Utility and Pitfalls of β-D-Glucan for Diagnosis and Response Monitoring of Chronic Disseminated Candidiasis in Paediatric Cancer Patients.

BACKGROUND: β-D-Glucan (BDG) is a useful but nonspecific biomarker in patients with suspected invasive fungal diseases including Pneumocystis pneumonia. Little is known, however, about its utility for response monitoring in chronic disseminated candidiasis (CDC). PATIENTS AND METHODS: We describe the utility and pitfalls of serum BDG in paediatric cancer patients with suspected CDC. BDG in serum was measured serially (i.e., 5 to 10 times of a time period of 200 to 400 days) by a commercially available assay (Fungitell; Associates of Cape Cod, MA, USA) and values were correlated to patient- an

Whole Abdominal Radiotherapy Is Tolerable and Effective in Children and Young Adults With Intra-Abdominal Sarcoma: A Canadian Sarcoma Research and Clinical Collaboration (CanSaRCC) Study.

BACKGROUND: Whole abdominal radiotherapy (WART) is utilized in children, adolescents, and young adults with intra-abdominal sarcoma, but efficacy and toxicity are uncertain. METHODS: Sarcoma patients diagnosed at ≤39 years of age between 1998 and 2023 who received WART were identified from a national registry. Patient and tumor characteristics, treatment, response, and survival outcomes were assessed. RESULTS: Twenty patients with desmoplastic small round cell tumor (DSRCT) (n = 7), rhabdomyosarcoma (RMS) (n = 6), Ewing sarcoma, rhabdoid tumor, endometrial stromal tumor (each n = 1), and sar

Slipped proximal humeral epiphysis: a complication of radiotherapy to the shoulder in children.

Slipped proximal humeral epiphyses as a sequela of radiotherapy were identified in two children treated for Ewing's sarcoma in whom the shoulder was included in the field of treatment. The radiation doses were slightly greater than those in children developing slipped capital femoral epiphyses following radiotherapy. The lesser stress about the shoulder compared to the hip is believed to be the reason this is an infrequent complication of radiotherapy.

Multiple aromatic side chains within a disordered structure are critical for transcription and transforming activity of EWS family oncoproteins.

Chromosomal translocations involving the N-terminal approximately 250 residues of the Ewings sarcoma (EWS) oncogene produce a group of EWS fusion proteins (EFPs) that cause several distinct human cancers. EFPs are potent transcriptional activators and interact with other proteins required for mRNA biogenesis, indicating that EFPs induce tumorigenesis by perturbing gene expression. Although EFPs were discovered more than a decade ago, molecular analysis has been greatly hindered by the repetitive EWS activation domain (EAD) structure, containing multiple degenerate hexapeptide repeats (consensu

Clear cell sarcoma of the mediastinum.

A 59-year-old woman presented with a large mediastinal mass. At thoracotomy, the mass was found tightly adherent to the esophageal wall and right lower lobe of the lung. Histological examination showed a solid tumor composed of closely packed nests of cells with clear and eosinophilic cytoplasm, which were strongly and diffusely positive for S100 protein but negative for HMB45 and Melan-A. The diagnosis of clear cell sarcoma was supported by demonstrating the presence of an EWS gene rearrangement by fluorescence in situ hybridization. There was no evidence that this lesion represented metastat

Dedifferentiated Osteosarcoma of the Wrist With BCOR Sarcoma Component in a 22-Year-Old Patient: A Case Report.

We report a unique case of a 22-year-old male with a progressively enlarging mass on the right thumb. Initial biopsies suggested a low-grade osteogenic tumor. Following partial amputation, gross examination revealed a well-circumscribed 3.7cm mass involving the metacarpal bone with soft tissue invasion. Histologically, the tumor displayed features of low-grade central osteosarcoma with areas transitioning to a round blue cell morphology resembling Ewing sarcoma. Comprehensive molecular testing-including next-generation sequencing (NGS) and cytogenomic microarray analysis (CMA)-identified ampli

Primary Adrenal BCOR::CCNB3 Sarcoma: Fine Needle Aspiration Cytology Findings.

