[Intensive outpatient group treatment for adolescents with eating disorders].

We present an intensive outpatient group treatment for girls with eating disorders (anorexia nervosa, bulimia nervosa, binge eating disorder) additionally to/instead of inpatient treatment or individually treatment by psychotherapists. The therapy concept is primarily behaviour therapy oriented, encouraging the self-management-abilities of the patients thereby learning self-determination and responsibility in dealing with their illness. The slow-open group concept provokes group cohesion, solidarity and support among girls, who share similar age-related development-stages and eating disorders.

Combined application of the International Classification of Functioning, Disability and Health and the NANDA-International Taxonomy II.

AIM: This paper presents a discussion of the conceptual and practical relationships between the International Classification of Functioning, Disability and Health and the NANDA-International Taxonomy II for nursing diagnoses, and their use in nursing practice. BACKGROUND: The ICF provides a common classification framework for all healthcare professionals, including nurses. Nursing care plans can be broadly based on NANDA-I taxonomies. No published attempt has been made to systematically compare the NANDA-I Taxonomy II to the ICF. DATA SOURCES: The most recently published descriptions of both

Developing a cross-organisational physiotherapy-specific critical care competency framework to enable collaborative working: an overview of the integration journey across an academic health science centre.

BACKGROUND: Competency frameworks are commonly used to define expectations, aid acquisition of appropriate skills and ensure safe delivery of care to patients. The Intensive Care Society's Allied Health Professionals Critical Care Professional Development Framework (CCPDF) provides broad descriptors of levels of practice ranging from novice to expert. Prior to May 2021, no standardised clinical competency frameworks governed physiotherapy practice or development in the critical care setting in the UK. METHODS: The aim was to develop and evaluate the value of a cross-organisational, physiother

A tailored, supportive care intervention using systematic assessment designed for people with inoperable lung cancer: a randomised controlled trial.

OBJECTIVE: People with inoperable lung cancer experience higher levels of distress, more unmet needs and symptoms than other cancer patients. There is an urgent need to test innovative approaches to improve psychosocial and symptom outcomes in this group. This study tested the hypothesis that a tailored, multidisciplinary supportive care programme based on systematic needs assessment would reduce perceived unmet needs and distress and improve quality of life. METHODS: A randomised controlled trial design was used. The tailored intervention comprised two sessions at treatment commencement and

Transforming primary care practices in a Hawai'i Island clinic: obtaining patient perceptions on patient centered medical home.

Health care access is a significant problem for residents of Hawai'i Island who are experiencing a healthcare provider shortage crisis. Many residents must travel far for routine medical care, and in some cases to other islands. Hamakua Health Center, Inc., which operates from two clinical sites (Honokaa and Kapaau), is transitioning towards a Patient-Centered Medical Home care model. Through focus groups, a qualitative study was completed to obtain patient perceptions on Patient-Centered Medical Home. The Hamakua and Kohala Family Health Center staff were asked to recommend a list of patients

A Healthier Future for Tissue Viability - An initiative to improve outcomes for patients with lower limb issues.

Reducing unwarranted variation in clinical care and access to care was a central goal of the Five Year Forward View, and the later Long Term Plan. Variation in provision and access to lower limb services in one Sustainability and Transformation Partnership were apparent. Through networking and collaboration of STP wide tissue viability and related specialist services a programme of service development was accepted as part of wider Frailty service provision. This led to a public health needs assessment of the problem being commissioned; 6 clinical pathways being produced; and clear recommendati

Potential for quality improvement of acute stroke management in a district general hospital.

BACKGROUND: Stroke units and thrombolysis are evidence based treatments for stroke patients. Few studies have prospectively assessed the success of, and obstacles to implementation of such strategies in patients admitted to district general hospitals. OBJECTIVE: To document delays in admissions of acute stroke patients to hospital, failures in accessing a stroke unit and the clinical impact of missed opportunities for intervention in acute stroke patients. DESIGN, SETTING AND METHODS: Prospective observational study in a district general hospital in Northern Ireland. Delays, demographic deta

Asthma in older patients: factors associated with hospitalization.

BACKGROUND: Although older adults (> or =65 years) with asthma have higher rates of hospitalization and death from asthma than younger adults, the reasons for this are not known. OBJECTIVES: To determine whether patterns of care were less favorable for older than younger adults with asthma and to assess whether patient characteristics such as symptom severity and comorbid illnesses explain the higher rate of hospitalization. METHODS: Prospective cohort study of 6590 adults with asthma in 15 managed care organizations in the United States. Participants completed a survey of demographics, symp

Emergency medical services and sudden cardiac arrest: the "chain of survival" concept.

Early access to the EMS system helps insure early CPR, defibrillation, and advanced care. Early access is easiest to achieve with 911 systems and widespread community education and publicity. It may also be taught during citizen-CPR classes. Early CPR helps patients by slowing the process of dying, but its effectiveness disappears within minutes, and defibrillation must soon follow. A citizenry well-informed about cardiac emergencies and well-trained in CPR appears to be the best method to achieve early recognition and early CPR. The earliest possible delivery of defibrillation is critical and

Reducing Hospital-Acquired Pressure Injuries in a Cardiothoracic Intensive Care Unit.

