Bilateral vestibulopathy: Diagnostic criteria Consensus document of the Classification Committee of the Bárány Society.

This paper describes the diagnostic criteria for bilateral vestibulopathy (BVP) by the Classification Committee of the Bárány Society. The diagnosis of BVP is based on the patient history, bedside examination and laboratory evaluation. Bilateral vestibulopathy is a chronic vestibular syndrome which is characterized by unsteadiness when walking or standing, which worsen in darkness and/or on uneven ground, or during head motion. Additionally, patients may describe head or body movement-induced blurred vision or oscillopsia. There are typically no symptoms while sitting or lying down under stati

Is Cerebellar Ataxia, Neuropathy, and Vestibular Areflexia Syndrome (CANVAS) a Vestibular Ganglionopathy?

OBJECTIVES: CANVAS is an acronym for cerebellar ataxia, neuropathy and vestibular areflexia syndrome. Limited autopsy data has suggested that CANVAS is caused by a focal dorsal root ganglionopathy that damages Scarpa's (vestibular) ganglion, but spares the Spiral (hearing) ganglion. If the vestibular areflexia of CANVAS is in fact due to ganglionopathy, then there should be global reduction of all vestibular responses. MATERIALS AND METHODS: With this hypothesis in mind, a retrospective review of 5 subjects who met the clinical criteria for CANVAS was performed. Recent advances in vestibular

Vestibular function in patients with Niemann-Pick type C disease.

We investigated whether vestibular dysfunction may cause or contribute to postural imbalance and falls in patients with Niemann-Pick type C disease (NP-C). Eight patients with NP-C disease and 20 healthy controls were examined using the video-based head impulse test (vHIT) and caloric irrigation to investigate horizontal canal function as well as ocular- and cervical vestibular evoked myogenic potentials (o- and cVEMP), and binocular subjective visual vertical estimation (SVV) for otolith function, and static posturography. There were no significant differences in vestibulo-ocular gain, calori

Vestibular neuritis has selective effects on air- and bone-conducted cervical and ocular vestibular evoked myogenic potentials.

OBJECTIVE: To characterise the changes in cervical (cVEMP) and ocular (oVEMP) vestibular evoked myogenic potentials to different stimuli in patients with vestibular neuritis (VN). METHODS: cVEMPs and oVEMPs were recorded using air-conducted (AC; clicks and short tone bursts) and bone-conducted (BC; lateral impulses and taps) stimuli in VN patients (n=23) and normals (n=40). RESULTS: AC evoked cVEMPs revealed few abnormalities, significantly less than for AC evoked oVEMPs (cVEMP: 22% vs oVEMP: 68%, P<0.001). Lateral impulses showed high rates of abnormalities (74% vs 70%, P>0.05) for both ref

A pilot study of gabapentin as treatment for acquired nystagmus.

The effects of the anticonvulsant gabapentin were measured on vision and eve movements in three patients with acquired pendular nystagmus. In two patients, the nystagmus was associated with multiple sclerosis and, in the other, it followed brainstem stroke. A single oral 600 mg dose of gabapentin produced improvement of vision due to changes in ocular oscillations in all three patients. The effect was sustained after five weeks of treatment in two patients who elected to continue taking gabapentin 900-1500 mg/day. The results of this pilot study suggest that a controlled trial of gabapentin sh

Mal de debarquement syndrome: a forgotten entity?

Mal de debarquement syndrome is defined by a persistent sensation of rocking and swaying commonly felt with sea travel that is first noted on return to land. Mal de debarquement syndrome is not to be confused with seasickness, which causes nausea, vomiting, diaphoresis, and headache. Four female patients with mal de debarquement syndrome are reviewed. Mal de debarquement is normally a short-lived phenomenon. The need for extensive evaluations may be avoided by an awareness of this entity.

Effects of rectilinear acceleration, caloric and optokinetic stimulation of human subjects in the Spacelab D-1 mission.

A set of vestibular experiments was performed during the course of the German Spacelab D-1 mission from 30 October to 6 November 1985 by a consortium of experimenters from various european countries. Similar to the Spacelab SL-1 mission all of the scientific crew members were theoretically and practically trained for the experiments. Baseline measurements for all tests were collected 113, 86, 44, 30 and 18 days prior to the mission and compared with data taken inflight, on the landing day and the consecutive 7 to 14 days. The hardware comprised mainly a motordriven accelerating platform, the S

The evaluation of gravitational recognition in patients with spinocerebellar degeneration using Listing's plane.