Pediatric adrenal tumors are uncommon, with the most common being peripheral neuroblastic tumors. Other neoplasms like adrenal cortical tumors, pheochromocytoma, and myelolipoma may also be encountered in this age group. Rare instances of mesenchymal tumors such as Ewing sarcoma and rhabdomyosarcoma have been described. We report the fine needle aspiration cytology (FNAC) findings of the first case of BCOR-altered sarcoma occurring in the adrenal gland. A 13-year-old boy presented with severe abdominal pain. Imaging revealed a mass in the right adrenal gland. FNAC showed loosely cohesive clust

Laryngeal sarcomas: A case series of 5 cases.

Primary sarcomas of the larynx are rare and miscellaneous. The most common is chondrosarcoma. Other sarcomas are very rare. Sarcomas can have heterogeneous morphologic features of spindle, small round, epithelioid, pleomorphic and giant cells. Laryngeal sarcomas may mimic carcinomas, lymphomas, small cell carcinoma, mesothelioma and melanoma. This imposes diagnostic challenges for unfamiliar pathologists particularly in small laryngeal biopsies. Our aim was to study the different types of sarcomas that can involve the larynx in our institution, to investigate their diagnostic challenges and po

Intracranial Myeloid Sarcoma as the First Manifestation of Acute Monoblastic Leukemia.

Background Myeloid sarcoma, also called granulocytic sarcoma, extramedullary myeloid tumor, or chloroma, is a manifestation of a myeloid neoplasm. These tumors can develop in various anatomical sites, including the central nervous system, presenting as an extra-axial tumor affecting the meninges or the ependyma. Diagnosis is challenging due to its resemblance to other intracranial tumors such as meningiomas. Case Description We present the case of a 17-year-old male with intermittent left frontotemporal headache, diplopia during horizontal gaze, and proptosis of the left eye. During the imagin

Tumor-targeting Salmonella typhimurium A1-R is a highly effective general therapeutic for undifferentiated soft tissue sarcoma patient-derived orthotopic xenograft nude-mouse models.

Undifferentiated soft tissue sarcoma (USTS) is a recalcitrant and heterogeneous subgroup of soft tissue sarcoma with high risk of metastasis and recurrence. Due to heterogeneity of USTS, there is no reliably effective first-line therapy. We have generated tumor-targeting Salmonella typhimurium A1-R (S. typhimurium A1-R), which previously showed strong efficacy on single patient-derived orthotopic xenograft (PDOX) models of Ewing's sarcoma and follicular dendritic cell sarcoma. In the present study, tumor resected from 4 patients with a biopsy-proven USTS (2 undifferentiated pleomorphic sarcoma

Postradiation sarcoma of bone: review of 78 Mayo Clinic cases.

Postradiation sarcoma of bone is an uncommon but serious sequela of radiation therapy. Seventy-eight Mayo Clinic patients have been treated for sarcomas arising in irradiated bones. They received their initial radiotherapy for a wide variety of nonneoplastic and neoplastic conditions, both benign and malignant. Thirty-five sarcomas arose in bone that was normal at the time of radiotherapy, and 43 arose in irradiated preexisting osseous lesions. The latent period between radiotherapy and diagnosis of sarcoma averaged 14.3 years. Ninety percent of the postradiation sarcomas were either osteosarc

Use of modern three-dimensional imaging models to guide surgical planning for local control of pediatric extracranial solid tumors.

INTRODUCTION: In complex pediatric surgical oncology, surgical planning is contingent upon data gathered from preoperative imaging. Three-dimensional (3D) modeling and printing has been shown to be beneficial for adult presurgical planning, though pediatric literature is less robust. The study reviews our institutional experience with the use of 3D image segmentation and printed models in approaching resection of extracranial solid tumors in children. METHODS: This is a single institutional series from 2021 to 2023. Models were based on computed tomography and magnetic resonance imaging studi

Intramyofiber invasion in a case of oral squamous cell carcinoma; an unusual pattern of skeletal muscle infiltration.

