reconstruction. In addition, surgical outcomes vary in different individuals, which results in an ambiguous future for tracheal surgery. Although transplantation was once an effective and promising method, it is limited by a shortage of donors and immune rejection. The development of bioengineering has provided an alterna

Current Concepts in Maxillary Reconstruction.

Defects involving the midface and maxilla pose unique challenges to reconstructive surgeons. The maxilla serves as the central keystone of the face and interfaces with various other facial subunits to affect form and function. Reconstructive methods range from prosthetics to free tissue transfer, and the indications for different options are reviewed here. Finally, special considerations including management of the orbit, dental rehabilitation, and future directions are discussed.

Labia Minora Reduction Using Central Wedge Technique: Central Wedge Technique.

The central wedge labiaplasty removes a "V" or a pie-slice piece of the most protuberant portion of the labium to make it smaller. The anterior and posterior edges of the labium are reapproximated to preserve the normal labial edge and anatomy with minimal scarring. The lateral or medial-lateral clitoral hood can be reduced at the same time. The complication rate is very low if done using meticulous technique, thus avoiding the problems with alternative labiaplasty methods. The patient satisfaction level is well over 90%.

Introducing the thin and ultrathin submental artery perforator flaps for precise reconstruction following T1-2 oral cavity cancer resection.

We reported a case series of thin (n = 32) and ultrathin (n = 4) submental artery perforator flaps (SMAPF) for precise reconstruction following T1-2 oral cavity cancer resection. The former is indicated for tongue reconstruction involving the tongue tip, and buccal lining replacement involving the mouth corner, while the latter is specifically tailored to restore superficial buccal defects after resection of small early cancer originating from oral submucous fibrosis. All flaps survived. Most flap reconstructions reached satisfactory cosmetic results. Based on this series, we discussed the ind

Reverse posterior interosseous flap: different approaches over the years - A systematic review.

The posterior interosseous flap (PIOF), as fasciocutaneous flap, represents an interesting option in reconstructive surgery of the upper limb. The PIOF allows coverage of losses of substance of the dorsal side of the hand and, with technical modifications, up to the proximal interphalangeal (PIP) joint without any need of microsurgical technique or the sacrifice of the main vascular axes for the hand. Its dimensions can reach up to 22 cm x12 cm and have excellent versatility since it can incorporate vascularized bone grafts and tendon grafts for complex substance losses. Despite the above-ment

The Homodigital Propeller Flap: A New Tool for Fingertip Reconstruction. Surgical Technique, Anatomical Study, and Clinical Relevance.

BACKGROUND: An ideal flap for fingertip injuries should cover a significant loss of substance and avoid joint flexion to prevent later contracture and joint stiffness. In this study, the authors describe a modified homodigital island flap-the homodigital propeller flap-to better meet these requirements. METHODS: An anatomical model of fingertip loss is used to analyze the advancement achieved without flexion of the finger. Twenty-four fresh-frozen fingers were dissected. Wires were inserted to maintain joints in extension. The skin paddle was similar in location to a homodigital island flap.

Beaming the Charcot Foot.

The Charcot diabetic foot presents unique challenges to the podiatric surgeon in the quest to salvage the limb. This disorder is an intersection of prototypical metabolic diseases and neurodegenerative disorder. Furthermore, it can be considered a disease of bone and ligaments that is often complicated by peripheral vascular disease and serious deep infection. Presently, simplistic ablative surgical procedures and the brace-makers art, still have a valid place in treating this disorder. Newer methods of surgical reconstruction are rapidly evolving to address distorted and nonfunctional limb. T

[Anterior sternoclavicular dislocation, description of surgical technique].

Sternoclavicular joint dislocation is an uncommon injury and is usually consequential to high-energy trauma. These can be classified as anterior or retro-sternal (posterior) dislocation. Sometimes they can be accompanied by injury to large vessels due to their anatomical proximity mainly retrosternal. Conceptually closed reduction is indicated as the first line of treatment in acute injuries and open reduction tends to be for failing the previous one. We present a case of anterior clavicular sternal dislocation, with recurrence after closed reduction under anesthesia and surgically operated wi

Vascularization strategies for tissue engineering for tracheal reconstruction.

Tissue engineering technology provides effective alternative treatments for tracheal reconstruction. The formation of a functional microvascular network is essential to support cell metabolism and ensure the long-term survival of grafts. Although several tracheal replacement therapy strategies have been developed in the past, the critical significance of the formation of microvascular networks in 3D scaffolds has not attracted sufficient attention. Here, we review key technologies and related factors of microvascular network construction in tissue-engineered trachea and explore optimized prepa
