[The diagnosis and treatment of perianal dermatitis].

Perianal dermatitis is one of the most common proctological disorders. The anatomy of the anal region provides suitable conditions for the development of dermatitis. In the diagnostic work-up and the management of patients with perianal dermatitis, three types need to be distinguished: irritant contact dermatitis, atopic dermatitis, and allergic contact dermatitis. Each type has its aetiological and pathogenetic factors, which will provide clues to the diagnosis and subsequent management of the condition. In the differential diagnosis of the condition, consideration should be given to inflamma

[Surgical management of anorectal complications in Crohn disease].

Anorectal manifestations can be expected in about 40% of all patients suffering from Crohn's disease. The frequency increases with extension of disease towards the anal canal. If the rectum is involved, they are obligatory present and contribute to the necessity of a stoma formation. Surgical indication is mostly given, especially in cases with putrid secretion. Primary aim of therapy is the control of peri-anorectal infection by excision of all the inflamed extramural tissue. Wether the underlying fistulas are excised too, depends on their topography to the sphincter system. Fistulas, running

[A clinical and pathological analysis of 20 cases of lymphal follicular rectitis].

Lymphal follicular rectitis (LFR) recognized in recent years is a kind of benign lesion localizing in rectum. The lack of specific clinical and endoscopic manifestation causes the difficulty in its diagnosis. It was easily confused with ulcerative rectitis, even there were some cases which were misdiagnosed malignant rectal lymphoma. A retrograde analysis was made in our study in the 176 cases of pathologically diagnosed chronic rectitis between January 1993 and July 1994 in our hospital. Among the 176 cases, 20 were certain to be LFR (2 formerly diagnosed, 18 confirmed in the retrograde study

[Endoscopic sonography of the anorectum in inflammatory rectal diseases].

213 patients, classified in 3 groups, were examined in a prospective trial using endorectal ultrasound. 80 patients (group A) had no anorectal disease, the endorectal ultrasound showed normal structures of the anorectum and the perirectal tissue. 80 patients (group B) suffered from Crohn's disease, 30 of them had no clinical signs, the rest of this group (50 patients) felt pain in the anal/perianal region. 83 patients (group C) had perianal abscesses or fistulas, Crohn's disease could be excluded. In all 64 perianal fistulas and 66 perianal abscesses were diagnosed. The results of the rectal-p

[The determination of the content of immunoglobulin isotypes in the tissues in suppurative-destructive diseases].

Low amounts of immunoglobulins of different classes may be measured by the red cell agglutination inhibition test with the red cells conjugated with the respective blood serum IgG and IgM and colostrum IgA isotypes. A feedback between IgM and IgG levels was detected in extracts of the tissues adjacent to the focus of inflammation during the necrotic phase of the process, whereas during the exudative phase the IgG/IgM ratio grows, probably at the expense of the IgM-IgG change over during lymphocyte maturation.

Malignant pyoderma or pyoderma gangrenosum of the head and neck?

Malignant pyoderma is a rapidly progressive ulcerating process of unknown origin that predominantly affects the head and neck of young adults. Malignant pyoderma has been considered distinct from pyoderma gangrenosum because of the predominant head and neck location of the ulcers and because the ulcers lack undermining and surrounding erythema. A case of a 22-year-old woman with a severe ulcerative process predominantly affecting the head and neck is described. A review of the literature indicates that malignant pyoderma and pyoderma gangrenosum are almost certainly identical disorders.

[Side effects of non-steroidal antirheumatic agents in the lower gastrointestinal tract].

Peptic gastroduodenal lesions due to nonsteroidal antiinflammatory drugs (NSAID) are well known, but not the adverse effects of these preparations in the lower GI tract. The cases of 5 patients with NSAID-induced colitis are described and the pertinent literature is reviewed, which reveals a wide spectrum of alterations ranging from mild non-specific colitis to acute bleeding or perforation. The most important key to correct diagnosis is a careful drug history, and treatment is to discontinue the NSAID.

Pulse granuloma in a rectocutaneous fistula.

Pulse granulomas are rare foreign body reactions to vegetable material characterized by hyaline rings or starch granules. Pulse granulomas have mainly been reported in association with lung aspirations, the oral cavity with a history of oral procedures and less frequently in intestinal fistulae, colonic diverticulae or stomach ulcers. Only one case involving the skin has been reported in a patient with jejunocutaneous and ileocutaneous fistulae. We report the second cutaneous pulse granuloma in a patient with a rectocutaneous fistula.

[Ambulatory surgical interventions in patients with proctological diseases].

The results of 629 operations for the most frequent proctological diseases (hemorrhoid, anal scissure, rectal fistula, acute paraproctitis) were analyzed. 134 operations were performed in outpatient clinic, 225--in one-day hospital and 270 (control group)--in coloproctology department. The place of operation did not influence postoperative period and temporary disability time. The mean stay of patient in outpatient department and hospital surgical unit was 3.8 and 324 hours, respectively. Since the second day after operation in specialized department it is possible to perform therapeutic manip…