Tumor infiltration in the skeletal muscle is a rare entity and usually associated with aggressive tumor phenotype. Intramyofiber skeletal muscle invasion is the rarest pattern of muscle invasion, where the architecture of the individual myofibers remain preserved. This uncommon pattern of skeletal muscle invasion has occasionally been identified in Ewing sarcoma, non-Hodgkin lymphomas and leukemias however there are sparse literatures available on carcinomas showing intramyofiber pattern of invasion. They can closely mimic myositis and can be missed on low magnification. Thus, careful examinat

Let it glow: Intraoperative visualization of pulmonary metastases using pafolacianine, a next-generation fluorescent agent, for young adults undergoing pulmonary metastasectomy.

A new generation of disease-specific molecular imaging agents is poised to revolutionize fluorescence-guided surgery. Pafolacianine has been approved for adult lung and ovarian cancers. We demonstrate a proof of concept for pediatric surgeons treating young adults with pulmonary metastatic sarcomas. Five successful fluorescence-guided pulmonary metastasectomy operations were performed in young adult patients with metastatic osteosarcoma or Ewing sarcoma following administration of pafolacianine. All osteosarcoma lesions identified using standard techniques were also markedly fluorescent in pat

The role of molecular pathology in mediastinal sarcomas.

Mediastinal sarcomas represent rare neoplasms of mesenchymal origin. Most published data on mediastinal sarcomas is primarily derived from small series and case reports. Although rare, primary mediastinal sarcomas have a clinically aggressive course with worse 10-year survival rates than other types of mediastinal tumors, highlighting the importance of adequate diagnosis of these lesions. The diagnosis of mediastinal sarcomas is complicated by the varied histologic subtypes of tumors that can occur and which can sometimes display overlapping clinical, morphological, imaging, and immunohistoche

Long-term results following surgical removal of pulmonary metastases in children with malignomas.

Between 1972 and June 1983, 21 children (mean age 9 years, range 20 months to 16.5 years) were operated on for pulmonary metastatic disease. Primary malignant tumors were Wilms-tumor (7 patients), osteogenic sarcoma (7 patients), Ewing's sarcoma (4 patients), hepatoblastoma (2 patients), and rhabdomyosarcoma (1 patient). The surgical intervention was part of a therapeutic pediatric oncological concept with curative purpose including chemotherapy and/or radiation in different combinations. Ten out of these 21 children survived disease-free 3 years and more after pulmonary metastasectomy. An agg

Round cell sarcoma of the colon with CIC rearrangement.

BACKGROUND: The CIC-rearranged sarcoma is a very rare highly aggressive malignant soft tissue group of tumors. It has recently been described as highly aggressive soft tissue tumors of children and young adults sharing similar morphological features with the Ewing sarcoma. The digestive localization is exceptional. CASE PRESENTATION: A 14-year-old male presented with a history of abdominal pain for 1 year, which increased in intensity over the last 2 months. Imaging findings showed a large heterogeneous mesenteric mass on the left flank of the abdomen. Exploratory laparotomy was performed and

Current concepts and future perspectives in retroperitoneal soft-tissue sarcoma management.

Retroperitoneal soft-tissue sarcomas are complex, heterogeneous cancers requiring expert multidisciplinary care. They can occur anywhere in the retroperitoneal abdominal or pelvic space. Usually large at presentation they present particular challenges for both local treatment and systemic control. The most common adult subtypes are liposarcomas and leiomyosarcomas, followed by pleomorphic sarcoma/malignant fibros histiocytoma (an entity not always easily distinguishable from dedifferentiated liposarcoma). A variety of additional histotypes may also be observed, but are uncommon in the retroper