Background: The thickness of Listing's plane accurately reflects gravitational recognition, constructed through the integration of visual, vestibular, and proprioceptive sensory input in the neural integrator of the central nervous system. Spinocerebellar degeneration (SCD) is a progressive degenerative disease mainly involving the cerebellum and brainstem that leads to destabilize the gravitational recognition, and may thicken thickening of Listing's plane. Objective: The aim of this study is to evaluate the gravitational recognition in response to positional change in patients with SCD, usin

A comparison of water and air caloric responses and their ability to distinguish between patients with normal and impaired ears.

OBJECTIVES: The caloric test is a mainstay of modern vestibular assessment. Yet caloric test methods have not been well standardized, and normal response values have not been universally agreed upon. The air caloric test has been particularly problematic. In this article, we present our efforts to establish a population-based description of the caloric response evoked by water and air stimuli at both cool and warm temperatures. DESIGN: Data were collected from a retrospective record review of patients who underwent caloric testing at Mayo Clinic Jacksonville between 2002 and 2006. Two subgrou

Stance performance under different sensorimotor conditions in patients with post-traumatic otolith disorders.

Dynamic posturography with the Equitest system is helpful to screen patients for balance deficits, but diagnostic specificity, for otolith disorders is unknown until now. It was therefore the aim of our present paper to examine patients with a well-defined otolith disorder on the Equitest at different test conditions while simultaneously recording trunk sway by means of the Sway Star system. A total of 22 patients with different types of otolith disorders were included in this study. All test results of the patients were matched with respect to age and gender to controls without history of ENT

Responses of locus coeruleus neurons to caloric stimulation in rats.

We examined the effects of caloric stimulation on the neuronal activity of the locus coeruleus (LC) and of the vestibular nucleus complex (VNC) in urethan-anesthetized rats. The single unit activity of neurons in the LC and VNC was extracellularly recorded. A polyethylene tube for caloric stimulation was inserted into the middle ear cavity on the ipsilateral side. Through the tube, the middle ear was irrigated by hot (44 degrees C), cold (30 degrees C), and ice (4 degrees C) water. The majority of neurons in the VNC showed excitation by middle ear irrigation with hot water and inhibition by ic

Effect of changes in blood pressure on per-rotational nystagmus in guinea pigs with obliterated endolymphatic sac.

The effect of blood pressure (BP) changes on per-rotational nystagmus (PRN) was compared between normal guinea pigs and guinea pigs with unilaterally obliterated endolymphatic sac. Eye movements were recorded 3 months postoperatively. When the animal was rotated sinusoidally, asymmetry of PRN was minimal in the control animals. In the operated guinea pigs, however, nystagmus oscillating toward the operated side was largely suppressed when BP was decreased (30-40 mmHg). Thus, directional preponderance of PRN toward the nonoperated side was established. Oxygen tension (pO2) was measured in the p

Acute Imbalance Syndrome vs Acute Vestibular Syndrome: Clinical and Topographical Differentiation.

BACKGROUND AND OBJECTIVES: Stroke is a major differential diagnosis in patients with acute vertigo/dizziness or imbalance. Current diagnostic algorithms such as head impulse test, gaze-evoked nystagmus, and test of skew are optimized for presentations with spontaneous nystagmus (SPN), traditionally called acute vestibular syndrome (AVS). However, in recent studies, a relevant proportion of patients with stroke-related vertigo/dizziness had no SPN, but rather postural or gait imbalance, a phenotype lately termed acute imbalance syndrome (AIS). We aimed to outline the pathoanatomical differences

Telemedicine Evaluations in Neuro-Ophthalmology During the COVID-19 Pandemic: Patient and Physician Surveys.

BACKGROUND: The novel coronavirus 2019 (COVID-19) pandemic has transformed health care. With the need to limit COVID-19 exposures, telemedicine has become an increasingly important format for clinical care. Compared with other fields, neuro-ophthalmology faces unique challenges, given its dependence on physical examination signs that are difficult to elicit outside the office setting. As such, it is imperative to understand both patient and provider experiences to continue to adapt the technology and tailor its application. The purpose of this study is to analyze both neuro-ophthalmology physi

Short-term retention effect of rehabilitation using head position-based electrotactile feedback to the tongue: influence of vestibular loss and old-age.

Our objective was to evaluate whether the severity of vestibular loss and old-age (>65) affect a patient's ability to benefit from training using head-position based, tongue-placed electrotactile feedback. Seventy-one chronic dizzy patients, who had reached a plateau with their conventional rehabilitation, followed six 1-h training sessions during 4 consecutive days (once on days 1 and 4, twice on days 2 and 3). They presented bilateral vestibular areflexia (BVA), bilateral vestibular losses (BVL), unilateral vestibular areflexia or unilateral vestibular losses and were divided into two age-su
