An optimality-guided model of differential adaptation to opposing perturbationsa).

This paper introduces an artificial neural-network-based computational model of differential sensorimotor adaptation to opposing auditory feedback perturbations in speech. A neural Optimality Predictor generating (near-)optimal articulation plans is trained to balance maximal intelligibility and minimal articulatory effort, based on lexical, phonemic, and stylistic context. Adaptation is driven by two main mechanisms: updates of internal sensory predictors estimating sensory outcomes of articulatory plans, and updates of the Optimality Predictor. Simulations replicate experimentally observed d

Automated cleft speech evaluation using speech recognition.

Perceptual evaluation remains the gold-standard evaluation of cleft speech, but with any human interpretation, there can be bias. Eliminating bias, allowing comparison of speech data between units, is labor and time intensive. Globally, there is a shortage of listeners. We have developed a computer learning system to evaluate cleft speech. Our automated cleft speech evaluator interprets resonance and articulatory cleft speech errors. Speech recognition engines typically ignore voice characteristics and speech errors of the speaker, but in cleft speech evaluation, these features are paramount.

Relationship between acoustic measures and speech naturalness ratings in Parkinson's disease: A within-speaker approach.

This study investigated the acoustic basis of across-utterance, within-speaker variation in speech naturalness for four speakers with dysarthria secondary to Parkinson's disease (PD). Speakers read sentences and produced spontaneous speech. Acoustic measures of fundamental frequency, phrase-final syllable lengthening, intensity and speech rate were obtained. A group of listeners judged speech naturalness using a nine-point Likert scale. Relationships between judgements of speech naturalness and acoustic measures were determined for individual speakers with PD. Relationships among acoustic meas

Conversational discourse analysis as a method for evaluating progress in aphasia: a case report.

The gains made by a woman with Broca's aphasia as documented by traditional measures were paralleled by changes in conversation, including increased verbal output and efficiency, and changes in conversation repair patterns. Her conversational partner decreased her verbal output, as predicted. The progress documented with conversational discourse analysis was not observable from other test measures. Clinical and theoretical implications are discussed.

Speech modification by a deaf child through dynamic orometric modeling and feedback.

Baseline physiologic, acoustic, and phonetic data are presented to characterize speech production of a 3 1/2-year-old deaf girl prior to a visual articulatory modeling and feedback program. These observations suggest that she used an articulatory strategy based on visual information about lip and jaw movements rather than tongue positions as a primary means of differentiating speech sounds. The training program which followed used instrumentally generated displays of tongue position and movements to teach production of the (i) and (a) vowels in single and bisyllable word contexts. Utilizing speech analysis to differentiate progressive supranuclear palsy from Parkinson's disease.

INTRODUCTION: Distinguishing Parkinson's disease (PD) from Progressive supranuclear palsy (PSP) at early disease stages is important for clinical trial enrollment and clinical care/prognostication. METHODS: We recruited 21 participants with PSP(n = 11) or PD(n = 10) with reliable caregivers. Standardized passage reading, counting, and sustained phonation were recorded on the BioDigit Home tablet (BioSensics LLC, Newton, MA USA), and speech features from the assessments were analyzed using the BioDigit Speech platform (BioSensics LLC, Newton, MA USA). An independent t-test was performed to com

Speech habilitation of hard of hearing adolescents using electropalatography and ultrasound as evaluated by trained listeners.

Four adolescents with moderate to severe sensorineural hearing losses and moderately unintelligible speech participated in a 14-week speech therapy study using two dynamic visual feedback technologies, electropalatography and ultrasound imaging. Electropalatography provides information about tongue-hard palate contact points. Ultrasound displays images of tongue shape and movement in two dimensions from the tip to the root. Treatment targets for all participants included a sibilant place contrast (/s/ versus /[symbol: see text]/), liquids /l/ and /[symbol: see text]/, and the tense-lax vowel c

Random versus blocked practice in treatment for childhood apraxia of speech.

PURPOSE: To compare the relative effects of random vs. blocked practice schedules in treatment for childhood apraxia of speech (CAS). Although there have been repeated suggestions in the literature to use random practice in CAS treatment, no systematic studies exist that have directly compared random with blocked practice in this population. METHOD: Using an alternating treatments single-subject design with multiple baselines across behaviors, the authors compared random and blocked practice in 4 children diagnosed with CAS in terms of retention and transfer. Random and blocked practice were

Connecting stuttering management and measurement: I. Core speech measures of clinical process and outcome.

BACKGROUND: There will always be a place for stuttering treatments designed to eliminate or reduce stuttered speech. When those treatments are required, direct speech measures of treatment process and outcome are needed in clinical practice. AIMS: Based on the contents of published clinical trials of such treatments, three 'core' measures of treatment outcome are presented for consideration by clinicians: measures of stuttering rate, speech rate and speech naturaleness. MAIN CONTRIBUTION: In contrast to clinical trials, however, clinicians are at liberty to have as many outcome measures as t

How Temporal Overlap in Task Processing and Acoustic Load Affect Listening Effort in Adults.

Listening effort reflects the cognitive resources allocated to overcome obstacles in auditory processing. While acoustic degradation (e.g., … A) significantly increased word recognition response times and subjective effort ratings, even when speech intelligibility remained high (∼90%). Noise effects emerged only at longer SOAs, but vanished at very short SOAs where capacity was exhausted, indicating that SOA is a stronger predictor of listening effort than noise exposure. Interestingly, mild reverberation red …

Transient auditory nerve demyelination as a new mechanism for hidden hearing loss.

Hidden hearing loss (HHL) is a recently described auditory neuropathy believed to contribute to speech discrimination and intelligibility deficits in people with normal audiological tests. Animals and humans with HHL have normal auditory thresholds but defective cochlear neurotransmission, that is, reduced suprathreshold amplitude of the sound-evoked auditory nerve compound action potential. Currently, the only cellular mechanism known for HHL is loss of inner hair cell synapses (synaptopathy). Here we report that transient loss of cochlear Schwann cells results in permanent auditory deficits

Functional outcomes of sensate versus insensate free flap reconstruction in oral and oropharyngeal reconstruction: A systematic review.

The purposes of this article were to review the literature regarding subjective outcomes, objective functional outcomes, and objective sensation return after sensate and/or insensate free flap reconstruction of the oral cavity and oropharynx. Sensate radial forearm and anterolateral thigh free flaps tend to have better static 2-point discrimination and pressure threshold sensitivity than insensate flaps. There is insufficient evidence to draw conclusions on whether sensate flaps improve functional speech and swallowing outcomes measured by videofluoroscopic swallowing studies and percentage of

[Hearing loss and Alzheimer's disease].

Recent studies suggest that subjects with hearing loss are more likely to develop Alzheimer's disease. Hearing loss can be consecutive to presbycusis and/or to central auditory dysfunction. Standard audiometric measures (pure tone and speech intelligibility) allow the diagnosis of presbycusis. However, to demonstrate central auditory dysfunction, specific audiometric tests are needed such as noisy and/or dichotic tests. Actually, no consensus exists to investigate hearing loss in people with Alzheimer's disease though hearing loss may be an early manifestation of Alzheimer's disease. Until now…