BACKGROUND: Hospital-acquired pressure injuries are a significant patient safety concern. The Centers for Medicare & Medicaid Services tracks hospital-acquired pressure injuries as a patient safety indicator. Health care organizations with higher-than-expected rates may incur penalties. LOCAL PROBLEM: The pressure injury prevalence rate in a cardiothoracic intensive care unit was above the National Database of Nursing Quality Indicators benchmark. The current standard of care-use of the Braden scale for pressure injury risk assessment and the SKIN (surface, keep turning, incontinence care, an

What rate of utilization is appropriate in musculoskeletal care?

UNLABELLED: Musculoskeletal procedures often show wide variation in rates across geographic areas, which begs the question, "Which rate is right?" Clearly, there is no simple answer to this question. We summarize a conceptual framework for thinking about how to approach this question for different types of interventions. One guiding principle is the "right rate" is usually the one that results from the choices of a fully informed and empowered patient population. For truly effective care without substantial tradeoffs, the right rate may approach 100%. The rate of operative treatment of hip fra

Transition from pediatric to adult care in patients with chronic intestinal failure on home parenteral nutrition: How to do it right?

BACKGROUND: Life expectancy of children with chronic intestinal failure (CIF) on home parenteral nutrition has greatly improved. Children are now able to grow into adulthood which requires transfer from pediatric to adult health care. A guideline for structured transition is lacking and the demand for a more standardized care for this patient group is necessary. Therefore, we investigated the perceptions of health care professionals from various disciplines working in this specific field, concerning effective interventions regarding transition to adult health care. AIM: To create a standardiz

Beyond pGALS: the need for a multifaceted musculoskeletal decision-making tool ('pGALSplus') in community-based clinical practice.

Musculoskeletal (MSK) problems in children are common, and health-care professionals must identify those requiring onward referral. Paediatric gait, arms, legs and spine (pGALS) is an MSK assessment to discern abnormal joints. We aimed to identify MSK assessments to add to pGALS (pGALSplus) to facilitate decision-making in the context of exemplar conditions representing a spectrum of MSK presentations, namely JIA, mucopolysaccharidoses, muscular dystrophy and developmental co-ordination disorder. A literature review identified 35 relevant articles that focused on clinical assessments [includin

Transgender and Gender-Diverse Patient Perspectives on Hereditary Breast Cancer Risk Assessment in Gender-Affirming Top Surgeries.

PURPOSE: Transgender and gender-diverse (TGD) people have increased cancer morbidity and mortality relative to cisgender individuals. Existing breast cancer prevention guidelines fail to adequately address gender-affirming care implications, especially in the context of gender-affirming mastectomies (top surgery). Evaluation of familial and germline genetic risk before top surgery is recommended by some transgender health guidelines. This evaluation can facilitate shared surgical decision making regarding breast tissue resection extent. In this study, we use in-depth interviews with TGD people

Supporting at-risk older adults transitioning from hospital to home: who benefits from an evidence-based patient-centered discharge planning intervention? Post-hoc analysis from a randomized trial.

BACKGROUND: Subgroups of older patients experience difficulty performing activities of daily living (ADL) following hospital discharge, as well as unplanned hospital readmissions and emergency department (ED) presentations. We examine whether these subgroups of "at-risk" older patients benefit more than their counterparts from an evidence-based discharge planning intervention, on the following outcomes: (1) independence in ADL, (2) participation in life roles, (3) unplanned re-hospitalizations, and (4) ED presentations. TRIAL DESIGN AND METHODS: This study used data from a randomized control

A Pilot Project Using Pediatricians as Initial Diagnosticians in Multidisciplinary Autism Evaluations for Young Children.

OBJECTIVES: Wait times for autism spectrum disorder (ASD) evaluations are long, thereby delaying access to ASD-specific services. We asked how our traditional care model (requiring all patients to see psychologists for ASD diagnostic decisions) compared to an alternative model that better utilizes the available clinicians, including initial evaluation by speech, audiology, and pediatrics (trained in Level 2 autism screening tools). Pediatricians could diagnose immediately if certain about diagnosis but could refer uncertain cases to psychology. Accuracy and time to diagnosis, charges, and pare

A qualitative study of patients' experience of recovery after a distal femoral fracture.

PURPOSE: This qualitative study was conducted as part of a feasibility study for TrAFFix, (ISRCTN92089567), a randomised controlled trial that will compare two surgical interventions used to fix distal femoral fractures. Our aim was to understand patients' experiences of treatment and the early phase of recovery after a distal femoral fracture. While, much is known about the experience of recovery from hip fracture, little is known about whether patients with other lower limb fragility fractures experience the same concerns and challenges. MATERIALS AND METHODS: Semi-structured interviews wer
