Safety and technical success of diphenhydramine as an alternative local anaesthetic agent for radiology procedures.

AIM: To evaluate retrospectively the safety and technical success of subcutaneous diphenhydramine as an alternative local anaesthetic for radiology procedures. MATERIALS AND METHODS: Between January 2000 and April 2021, 84 image-guided procedures were performed in 81 adult patients (mean age 61 years, 86% female) using 1% injectable diphenhydramine as a local anaesthetic. Indications were history of severe allergy to "-caine" local anaesthetics in 76 (90%) patients and recent administration of bupivacaine liposomal injectable suspension in eight (10%) patients. Twelve of the 84 (14%) procedur

[The updated S3 guidelines: sedation in gastrointestinal endoscopy : Innovations and relevant aspects for the routine].

In 2023 the third revision of the S3 guidelines on sedation in gastrointestinal endoscopy was presented. As with the two previous versions of the guidelines, the relevant and current literature was reviewed and evaluated by a group of experts from various specialist societies under the guidance of the German Society for Gastroenterology, Digestive and Metabolic Diseases (DGVS). In addition to confirming some of the older recommendations, a number of recommendations were revised or newly added. A brief overview of the current version of the S3 guidelines is provided with a focus on relevant cha

Reducing Time to Postintubation Sedation in a Pediatric Emergency Department.

BACKGROUND AND OBJECTIVE: Inadequate postintubation sedation (PIS) can lead to unplanned extubations, conscious paralysis, and overall unsafe care of patients. From 2018 to 2020, we realized at our hospital that ∼25% of children received sedation in an adequate time frame in the pediatric emergency department, with 2 unplanned dislodgements of the endotracheal tube. Our objective was to reduce time to initiating PIS from a mean of 39 minutes to less than 15 minutes in our pediatric emergency department by September 2021. METHODS: A multidisciplinary team was formed in March 2020 to develop a

Assessment of sedation by automated pupillometry in critically ill patients: a prospective observational study.

BACKGROUND: Quantitative measurement of pupil change has not been assessed against the Richmond Agitation and Sedation Scale (RASS) and spectral edge frequency (SEF) during sedation. The aim of this study was to evaluate pupillometry against these measures in sedated critically ill adult patients. METHODS: In ventilated and sedated patients, pupillary variables were measured by automated pupillometry at each RASS level from -5 to 0 after discontinuation of hypnotics, while processed electroencephalogram variables were displayed continuously and SEF was recorded at each RASS level. Correlation

Awake fiberoptic orotracheal intubation: a protocol feasibility study.

OBJECTIVE: To assess the feasibility of an awake fiberoptic intubation (AFOI) protocol. METHODS: We enrolled 40 patients with simulated difficult intubation. The protocol consisted of conscious sedation (midazolam, 0.03 mg/kg and sufentanil, 0.1 µg/kg), regional anesthesia, and intubation. The time, first-attempt intubation success rate, hemodynamic parameters, blood oxygen saturation (SpO2), intubation amnesia rate, patient satisfaction, and relative complications were recorded. RESULTS: AFOI was completed in all patients. The average total AFOI time was 14.17 ± 1.47 minutes, and the time t

Evaluation of the Integrated Pulmonary Index® during non-anesthesiologist sedation for percutaneous endoscopic gastrostomy.

Standard monitoring of heart rate, blood pressure and arterial oxygen saturation during endoscopy is recommended by current guidelines on procedural sedation. A number of studies indicated a reduction of hypoxic (art. oxygenation < 90% for > 15 s) and severe hypoxic events (art. oxygenation < 85%) by additional use of capnography. Therefore, U.S. and the European guidelines comment that additional capnography monitoring can be considered in long or deep sedation. Integrated Pulmonary Index® (IPI) is an algorithm-based monitoring parameter that combines oxygenation measured by pulse oximetry (a…

Use of Acoustic Transmitter-Equipped Remote Sedation to Aid in Tracking and Capture of Entangled California Sea Lions (Zalophus californianus).

Free-ranging California sea lions ( Zalophus californianus ) with marine debris entanglements were darted with a combination of medetomidine, butorphanol, and midazolam by using acoustic transmitter-equipped darts. Of the 15 animals sedated, 13 (87%) reentered the water and were tracked by using a unidirectional hydrophone. Sea lions that entered the water continued to surface and breathe postsedation. There were three mortalities (20%) during the course of this study due to the following: suspected drowning caused by entrapment under a dock, overdose due to inaccurate weight estimation, and t

Bispectral index monitoring in the management of sedation in an intensive care unit patient with locked-in syndrome.

