[Fertility of the aging male].

Although the effect of ageing on female fertility has been well documented, the reproductive functions of ageing males have been less extensively investigated. Unlike females, males do not experience a sudden arrest of gonadal endocrine and exocrine functions characteristic of menopausal ovarian deficiency. It therefore appeared useful to review the course of male fertility with age and the genetic consequences for children related to paternal ageing. The most recent histological and endocrine examinations demonstrate progressive alterations of testicular endocrine and exocrine function with a

Ejaculatory physiology and pathophysiology: assessment and treatment in male infertility.

Azoospermia is a heterogeneous condition with multiple etiologies and a variety of treatments. In this chapter we present a summary of retrograde ejaculation and anejaculation, both of which are characterized by an absence of antegrade semen propulsion through the male reproductive tract. Each of these affects fertility, but is pathophysiologically distinct disorders with differing evaluation and treatment. Retrograde ejaculation has a myriad of well-characterized causes, from pharmacologic disruption to interference of neural mechanisms by surgical intervention for a variety of diseases. Medi

Racialized ethics of surrogacy: Transnational reproductive tourism and local surrogacy practices in Uganda.

This study examines surrogacy practices in Uganda by analyzing how various actors operationalize "ethical surrogacy" in inbound and local contexts. Drawing on ethnographic fieldwork in Kampala, the study compares surrogacy programs organized by a foreign agency for Western intended parents with locally arranged surrogacy involving Ugandan agents and medical practitioners. The foreign agency promotes ethics through narratives of fair compensation and emotional connection. In contrast, local professionals articulate an ethical framework centered on strict separation to prevent emotional attachme

Fertility treatment using assisted reproductive technology during immune checkpoint inhibition: report of two cases.

There are limited data to guide fertility counseling for patients on prolonged immune checkpoint inhibition. Concerns include potential teratogenicity, decline in ovarian reserve and spermatogenesis, and endocrine immune-related adverse events such as hypophysitis. As indications for checkpoint inhibition expand, an increasing number of reproductive-aged patients face fertility-related decisions around active therapy. We present two patient cases, one male and one female, who used assisted reproductive technology to support family building during maintenance immune checkpoint inhibitor therapy

Single-cell and proteomic profiling unveil aberrant immune phenotypes in gestational diabetes following assisted reproductive technology.

Pregnant women undergoing assisted reproductive technology (ART) have an increased risk of gestational diabetes mellitus (GDM), potentially linked to altered immune regulation, but the underlying circulating immune features remain unclear. Here we show an integrated single-cell transcriptomic, immune receptor repertoire, and plasma proteomic analysis of peripheral blood from 32 women with GDM and 31 normal pregnancies after ART. GDM is associated with increased proportions of CD8+ T cells and elevated plasma levels of CD6, CXCL5, MMP10, and 4E-BP1. Cytotoxic CD8+ T cell subsets display enhance

Extreme ambient heat and outcomes of assisted reproduction in the continental United States, 1996 to 2018.

BACKGROUND: While extreme ambient heat has been associated with several adverse pregnancy and birth outcomes little is known regarding the link between extreme ambient temperatures and human fertility. OBJECTIVE: To evaluate the associations between extreme ambient heat and assisted reproductive technology (ART) outcomes. METHODS: Our analysis includes 1,087,379 patients in the continental United States who underwent 1,648,907 autologous oocyte, fresh embryo ART cycles reported to the National ART Surveillance System (1996-2018). Daily ambient temperature was derived by linking patient resid

[Growth hormone and fertility: molecular mechanisms and experience of adjuvant therapy in reproductology].

Growth hormone (GH) and insulin-like growth factor-1 (IGF-1) are pivotal hormones regulating not only postnatal growth, development, aging, and longevity but also female reproductive function. In clinical practice, GH is widely utilized as an ad juvant therapy to enhance various clinical outcomes in assisted reproductive technology (ART). It is aimed at increasing the yield and quality of oocytes and embryos, as well as improving pregnancy and live birth rates both in the general population and among patients with GH deficiency. Over the past three decades, the roles of GH and IGF-1 in regulat

Variation in gestational outcomes among PCOS phenotypes in natural and art-conceived pregnancies.

