[Circulatory dysfunction syndrome associated with liver cirrhosis].

Splanchnic arterial relaxation is the most important pathology in systemic circulation of portal hypertensive patients. Progressive decline of splanchnic vascular resistance is responsible for development of circulatory dysfunction syndrome (CDS), associated with reduction of effective blood volume within central vascular compartment and compensatory stimulation of vasopressor and natrium retaining hormonal mechanisms. Advanced CDS is characterized by increased cardiac output, tachycardia and low arterial pressure. Complications of CDS have functional nature and comprise: renal failure (hepato

Noncirrhotic portal hypertension: recent concepts.

NCPH is a result of obliteration of portal veins. Many inflammatory conditions may initiate the process by causing endothelial injury. As obliteration progresses, there is local stasis and low-grade portal hypertension. In many cases, superimposed PVT occurs before portal hypertension becomes clinically evident. Hypercoagulability is an important cofactor. Diagnosis requires the exclusion of cirrhosis. Focal atrophy and nodular hyperplasia on biopsy may be a clue to the presence of small vessel obliteration. The distribution of vascular disease should be documented with Doppler ultrasound of b

Proximity of the Neurovascular Bundle During Posterior-Lateral Meniscal Repair: A Comparison of the Transpatellar, Anteromedial, and Anterolateral Portals.

PURPOSE: To compare the neurovascular proximity of the transpatellar portal with that of the medial and lateral portals and to determine the safe penetration depth for an all-inside device for use on the posterior horn lateral meniscus. METHODS: Dissection of the popliteal fossa was performed in 10 cadaveric knees to identify all structures. Arthroscopy was performed using penetration depths of 10, 12, 14, and 16 mm with the all-inside system through the anteromedial, anterolateral, and transpatellar portals. Penetrations were made 5 and 10 mm lateral to the posterior horn root at the menisco

Lumped parameter modeling of changes in liver hemodynamics due to cirrhosis.

This numerical study employs a computational model of human hepatic blood flow to investigate the hemodynamic consequences of cirrhosis. The … results demonstrate that while the dilatation of portosystemic collaterals effectively offloads the portal system, it leads to a substantial increase in overall blood flow, contributing to hyperdynamic circulation (a hallmark feature of cirrhosis). These predictions provide quantitative, mechanistic insights into the hemodynamic and anatomical alterations of ci …

Out of reach? Correlates of cervical cancer underscreening in women with varying levels of healthcare interactions in a United States integrated delivery system.

One in five U.S. women with health insurance are underscreened for cervical cancer. We sought to identify whether underscreening correlates differed among women with different levels of health care interaction. Among women age 30-64 years who were members of an integrated U.S. health system, we used 2014-2015 electronic health record data to identify underscreened cases (≥3.4 years since last Papanicolaou (Pap) test, n=3352) and screening-adherent controls (<3.4 years since last Pap test, n=45,359) and extracted data on potential underscreening correlates (demographics, health history, and hea

Superoxide dismutase gene transfer reduces portal pressure in CCl4 cirrhotic rats with portal hypertension.

BACKGROUND: Increased intrahepatic vascular tone in cirrhosis has been attributed to a decrease of hepatic nitric oxide (NO) secondary to disturbances in the post-translational regulation of the enzyme eNOS. NO scavenging by superoxide (O(2)(-)) further contributes to a reduction of NO bioavailability in cirrhotic livers. AIM: To investigate whether removing increased O(2)(-) levels could be a new therapeutic strategy to increase intrahepatic NO, improve endothelial dysfunction and reduce portal pressure in cirrhotic rats with portal hypertension. METHODS: Adenoviral vectors expressing extra

The Covert Side of Ascites in Cirrhosis: Cellular and Molecular Aspects.

Ascites, a common complication of portal hypertension in cirrhosis, is characterized by the accumulation of fluid within the peritoneal cavity. While traditional theories focus on hemodynamic alterations and renin-angiotensin-aldosterone system (RAAS) activation, recent research highlights the intricate interplay of molecular and cellular mechanisms. Inflammation, mediated by cytokines (interleukin-1, interleukin-4, interleukin-6, tumor necrosis factor-α), chemokines (chemokine ligand 21, C-X-C motif chemokine ligand 12), and reactive oxygen species (ROS), plays a pivotal role. Besides pro-inf

The vascularization of the digestive tract studied by scanning electron microscopy with special emphasis on the teeth, esophagus, stomach, small and large intestine, pancreas, and liver.

