Phenothiazine treatment and respiratory distress syndrome in a child.

An 11-year-old boy who was treated with a relatively high dose of methotrimeprazine meleate (Levemepromazine) a phenothiazine antipsychotic drug, was admitted to the pediatric intensive care unit suffering from respiratory distress syndrome. He required intensive treatment and support for 13 days. The persistent effects of methotrimeprazine meleate on various organs are typical of the prolonged biological action of the phenothiazine metabolites. The association of phenothiazine overdose and respiratory distress syndrome merits consideration.

Treatment of hypersomnolence in myotonic dystrophy with a CNS stimulant.

A 42-year-old man with myotonic dystrophy developed severe hypersomnolence during the year prior to admission for acute respiratory insufficiency. However, his hypersomnolence could be attributed to a central dysfunction, and during a follow-up period of 3 years, was successfully treated with methylphenidate.

A deep intronic TCTN2 variant activating a cryptic exon predicted by SpliceRover in a patient with Joubert syndrome.

The recent introduction of genome sequencing in genetic analysis has led to the identification of pathogenic variants located in deep introns. Recently, several new tools have emerged to predict the impact of variants on splicing. Here, we present a Japanese boy of Joubert syndrome with biallelic TCTN2 variants. Exome sequencing identified only a heterozygous maternal nonsense TCTN2 variant (NM_024809.5:c.916C >T, p.(Gln306Ter)). Subsequent genome sequencing identified a deep intronic variant (c.1033+423G>A) inherited from his father. The machine learning algorithms SpliceAI, Squirls, and Pang

Association between major depressive disorder and multiple disease outcomes: a phenome-wide Mendelian randomisation study in the UK Biobank.

Depression affects all aspects of an individual's life but evidence relating to the causal effects on health is limited. We used information from 337,536 UK Biobank participants and performed hypothesis-free phenome-wide association analyses between major depressive disorder (MDD) genetic risk score (GRS) and 925 disease outcomes. GRS-disease outcome associations passing the multiple-testing corrected significance threshold (P < 1.9 × 10-3) were followed by Mendelian randomisation (MR) analyses to test for causality. MDD GRS was associated with 22 distinct diseases in the phenome-wide discover

Mitochondrial disorders.

Mitochondrial respiration, the most efficient metabolic pathway devoted to energy production, is at the crosspoint of 2 quite different genetic systems, the nuclear genome and the mitochondrial genome (mitochondrial DNA, mtDNA). The latter encodes a few essential components of the mitochondrial respiratory chain and has unique molecular and genetic properties that account for some of the peculiar features of mitochondrial disorders. However, the perpetuation, propagation, and expression of mtDNA, the majority of the subunits of the respiratory complexes, as well as a number of genes involved i

Stability, continence and breathing: the role of fascia following pregnancy and delivery.

Pregnancy-related pelvic girdle pain (PRPGP) has a prevalence of approximately 45% during pregnancy and 20-25% in the early postpartum period. Most women become pain free in the first 12 weeks after delivery, however, 5-7% do not. In a large postpartum study of prevalence for urinary incontinence (UI) [Wilson, P.D., Herbison, P., Glazener, C., McGee, M., MacArthur, C., 2002. Obstetric practice and urinary incontinence 5-7 years after delivery. ICS Proceedings of the Neurourology and Urodynamics, vol. 21(4), pp. 284-300] found that 45% of women experienced UI at 7 years postpartum and that 27%

Respiratory rhythm irregularity after carotid body denervation in rats.

Respiratory activity is controlled by inputs from the peripheral and central chemoreceptors. Since overactivity of the carotid bodies, the main peripheral chemoreceptors, is linked to the pathophysiology of disparate metabolic and cardiovascular diseases, carotid body denervation (CBD) has been proposed as a potential treatment. However, long-term effects of CBD on the respiratory rhythm and regularity of breathing remain unknown. Here, we show that five weeks after bilateral CBD in rats, the respiratory rhythm was slower and less regular. Ten weeks after bilateral CBD, the respiratory frequen

Respiratory dysfunction and management in spinal cord injury.

Respiratory dysfunction is a major cause of morbidity and mortality in spinal cord injury (SCI), which causes impairment of respiratory muscles, reduced vital capacity, ineffective cough, reduction in lung and chest wall compliance, and excess oxygen cost of breathing due to distortion of the respiratory system. Severely affected individuals may require assisted ventilation, which can cause problems with speech production. Appropriate candidates can sometimes be liberated from mechanical ventilation by phrenic-nerve pacing and pacing of the external intercostal muscles. Partial recovery of res

[Sleep and respiratory disorders in myotonic dystrophy of Steinert].

BACKGROUND: It has been hypothesized that hypersomnia and sleep related respiratory impairment are both central in origin in myotonic dystrophy. OBJECTIVE: To describe by means of video-polysomnographic recordings the central origin of the sleep respiratory disorders. PATIENTS AND METHODS: We studied 11 patients, 6 men and 5 women (mean age 42.7 years) with myotonic dystrophy. A moderate to severe ventilatory impairment of a primarily restrictive type was seen in all patients, three of them after the first episode of respiratory insufficiency. The patients were evaluated in order to determin

The diaphragm and respiratory muscles.