OBJECTIVE: Polycystic ovary syndrome (PCOS) is the most common endocrine disorder affecting women of reproductive age. While diagnostic criteria have historically varied, the Rotterdam criteria, established in 2003, are now the standard. This classification identifies four PCOS phenotypes based on the presence and/or absence of three key features: ovulatory dysfunction (OD), hyperandrogenism (HA), and polycystic ovarian morphology (PCOM). Despite phenotypic differences, current clinical guidelines treat PCOS as a homogeneous condition when assessing pregnancy risks. Therefore, this narrative r

Platelet-Rich Plasma in Sperm Processing for Assisted Reproductive Technology: Molecular Mechanisms, Clinical Applications, and Future Directions.

Male infertility contributes to nearly half of infertile couples, with asthenozoospermia and oligoasthenoteratozoospermia as predominant factors. Despite advancements in sperm processing techniques, the outcomes remain limited in severe cases, particularly concerning motility, mitochondrial function, and DNA integrity. Platelet-rich plasma (PRP), an autologous concentrate rich in platelets (>1 × 106/μL) and growth factors, has recently gained attention as an adjunctive therapy in andrology and assisted reproduction. This review systematically evaluated studies published between 2015 and 2025 i

Fertility Considerations for Gender-Diverse People.

Individuals pursuing gender-affirming care should receive comprehensive counseling regarding their reproductive options and be offered fertility preservation prior to medical or surgical transition. All fertility preservation and infertility treatment modalities available to cisgender individuals are also available to transgender individuals. Fertility preservation should prioritize methods with the highest potential for future live birth, particularly mature oocyte or sperm cryopreservation. Gender-affirming hormone therapy may reduce spontaneous fertility, though this effect is incomplete an

Diagnosis and treatment of luteal phase deficiency: a Committee Opinion.

Luteal phase deficiency (LPD) is a clinical diagnosis associated with abnormal luteal phase length of ≤10 days. Potential etiologies of LPD include inadequate progesterone duration, inadequate progesterone levels, or endometrial progesterone resistance. Luteal phase deficiency has been described in association with medical conditions, but also in fertile, normally menstruating women. Although progesterone is important for the process of implantation and early embryonic development, LPD has not been proven to be an independent entity causing infertility or recurrent pregnancy loss. Controversy

Antisperm antibodies and autoimmunity in the era of assisted reproduction.

Male factor infertility acts as a contributing or sole cause in approximately half of all infertility cases, with autoimmunity against spermatozoa-manifested as antisperm antibodies (ASA)-affecting 5%-12% of infertile men. The ASA formation typically results from a disruption of the blood-testis barrier because of trauma, obstruction, or inflammation, leading to the exposure of immunogenic sperm antigens to the systemic immune system. These antibodies, predominantly of the immunoglobulin G and immunoglobulin A classes, impair fertility by hindering sperm motility, preventing cervical mucus pen

Special considerations in assisted reproductive technology for patients with hematologic disease.

Populations with hematologic disorders seeking fertility care often present with a complex clinical picture, including disease-specific sequelae and comorbid conditions. Limited literature exists to help guide fertility clinics on the management of these patients, many of whom require multidisciplinary care coordination centered on patient-specific fertility goals. Thanks to advancements in life-prolonging therapies for hematologic disorders, growing numbers of affected individuals are seeking assisted reproductive technologies for family building. Thus, it is important to be aware of the uniq

Influence of Varicocelectomy on Assisted Reproductive Technology Outcomes of Infertile Patients with Varicocele: A Systematic Review and Meta-Analysis.

Varicocele can lead to impaired semen parameters and induce infertility. Varicocelectomy is considered the gold standard for varicocele treatment. However, its impact on improving assisted reproductive technologies (ARTs) outcomes remains contentious. This study seeks to compare reproductive outcomes between infertile men who underwent varicocelectomy and those who did not prior to ARTs. In addition, it evaluates the influence of types of fertilization, couples' ages, and treatment years on clinical pregnancy rates. A comprehensive search was conducted through February 2023 across eight electr

Delayed transitions to adulthood and assisted reproduction: A study of educational differences in Spain.

Transitions to adulthood are increasingly delayed in low-fertility countries, particularly among highly educated women, with significant implications for the timing of attempts to conceive and parenthood. Delayed childbearing increases the risk of infertility and the reliance on assisted reproductive technologies (ART). Spain has experienced pronounced delays in transitions to adulthood alongside a substantial rise in ART use over recent decades. This research adopts a life course approach to examine the association between delayed transitions to adulthood, the likelihood of using ART, and the
