The biomechanics of the supraspinatus-deficient shoulder treated with superior capsular reconstruction vs. reverse total shoulder arthroplasty-experimental study.

AIM OF THE STUDY: Our objective was to compare biomechanical effects of superior capsular reconstruction (SCR) and reverse total shoulder arthroplasty (rTSA) on shoulder motion, in the setting of an irreparable supraspinatus (SS) tear. We hypothesized that rTSA would produce greater improvement in abduction force and shift the humerus inferiorly, while SCR would produce greater range of motion (ROM) and prevent superior migration of the humerus during abduction. METHODS: Six cadaveric shoulders were evaluated using a custom biomechanical apparatus. Each shoulder underwent four experimental co

Rotator Cuff Dysfunction after Anatomic Total Shoulder Arthroplasty: Who is at Risk?

We compare posttotal shoulder arthroplasty (TSA) patients with suspected cuff dysfunction to a successfulTSA control group. Thirtynine patients met suspectedcuffdysfunction criteria. Forty matched controls were selected. Radiographic and functional outcomes and demographics were compared preoperatively and postoperatively. Humeral head height above the greater tuberosity and postoperative humeral head translations were significantly greater in the dysfunction group. The dysfunction group demonstrated a significant correlation between higher humeral heads and earlier postoperative dysfunction,

Regeneration of a full-thickness defect of rotator cuff tendon with freshly thawed umbilical cord-derived mesenchymal stem cells in a rat model.

BACKGROUND: It is difficult to immediately use mesenchymal stem cells (MSCs) for the patient with rotator cuff disease because isolation and culture time are required. Thus, the MSCs would be prepared in advanced in cryopreserved condition for an "off-the-shelf" usage in clinic. This study investigated the efficacy of freshly thawed MSCs on the regeneration of a full-thickness tendon defect (FTD) of rotator cuff tendon in a rat model. METHODS: We evaluated morphology, viability, and proliferation of cultured umbilical cord-derived MSCs (C-UC MSCs) and freshly thawed umbilical cord-derived MSC

Comparison of Surgeon Performance of Rotator Cuff Repair: Risk Adjustment Toward a More Accurate Performance Measure.

BACKGROUND: Movement toward a value-based health-care system necessitates the development of performance measures to compare physicians, hospitals, and health-care systems. Patient-reported outcomes (PROs) are a potential metric. However, valid use of PROs hinges on the ability to risk-adjust for baseline patient differences across a surgeon's panel of patients. The purpose of this study was to propose an approach for baseline risk adjustment and evaluate the importance of risk adjustment when comparing surgeons' performance of rotator cuff repair. METHODS: Patients (n = 995) treated with art

Tissue-engineered tendon constructs for rotator cuff repair in sheep.

Current rotator cuff repair commonly involves the use of single or double row suture techniques, and despite successful outcomes, failure rates continue to range from 20 to 95%. Failure to regenerate native biomechanical properties at the enthesis is thought to contribute to failure rates. Thus, the need for technologies that improve structural healing of the enthesis after rotator cuff repair is imperative. To address this issue, our lab has previously demonstrated enthesis regeneration using a tissue-engineered graft approach in a sheep anterior cruciate ligament (ACL) repair model. We hypot

Early versus late repair of rotator cuff tears in rats.

BACKGROUND: In the event of a traumatic rotator cuff tear, patients are routinely advised that early surgical intervention produces an optimal repair, despite a lack of direct evidence to support this recommendation. To address this knowledge gap, massive rotator cuff tears in rats were assessed by biomechanical and bone morphometric analyses after early or late repair. METHODS: Combined supraspinatus and infraspinatus tendon tears of the left shoulder were created in 21 adult Wistar rats, which were divided into 2 groups. The tendons of the injured shoulder in the animals in group I were sur

Muscle progenitor cell regenerative capacity in the torn rotator cuff.

Chronic rotator cuff (RC) tears affect a large portion of the population and result in substantial upper extremity impairment, shoulder weakness, pain, and limited range of motion. Regardless of surgical or conservative treatment, persistent atrophic muscle changes limit functional restoration and may contribute to surgical failure. We hypothesized that deficits in the skeletal muscle progenitor (SMP) cell pool could contribute to poor muscle recovery following tendon repair. Biopsies were obtained from patients undergoing arthroscopic RC surgery. The SMP population was quantified, isolated, a

Age-related structural changes in upper extremity muscle tissue in a nonhuman primate model.

