BACKGROUND: Operative management of flail chest injury is receiving increasing interest. However, we have noticed in our own practice the difficulty in achieving reliable results with posterior rib fracture fixation. In this article, we analyze and model the physiologic forces acting on posterior rib fractures and assess the suitability of an intramedullary screw fixation technique in this site.

BACKGROUND: Large defects of the anterior wall of the frontal sinus require closure using either autologous or foreign material. In cases of osteomyelitis, the reconstruction must be resistant to bacterial infection. Split-rib osteoplasty can be used in different sites.
METHODS: Two patients with malignant sinonasal tumours underwent repeated treatment, and subsequently developed osteomyelitis of the frontal bone. After adequate therapy, a large defect of the anterior wall persisted. Reconstruction was performed using the split-rib method. The literature on this topic was reviewed.

Rib fractures in infants and young children are highly associated with abuse. Fractures near the costovertebral junction carry a high specificity for abuse because they result from excessive anteroposterior chest compression, which may occur when a child is gripped around the chest by an assailant. Benign fibrous histiocytoma (BFH) of bone is rare in occurrence, and rib is an unusual site. Hyper IgE syndrome (HIES) is a rare immunodeficiency syndrome characterized by a triad of cutaneous abscesses, mostly caused by Staphylococus aureus; pneumonia; and raised IgE levels. Nonimmunological associations include course facial features, multiple bone fractures, joint hyperextensibility, and retained primary dentition. Patients require long-term antibiotic therapy. We report here a classical case of HIES with rare associations of natal teeth, bilateral cervical ribs, and conductive deafness. The patient was being treated with monteleukast and dapsone.

We present a new model-based approach for an automated labeling and segmentation of the rib cage in chest CT scans. A mean rib cage model including a complete vertebral column is created out of 29 data sets. We developed a ray search based procedure for rib cage detection and initial model pose. Salmonella osteomyelitis of the rib is a rare clinical entity. In our case, a muhidrug resistant Salmonella enterica serotype Typhi was isolated from an immuno-competent patient with osteomyclitis of the ribs, who was treated earlier with ciprotloxacin for typhoid fever. The patient was successfully treated for osteomyclitis with intravenous ceftriaxone.

Anomalous accumulation of intermediate filaments, lipid and glycogen, may substitute the cytoplasm organells in Marfan's syndrome. Pathologic disturbances of costal cartilage, connected with ultrastructural changes of chondrocytes, intercellular substance and extracellular matrix are morphologic substrate of different chest deformations.

A morphological study of the skeletal system in a case of short rib polydactyly syndrome type III (SRPS-III) documented a "bajonet" deformity of the ribs for misalignment and overlap of cartilaginous and bony ends.