The periodontal vessels in adult rats show a ladderlike pattern; in guinea pigs molars, by contrast, they present a honey-comb pattern. The vascular architecture in human teeth seems to be similar to that of rabbits. In guinea pigs, rats, rabbits and humans esophagus circumferential vessels give off perforating vessels. In human esophagus the number and diameter of the vessels in the submucous venous plexus decrease from proximal to distal. In the stomach the subepithelial capillary network shows a honey-comb pattern reflecting the arrangement of the gastric pits. A local portal system between…

Anatomical Variations of Portal Vein Branches in Subsegment IVb: A 3D Reconstruction-Based Classification to Inform Radical Resection for Gallbladder Cancer.

INTRODUCTION: Subsegment IVb (S4b) is a common site of direct invasion in gallbladder cancer, and its complete resection is critical to achieve negative surgical margins. However, the portal venous anatomy of S4b is highly complex and exhibits significant interindividual variation, which the conventional Couinaud classification fails to adequately delineate. This study utilized three-dimensional visualization to systematically characterize the anatomical features and branching patterns of the portal veins supplying S4b, with the goal of establishing a practical anatomical classification to inf

Incidental Detection of Portal Venous Gas During POCUS in a Neonate With Necrotizing Enterocolitis and Intestinal Perforation: A Case Report.

In a postoperative cardiac neonate undergoing bedside hemodynamic assessment for suspected venous congestion, point-of-care ultrasound performed by a pediatric intensivist unexpectedly revealed hepatic portal venous gas. This finding prompted immediate abdominal reassessment and urgent radiography, which demonstrated pneumoperitoneum. Exploratory laparotomy revealed terminal ileal perforations with ischemic changes and fecal peritonitis. Although the examination was performed for hemodynamic purposes, careful assessment of the portal venous system disclosed a clinically important abnormality u

[Endohepatology in 2026 : innovations and perspectives].

Endohepatology is a rapidly expanding field that combines hepatological expertise with advanced interventional endoscopy. It refers to the use of endoscopic techniques for the diagnosis and management of hepatobiliary diseases and, in particular, complications related to portal hypertension. This approach primarily relies on endoscopic ultrasound and endoscopic retrograde cholangiopancreatography, which allow for a direct and precise access to the liver, the biliary tree, and the portal venous system. Endohepatology thus exemplifies the shift toward an integrated and minimally invasive managem

[Management of chylous ascites and chylothorax. Report of one case].

Chylous Ascites (CA) and chylothorax (CTx) are associated with obstruction, disruption or insufficiency of the lymphatic system. We report a 68-year-old male, with a history of alcoholic cirrhosis, who had recurrent events of CTx and CA. After a complete study, no other etiologies other than portal hypertension were found. Therapy with diuretics, nothing per mouth, parenteral feeding plus octreotide did not relieve symptoms. A transjugular intrahepatic portosystemic shunt (TIPS) was successfully placed and pleural effusion subsided. This case shows that CA and CTx can be caused by portal hyper

Combined piggyback technique and cavoportal hemitransposition for liver transplant.

Portal Vein thrombosis (PVT) increases the difficulty of liver transplant; however, it is not an absolute contraindication. Cavoportal hemitransposition (CPH) is an option for patients with complete PVT and no alternative collateral vein. Our center often performs the piggyback technique for the hepatic vein reconstruction, which allows for great access to the recipient vena cava in patients with known complete PVT that may need a CPH preformed to successfully restore flow to the portal system of the donor liver. We describe the use of the piggy-back technique to prepare the vena cava for poss…

[Esophagogastroduodenoscopy in evaluation of the results of portacaval shunt].

The article discusses the results of endoscopic examination of 44 children with extrahepatic portal hypertension before and after various types of portocaval shunting. Thirteen children were also examined in the late-term postoperative periods. It was found that decompression of the portal system not only leads to rapid reduction of the esophageal varicosities but also facilitates relief from gastritis and esophagitis. An unfavorable effect of cava-mesenteric shunts on the development of the bulbo-gastric reflux was revealed. It is shown that long-term regular medical observation over these pa…

Budd-Chiari syndrome: shunt selection and post-operative assessment.

Radiological investigation of patients with the Budd-Chiari syndrome is helpful in making the diagnosis and to determine the most suitable method of surgical decompression of the portal venous system where this is indicated. The radiological findings in four patients with Budd-Chiari syndrome who were treated by meso-atrial shunt operations are described. The key pre-operative investigation is inferior vena cavography with pressure measurements. Postoperatively shunt patency is most reliably assessed by dynamic CT scanning. Doppler ultrasound studies may detect flow within the shunt but patenc