BACKGROUND: Longitudinal studies of upper extremity aging in humans include logistical concerns that animal models can overcome. The vervet is a promising species with which to study aging-related processes. However, age-related changes in upper extremity muscle structure have not been quantified in this species. This study measured age-related changes to muscle structure, examined relationships between muscle structure and measures of physical performance, and evaluated the presence of rotator cuff tears. METHODS: Muscle structure (volume, optimal fiber length, and physiologic cross-sectiona

Spectrum of suprascapular nerve lesions: normal and abnormal neuromuscular imaging appearances on 3-T MR neurography.

OBJECTIVE. In this article, we will review the normal anatomy and imaging features of various neuromuscular abnormalities related to suprascapular neuropathy. CONCLUSION. Suprascapular neuropathy can be difficult to distinguish from rotator cuff pathology, plexopathy, and radiculopathy. Electrodiagnostic studies are considered the reference standard for diagnosis; however, high-resolution 3-T MR neurography (MRN) can play an important role. MRN enables direct visualization of the nerve and simultaneous assessment of the cervical spine, brachial plexus, and rotator cuff.

Supraspinatus atrophy as a predictor of rotator cuff tear size: an MRI study utilizing the tangent sign.

HYPOTHESIS: We hypothesize that the degree of supraspinatus atrophy, as assessed with the tangent sign will significantly correlate with increasing rotator cuff tear size. METHODS: A retrospective review was performed assessing presence of muscle atrophy using the tangent sign, fatty atrophy based on Goutallier classification, and size of rotator cuff tear in 34 patients. A correlation analysis was performed. RESULTS: All agreement measures among both evaluators were statistically significant (P < .05). Agreement among the 2 physicians for the tangent sign was consider almost perfect k = 87.

Coverage of the humeral head by the coracoacromial arch: relationship with rotator cuff tears.

The pathogenetic roles of the coracoacromial arch in the development of rotator cuff tears are still controversial. This study compared the anteroposterior coverage of the humeral head by the coracoacromial arch between shoulders with and without full-thickness rotator cuff tears. Forty-two shoulders from 21 embalmed cadaveric specimens were macroscopically examined. Specimens were divided into 2 groups:shoulders with full-thickness cuff tears (tear group) and those with intact cuff tendons (normal group). The coverage angle of each component of the coracoacromial arch was measured using true

Arthroscopic repair techniques for massive rotator cuff tears.

Patients with massive rotator cuff tears present with pain, weakness, and loss of function. Candidates for arthroscopic repair include symptomatic, young, active patients; those with an acute tear or tears with early changes of atrophy; and patients willing to comply with recovery and rehabilitation processes after surgery. As massive rotator cuff tears extend, the glenohumeral articulation is destabilized, allowing superior migration. Repair of the force couples and reinforcement of the anterosuperior rotator cuff cable can restore functional elevation via the deltoid. Muscle changes, includi

Outcome of latissimus dorsi transfer for irreparable rotator cuff tears.

The aim of this retrospective study was to assess the mid- and long-term functional and radiological results following latissimus dorsi transfer for symptomatic irreparable tears of the supra- and infraspinatus tendons (postero-superior cuff tears). Twenty-six shoulders with an irreparable posterosuperior rotator cuff tear were assessed preoperatively and after latissimus dorsi transfer, using the Constant score and standard radiographs to determine the degree of osteoarthritis and to measure the acromio-humeral distance. After a minimal follow-up of one year (range: 13 to 124 months) we obser

The effect of screw position on the initial fixation of a reverse total shoulder prosthesis in a glenoid with a cavitary bone defect.

Patients with rotator cuff tear arthropathy can be treated successfully with a reverse total shoulder prosthesis. In patients with significant glenoid bone loss, achieving stable bone fixation can be challenging, as the surgeon must know when bone grafting is necessary and when the plan to implant the reverse total shoulder prosthesis should be abandoned because of the likelihood of early implant loosening. The purposes of this study were (1) to determine the initial stability of a metal glenoid implant fixed in a glenoid with a central cavitary defect and (2) to determine whether an altered s

Recurrent anterior glenohumeral instability with onset after forty years of age: the role of the anterior mechanism.

Recurrent instability in patients over forty years of age is felt to occur primarily as a result of an associated rotator cuff tear. This is often referred to as the "posterior mechanism." We reviewed our patients over the age of forty who underwent an anterior shoulder repair to identify the incidence of capsulolabral detachments and the role of an "anterior mechanism" in this patient population. A retrospective review of all patients from 1985 to 2000 was performed to identify patients who had surgery for recurrent instability that began after forty years of age. Of the 265 patients records
