Pastoral care and counseling with the "un-homeless homeless": understanding cultures of homelessness.

This article presents a subset of findings from a larger study exploring the lived experiences of 16 former residents of a 90-day emergency family shelter program in Los Angeles County. Interpretative phenomenological analysis serves as a qualitative method for understanding the cultural uniqueness of the "un-homeless homeless." The findings offer implications for culturally competent pastoral care and counseling in the context of family homelessness and attend to both the process and content of caregiving.

Acute phase cognitive therapy for recurrent major depressive disorder: who drops out and how much do patient skills influence response?

OBJECTIVE: The aims were to predict cognitive therapy (CT) noncompletion and to determine, relative to other putative predictors, the extent to which the patient skills in CT for recurrent major depressive disorder predicted response in a large, two-site trial. METHOD: Among 523 outpatients aged 18e70, exposed to 12e14 weeks of CT, 21.6% dropped out. Of the 410 completers, 26.1% did not respond. To predict these outcomes, we conducted logistic regression analyses of demographics, pre-treatment illness characteristics and psychosocial measures, and midtreatment therapeutic alliance. RESULTS:

Financial incentives for antipsychotic depot medication: ethical issues.

BACKGROUND: Giving money as a direct incentive for patients in exchange for depot medication has proved beneficial in some clinical cases in assertive outreach (AO). However, ethical concerns around this practice have been raised, and will be analysed in more detail here. METHOD: Ethical concern voiced in a survey of all AO teams in England were analysed regarding their content. These were grouped into categories. RESULTS: 53 of 70 team managers mentioned concerns, many of them serious and expressing a negative attitude towards giving money for depot adherence. Four broad categories of ethic

Group meetings for parents and spouses of bone marrow transplant patients.

To reduce the stress and social isolation of being the parent or spouse of a patient hospitalized for bone marrow transplantation (BMT), a support group for family members was established. Sharing the common experience of the BMT procedure was valuable to family members despite differences in patient's age or disease, relationship to the patient, or socio-economic status. Recommendations are made to include similar groups in planning to meet the psychosocial needs of family members at other centers where patients are treated with high-risk, high intensity procedures.

Bringing physician nutrition specialists into the mainstream: rationale for the Intersociety Professional Nutrition Education Consortium.

Given the prevalence of nutritionally related chronic diseases in American society, the training of physicians should include a focus on the relations of diet to disease. Yet, despite scientific data, public interest, US government reports, society studies, and congressional mandates, the teaching of nutrition in medical schools and residency programs remains inadequate. The authority whom patients most wish to consult for information on health-their physician-thus remains insufficiently informed about the role of diet in the prevention and treatment of disease. New efforts must be initiated t

From idealistic helper to enterprising learner: critical reflections on personal development through experiences from Afghanistan.

There is little written about the cultural, social, and ethical challenges encountered by physiotherapists engaging in development work. This article takes a critical perspective on what it means to engage in development work as an expatriate physiotherapist, through a self-critical reflection on experiences from Afghanistan. The field notes from an ethnographic study of a development project conducted in Afghanistan were analysed to explore the transformative process of personal and professional development of the development worker. The critical reflective process entailed a change in meanin

Overdue choices: how information and role in decision-making influence women's preferences for induction for prolonged pregnancy.

BACKGROUND: Enabling women to make informed decisions is a crucial component of consumer-focused maternity care. Current evidence suggests that health care practitioners' communication of care options may not facilitate patient involvement in decision-making. The aim of this study was to investigate the effect of specific variations in health caregiver communication on women's preferences for induction of labor for prolonged pregnancy. METHODS: A convenience sample of 595 female participants read a hypothetical scenario in which an obstetrician discusses induction of labor with a pregnant wom

Model of interprofessional care for the newborn and family during neonatal death and dying: a multistage evaluation mixed methods study.

An interprofessional care model is crucial to advancing the quality of integrated, patient-centered care, particularly in critical situations. The purpose of this study was to develop an interprofessional care model for newborns and their families experiencing neonatal death and dying. This is an explanatory sequential mixed-methods design with three phases. Data were collected in two public neonatal units in Brazil. In Phase I, quantitative data were collected from 66 healthcare professionals using two instruments: demographic characterization and the Profile of Attitudes Toward Death - Brazi

Practitioner perspectives on working with older patients in opioid agonist treatment (OAT) in Norway: opportunities and challenges.

BACKGROUND: Norway has a growing proportion of ageing opioid agonist treatment (OAT) patients, with 42% of the 8300 Norwegian OAT patients aged over 50 in 2022. This study aims to explore practitioners' views and experiences from treatment of ageing OAT patients. METHODS: Data were collected as a series of semi-structured interviews with treatment staff (roles interviewed: doctor, psychologist, social worker, nurse, and learning disability nurse). Participants were recruited from three OAT outpatient clinics, one with an urban catchment area and two with a mix of urban and rural. The intervie

Intimate partner violence screening and counseling in the health care setting: Perception of provider-based discussions as a strategic response to IPV.

Intimate partner violence (IPV) affects women worldwide, and is addressable in the health care setting not only via screening, but also through provider-based counseling and referral to legal or social services, as appropriate. We conducted a study in Pennsylvania (USA) examining factors associated with receipt of IPV screening and women's perceptions of counseling discussions as a strategic response. We found that women with past-year IPV were more likely to receive screening (aOR: 2.0, 95%CI: 1.2,3.5) and to consider counseling discussions to be a strategic response to IPV exposure (aOR: 2.7

Treatment may influence self-report and jeopardize our understanding of outcome.

OBJECTIVE: Standardized measures of self-reported alcohol use are the predominant method by which change in alcohol use following interventions is evaluated. This study examined whether the invariance of the test-retest pretreatment Alcohol Use Disorders Identification Test (AUDIT) was affected by the treatment experience. In this study, the intervening exposure was to motivational interviewing (MI) versus community service (CS), the treatment-as-usual control group. METHOD: Analyses were conducted on a subsample of court-referred 16- to 21-year-olds recruited into a randomized controlled tri

Health workers' disrespectful and abusive behaviour towards women during labour and delivery: A qualitative study in Durban, South Africa.

BACKGROUND: A high prevalence of disrespectful and abusive behaviour by health workers towards women during labour and delivery has been widely described in health facilities, particularly in Africa, and is a worldwide public health concern. Such behaviours are barriers to care-seeking, and are associated with adverse outcomes for mothers and newborns. This paper reports experiences of disrespectful care among informal working women in three public health facilities in Durban, South Africa. METHODS: A qualitative longitudinal study was conducted among a cohort of informal working women recrui