This article describes the functional anatomy of the muscles that contribute to the generation of ventilatory pressures. The physiologic principles that guide their function are analyzed, and their composition and metabolism are described, inasmuch as this affects their adaptation and capacity to face acute and chronic loads. Special emphasis is placed on the clinical and laboratory evaluation of their function. In addition, the concept of respiratory muscle fatigue is developed so the reader can place all these concepts in the context of their clinical applications.

Ventilatory support of the trauma patient with pulmonary contusion.

Pulmonary contusion is a common clinical condition seen in patients with thoracic trauma. The mortality rate from isolated pulmonary contusion is low, but when combined with other severe injuries patients with pulmonary contusion can have mortality rates as high as 10% to 57%. Early aggressive mechanical ventilatory support can prevent progressive atelectasis and worsening of arterial oxygenation. Careful titration of ventilatory support to physiologic end points is a reasonable approach. Unfortunately, data comparing the means of achieving the end points are lacking and no published prospecti

Epidemiological analysis of the increased risk of disturbed neonatal respiratory adaptation after caesarean section.

In a prospective, unselected study of all 4,659 infants born in Göteborg, Sweden, risk factors for all kinds of neonatal respiratory disturbances (RD) after Caesarean section (CS) were analyzed. After CS, a significantly increased incidence rate of RD was found compared to vaginal delivery (24.6% vs. 5.5%). The increased overall risk affected full term infants only but IRDS was more common after CS in preterm infants. Rupture of membranes or uterine contractions prior to CS significantly reduced the incidence rate of RD in full term infants. Acute maternal complications did not affect the inci

[Mechanical effects of funnel chest (author's transl)].

In the assessment of the deformed chest wall, morphologic and cosmetic considerations are prominent. Classification should not depend on the observer's subjective impression. The measuring procedure presented and tested in 142 patients shows that relationships exist between the shape of the chest wall and the position of the heart, but no utilizable correlations to total function values. A growth-dependent progression of thoracic space requirement is not be expected as a rule.

Biallelic Variants in SLC27A3 Cause a Complex Form of Neurodegeneration with Brain Iron Accumulation.

BACKGROUND: Complex lipid metabolism is one of the main biological pathways disrupted in neurodegeneration with brain iron accumulation (NBIA). SLC27A3 gene encodes for the very long-chain acyl-CoA synthetase 3, an acyl-CoA ligase that activates long and very long-chain fatty acids. OBJECTIVE: We report on a 19-year-old patient with an NBIA pattern harboring a homozygous, nonsense SLC27A3 variant. METHODS: SLC27A3 variants were identified using whole exome sequencing (WES). Their impact on protein function was assessed in patient fibroblasts using Western blot analysis, aerobic metabolism an

Sleep and medical disorders.

Extensive evidence links cardiovascular disease and sleep disordered breathing. OSA has adverse effects on blood pressure, cardiovascular status, and mortality. Effective CPAP therapy can improve blood pressure and cardiac function in patients who have OSA. Patients who have congestive heart failure have a high prevalence of sleep-disordered breathing, with OSA occurring in 30% of such patients and Cheyne-Stokes respiration in 40%.CPAP is the preferred mode of therapy for both types of sleep-disordered breathing in patients who have coexistent congestive heart failure. [Respiratory failure: respiratory disorder during sleep in patients with myotonic dystrophy].

Myotonic dystrophy (MyD) patients have been reported to show severe nocturnal desaturation related to sleep respiratory disorders. However, the reason of respiratory failure in MyD has remained unclear. In this study, ten patients with MyD underwent overnight polysomnography to evaluate the mechanisms which would cause respiratory failure, compared with Duchenne muscular dystrophy (DMD) patients in these three views: 1) sleep-related nocturnal desaturation, 2) histopathological evaluation of respiratory muscles, and 3) abnormalities of respiratory center. Nocturnal desaturation was more promin

Mesenchymal Stromal Cells and Extracellular Vesicles: A Novel Therapeutic Paradigm for Mitochondrial Dysfunctions.

Mitochondrial dysfunction is a central pathological feature of a wide range of inherited and acquired disorders and is characterized by impaired oxidative phosphorylation, disrupted cellular energy metabolism, and excessive oxidative stress. Although advances in molecular diagnostics have improved disease recognition, effective disease-modifying therapies remain limited, and clinical outcomes are often suboptimal, highlighting the need for novel therapeutic strategies. Mesenchymal stromal cells (MSCs) and their extracellular vesicles (MSC-EVs) have emerged as promising candidates for targeting

Intercellular Communication-Driven Mitochondrial Transfer via CXCR4-Engineered Macrophages Reprograms Bone Marrow Metabolism for Osteoporosis Therapy.

Osteoporosis is characterized by impaired bone formation and disrupted bone marrow homeostasis, largely driven by mitochondrial dysfunction in bone marrow mesenchymal stem cells (BMSCs). To address this, a live mitochondrial delivery system composed of CXCR4-engineered macrophages loaded with nanozyme-functionalized mitochondria (CM-MTBM). This system integrates bone-targeted migration, reactive oxygen species scavenging, and communication-mediated mitochondrial transfer. CM-MTBM restores mitochondrial respiration, enhances osteogenic differentiation, and alleviates oxidative apoptosis in BMSC
