Predicting pattern and spread of brain atrophy and tau deposition in progressive supranuclear palsy.

BACKGROUND: Progressive supranuclear palsy (PSP) is a 4-repeat tauopathy characterized by clinicopathological heterogeneity. The complex interplay between tau deposition, structural changes, and disease spread are unclear. OBJECTIVES: To investigate the temporospatial patterns of tau propagation and neurodegeneration using multimodal imaging with MRI and flortaucipir (FTP) PET, and determine relationship with clinical heterogeneity. METHODS: 150 PSP patients (n = 66 Richardson's syndrome [PSP-RS], n = 26 parkinsonism [PSP-P], n = 25 speech-language [PSP-SL], n = 13 progressive gait freezing

Uncovering Image-Driven Subtypes with Distinct Pathology and Clinical Course in Autopsy-Confirmed Four Repeat Tauopathies.

OBJECTIVES: The four-repeat (4R) tauopathies are a group of neurodegenerative diseases, including progressive supranuclear palsy (PSP), corticobasal degeneration (CBD), and globular glial tauopathy (GGT). This study aimed to characterize spatiotemporal atrophy progression using structural magnetic resonance imaging (MRI) and to examine its relationship with clinical course and neuropathology in a cohort of autopsy-confirmed 4R tauopathies. METHODS: The study included 85 autopsied patients (54 with PSP, 28 with CBD, and 3 with GGT) who underwent multiple 3T MRI scans, as well as neuropsycholog

Multidisciplinary management of laryngeal pathology identified in patients with COVID-19 following trans-laryngeal intubation and tracheostomy.

BACKGROUND: COVID-19 disease often requires invasive ventilatory support. Trans-laryngeal intubation of the trachea may cause laryngeal injury, possibly compounded by coronavirus infection. Fibreoptic Endoscopic Evaluation of Swallowing (FEES) provides anatomical and functional assessment of the larynx, guiding multidisciplinary management. Our aims were to observe the nature of laryngeal abnormalities in patients with COVID-19 following prolonged trans-laryngeal intubation and tracheostomy, and to describe their impact on functional laryngeal outcomes, such as tracheostomy weaning. METHODS:

Picture Description and Functional Communication Rating Correlates in Variants of Primary Progressive Aphasia.

BACKGROUND: Generalization of speech-language pathology treatment is an important goal in clinical practice and research. Functional communication rating scales are often used to investigate potential treatment effects in daily life. Connected speech samples more closely reflect meaningful changes in communication as a result of treatment than test/re-test outcomes. Little attention has been directed to this relationship in aphasia due to neurodegenerative etiologies. AIMS: We investigated whether correlations between ratings on the Georgetown University Center for Aphasia Research and Rehabi…

A stepwise approach to the diagnosis and treatment of hereditary hearing loss.

What To Do Do suspect a genetic cause in all cases of hearing loss. Do develop a working knowledge of common types of HHI that you may draw on to aid in diagnosis. Do think of HHI when the audiogram reveals a hearing loss with a "cookie bite" configuration. Do refer the infant to a geneticist in cases where you suspect a syndromic HHI, a nonsyndromic HHI, and in cases of "cryptogenic" hearing loss where an underlying HHI may be present. Often, the associated symptoms are subtle and best detected by a professional who deals with these issues on a daily basis. Do get the infant or family plugged

[Foreign accent syndrome in a case of dissociative (conversion) disorder].

Psychiatric symptoms are often manifested in verbal expression. Generally, the contents of such expression are pathological. The formal abnormalities of speech are also observed in various mental disorders, as far as quantitative abnormalities are concerned. However, disturbance of intonation, namely disprosody, is more commonly observed in organic disorders of the brain. When the accent of words and the intonation of sentences changes from that of a native speaker, the speech sounds like the broken language of untrained foreigners. Such foreign accent syndrome is usually an issue of neuropsyc

[The symptomatic treatment of amyotrophic lateral sclerosis].

Patients with amyotrophic lateral sclerosis (ALS) have symptoms of progressive muscle weakness, of disturbed speech and swallowing, and in the terminal phase those of respiratory weakness. Treatment options, in particular those for excessive weight loss and respiratory weakness, should be introduced to the patients and their families when the patient is emotionally capable and before dysarthria severely hampers communication. Special equipment for keeping the patient as mobile as possible should be made available much earlier than in the case of other diseases of the muscles as in ALS progress

The Persian Version of Reflux Sign Assessment Scale: Validity and Reliability for the Examination of Patients with Laryngopharyngeal Reflux Disease.

OBJECTIVES/HYPOTHESIS: The non-specificity of signs associated with laryngopharyngeal reflux disease (LPRD) makes the diagnosis challenging. This study aimed to evaluate the psychometric properties of the Persian version of reflux sign assessment (RSAp) in LPRD. STUDY DESIGN: This was a methodological study. METHODS: The prefinal version of RSAp was developed in a forward and backward translation protocol. It was completed by a speech-language pathologist (SLP) and an otolaryngologist for 20 LPRD patients to provide a final version. The final version was completed by a SLP (rater 1) for 42 L

Is Thyrohyoid Space Palpation a Reliable and Useful Clinical Tool Within Diagnostic Protocol for Voice Disorders?