Locked-in syndrome is an extremely rare neurological state caused by injury of the ventral pons. The syndrome is characterized by quadriplegia and anarthria with concomitant preservation of cortical function. When a reversible underlying pathological abnormality is identified and managed aggressively, meaningful recovery is possible. Because patients retain consciousness throughout their illness, a dependable method for titrating sedation may improve their quality of life. The case presented suggests that bispectral index monitoring may be a cost-effective and reliable method for managing seda

Noninvasive ventilation during percutaneous gastrostomy placement in Duchenne muscular dystrophy.

Noninvasive positive pressure ventilation (NPPV) is used for respiratory support in a number of diseases causing acute or chronic respiratory failure. We describe a novel use of NPPV to provide respiratory support during sedation for percutaneous placement of a gastrostomy tube in a patient with Duchenne muscular dystrophy (DMD). The patient had severe respiratory insufficiency, progressive dysphagia, and undernutrition. In addition to the case in this report, we have used NPPV to provide respiratory support to DMD patients during five other gastrointestinal endoscopies without complication. T

Short-term outcomes across sedation practice patterns in mechanically ventilated patients with bacterial pneumonia: a nationwide cohort study.

BACKGROUND: Although guidelines recommend light sedation with propofol or dexmedetomidine in mechanically ventilated patients, appropriate management with monotherapy is often challenging. Whether combined sedation reduces dose-dependent adverse effects of each agent or causes additive harm remains unclear. We investigated short-term outcomes associated with early sedation practice patterns in mechanically ventilated patients with bacterial pneumonia. METHODS: Using the Japanese Diagnosis Procedure Combination database, we identified adult intensive care unit patients with bacterial pneumonia

Initial assessment of medical post-traumatic stress among patients with chronic esophageal diseases.

BACKGROUND: Diagnosis and management of chronic esophageal disease requires the use of potentially traumatic medical procedures, performed with or without sedation. Medical trauma and post-traumatic stress (PTS) are emerging as important considerations in patients with digestive illness. To date, no study assesses medical PTS from procedures in patients with esophageal disease. METHODS: Adult patients with achalasia, eosinophilic esophagitis, gastroesophageal reflux disease, or functional esophageal disease at a university-based gastroenterology clinic completed: Post-Traumatic Stress Disorde

[Anaesthesia for bronchial echoendoscopy: experience with the laryngeal mask].

INTRODUCTION: Endobronchial ultrasound guided transbronchial needle aspiration (EBUS-TBNA) has recently been shown to be an accurate modality in the diagnosis and staging of mediastinal lymph node metastases. This procedure takes significantly longer than a conventional bronchoscopy and may therefore cause more discomfort. Since its introduction into respiratory practice in France, several airway management strategies have been used. PATIENTS AND METHODS: Both anaesthesia care and procedural sedation services share the goals of providing the patient with comfort during a potentially distressi…

[Assessment of different pain management methods for the treatment of claw lesions in Meat Merino ewes].

OBJECTIVE: Pain management during veterinary procedures is a significant component of animal welfare and has legal as well as ethical implications. Even though regional intravenous anaesthesia (RIVA) is an accepted method for painful procedures involving the distal digits of sheep, this anaesthetic technique is rarely applied in the field. The primary goal was to investigate the feasibility, safety and efficacy of RIVA in sheep. A secondary goal was to examine whether the anaesthetic procedure can be improved by combining RIVA with sedation and whether these methods have a positive effect on t

Improved outcome and cost effectiveness in ablation of persistent atrial fibrillation under general anaesthetic.

AIMS: Outcome of persistent atrial fibrillation (AF) ablation remains suboptimal. Techniques employed to reduce arrhythmia recurrence rate are more likely to be embraced if cost-effectiveness can be demonstrated. A single-centre observational study assessed whether use of general anaesthesia (GA) in persistent AF ablation improved outcome and was cost-effective. METHODS AND RESULTS: Two hundred and ninety two patients undergoing first ablation procedures for persistent AF under conscious sedation or GA were followed. End points were freedom from listing for repeat ablation at 18 months and fr