OBJECTIVES: Palpation of the extrinsic laryngeal muscles is a common part of examination performed by otolaryngologists and speech-language pathologists on individuals presenting with voice complaints, thought to aid in diagnosis and treatment planning. While research has identified a significant relationship between thyrohyoid tension and hyperfunctional voice disorders, we are not aware of any studies exploring correlations between thyrohyoid posture during palpation and the full spectrum of voice disorders. This study aims to identify whether patterns in thyrohyoid posture at rest and durin

A cleft care workshop for speech and language pathologists in resource-limited countries: The participants' experiences about cleft care in Uganda and satisfaction with the training effect.

OBJECTIVES: workshops and specialized training programs are often inaccessible for speech and language pathologists (SLPs) based in resource-limited countries given the lack of supply, the long travel distances and the excessive participation fees. To stimulate life-long learning opportunities for all, this study described and measured the effect of a free, two-day cleft care workshop for SLPs in Uganda. The workshop included different topics related to the assessment and treatment of children with a cleft of the palate with or without a cleft of the lip (CP ± L). METHODS: The participants wh

Message banking: Perceptions of persons with motor neuron disease, significant others and clinicians.

PURPOSE: Message banking is an intervention strategy that has the potential to facilitate effective communication for people with motor neuron disease when their condition deteriorates to the extent that they cannot communicate using natural speech. The aim of the current study was to determine and compare the perceptions on message banking of three stakeholder groups, namely, persons with motor neuron disease, their significant others and speech-language pathologists. METHOD: A comparative group survey design was used. Participants listened to a short presentation about message banking, afte

Management of chronic refractory cough.

Chronic refractory cough (CRC) is defined as a cough that persists despite guideline based treatment. It is seen in 20-46% of patients presenting to specialist cough clinics and it has a substantial impact on quality of life and healthcare utilization. Several terms have been used to describe this condition, including the recently introduced term cough hypersensitivity syndrome. Key symptoms include a dry irritated cough localized around the laryngeal region. Symptoms are not restricted to cough and can include globus, dyspnea, and dysphonia. Chronic refractory cough has factors in common with

VFS interjudge reliability using a free and directed search.

Reports in the literature suggest that clinicians demonstrate poor reliability in rating videofluoroscopic swallow (VFS) variables. Contemporary perception theories suggest that the methods used in VFS reliability studies constrain subjects to make judgments in an abnormal way. The purpose of this study was to determine whether a directed search or a free search approach to rating swallow studies results in better interjudge reliability. Ten speech pathologists served as judges. Five clinical judges were assigned to the directed search group (use checklist) and five to the free search group (u

Dysphagia following cardiovascular surgery: a clinical overview.

The objective of this clinical paper is to enhance nurses' understanding of normal swallowing physiology and post-operative swallowing impairments (dysphagia) in patients following cardiovascular surgery. Ultimately, the goals of dysphagia assessment and management are to prevent pulmonary complications secondary to aspiration and to ensure safe and adequate nutritional intake and hydration. Risk factors for dysphagia include stroke, compromised respiratory status, endotracheal intubation and/or tracheostomy tubes, and a dependency for feeding and oral care. Other patient characteristics assoc

Speech disfluencies in children with developmental dyslexia: How do they differ from typical development?

BACKGROUND: Disfluency is a multifactorial concept that can be linked to several of the language production levels, in both typical and atypical populations. In children, the language system is still developing and few studies have explored disfluency patterns. In typical development (TD), in particular, studies have shown discrepancies according to the language being considered. In neurodevelopmental disorders, such as developmental dyslexia (DD), it is still unclear whether the pattern of disfluency is similar to TD. AIMS: To analyse the type of disfluency and their evolution in French chil

Speech pathologists' experience of involving people with stroke-induced aphasia in clinical decision making during rehabilitation.

PURPOSE: Although client participation has been part of legislation and clinical guidelines for several years, the evidence of these recommendations being implemented into clinical practice is scarce, especially for people with communication disorders. The aim of this study was to investigate how speech pathologists experienced client participation during the process of goal-setting and clinical decision making for people with aphasia. METHODS: Twenty speech pathologists participated in four focus group interviews. A qualitative analysis using Systematic Text Condensation was undertaken. RES

Learning in the arena: healthcare students' lived experience of interprofessional education in equine-assisted services.

This qualitative study explored healthcare students' lived experiences during an interprofessional education (IPE) event incorporating equine-assisted services (EAS). Traditional IPE events often rely on classroom settings that may limit interprofessional engagement, collaboration, and application. Utilizing interpretive phenomenological analysis, researchers examined how the equine environment influenced interprofessional learning among fourteen graduate students from diverse healthcare disciplines at a western United States public university. Participants engaged in experiential demonstratio
