CHRNA1 promotes the pathogenesis of primary focal hyperhidrosis.

The aim of the study was to elucidate the involvement of cholinergic receptor nicotinic alpha 1 subunit (CHRNA1) in the pathogenesis of primary focal hyperhidrosis (PFH). The hyperhidrosis mouse model was constructed using pilocarpine injection. The expression levels of CHRNA1 in sweat gland tissues of PFH patients and hyperhidrosis mice were compared using Western blots and quantitative real-time PCR (qRT-PCR) analyses. Sweat secretion in hyperhidrosis mice treated with small-interfering RNA (siRNA) targeting CHRNA1 (si-CHRNA1) or non-specific siRNA were compared. Sweat secretory granules in …

Impact of CFTR modulator concentrations on clinical response in cystic fibrosis.

RATIONALE: Cystic fibrosis (CF) is caused by variants in the CF transmembrane conductance regulator (CFTR), leading to defective chloride ion transport and multiorgan dysfunction. CFTR modulators substantially improve chloride ion transport and disease severity, but responses vary, which may in part be due to variation in drug concentrations. OBJECTIVES: This study aimed to evaluate modulator concentrations among people with CF and the potential impact of cytochrome P450 (CYP)3A5 genotypes on sweat chloride. METHODS: This multicentre study enrolled 97 children and adult participants establis…

"Pagetoid" eccrine carcinoma of the vulva: report of an unusual case with review of the literature.

Sweat gland carcinoma of the vulva is rare and may be classified as being of eccrine, apocrine, or mixed origin. Most reported cases of vulvar sweat gland carcinomas associated with extramammary Paget disease describe a tumor of apocrine origin. We report a case of a vulvar sweat gland carcinoma of eccrine origin associated with Pagetoid extension. A review of the literature and the differential diagnosis are also presented. To our knowledge, this is the second case of vulvar sweat gland carcinoma of eccrine origin associated with extramammary Paget disease.

[Sudomotor dysfunction in Parkinson's disease].

Sudomotor function was evaluated by using the sympathetic skin response (SSR) and the sweat response to intradermal acetylcholine (ACh) injection in 69 patients with Parkinson's diseases (PD). The incidence of SSR abnormality (34.8%) was as high as that of orthostatic hypotension (30.4%) and increased with the severity of the illness. Anticholinergic drug did not influence the incidence of SSR abnormality. Therefore, the SSR is useful in evaluating sudomotor efferent pathway in PD patients. Moreover, in all patients, sweat response to ACh showed a reduced number of excitable sweat glands and a…

Anhidrosis accompanied by cholinergic urticaria-like rash and dermal pain in a patient with Sjögren's syndrome.

Cholinergic urticaria (CholU)-like rash and dermal pain on sweating occur in patients with acquired idiopathic generalized anhidrosis (AIGA). However, it is unclear whether these are symptoms specific to AIGA among the various types of acquired generalized anhidrosis/hypohidrosis (AGAH). Moreover, the pathogenesis underlying CholU-like rash and dermal pain observed with anhidrosis remains to be clarified. A 20-year-old Japanese man with Sjögren's syndrome (SS) presented with anhidrosis. Transient stinging pain on the skin and pinpoint wheals were observed when his body temperature increased. T…

Equine anhidrosis: a review of pathophysiologic mechanisms.

Anhidrosis is loss of the ability to sweat. The problem is seen in horses kept in a hot humid climate, and it may cause severe impairment of thermoregulation in the equine athlete. British Thoroughbreds imported to her tropical colonies are the earliest recorded cases, and since then the syndrome has come to be described as one of Thoroughbreds, usually performance athletes, undergoing acclimatization to heat and humidity. A recent epidemiologic study of cases in Florida has shown, however, that many different breeds, and long time inhabitants of a hot climate, may be affected. Equine sweat gl…

Wearable sensors for monitoring drug pharmacokinetics in patients with Parkinson's disease.

Wearable sweat sensors have attracted significant attention in recent years due to their potential for noninvasive health monitoring and point-of-care treatment. In this study, we developed and clinically evaluated wearable electrochemical sweat sensors in 32 patients with Parkinson’s disease (PD) at Taichung Veterans General Hospital, Taiwan. PD patients were treated with standard oral L-DOPA formulations, in terms of Madopar or Sinemet. PD symptoms were assessed using standard MDS-UPDRS scores, 10 clinical movement metrics, wearable inertial measurement units (IMUs) data, and sweat L-DOPA co…

A case of severe refractory chronic urticaria: a novel method for evaluation and treatment.

With cholinergic urticaria (ChU), the ultimate diagnosis often depends on the demonstration of characteristic urticaria by appropriate provocation. Several treatment options may be helpful but traditional options (antihistamines, leukotriene inhibitors, and immunosuppressives) may be exhausted by the refractory ChU patient. Here, we describe such a case. Demonstration of immediate hypersensitivity to autologous sweat skin testing (ASwST) may provide a rationale for use of omalizumab (Xolair, Genentech Novartis, South San Francisco, CA). Patients with severe ChU may have difficulty producing su…

Sudomotor function in familial dysautonomia.

BACKGROUND: Patients with familial dysautonomia (FD) manifest episodic hyperhidrosis despite the reduction of sudomotor fibres and sweat glands associated with this autonomic neuropathy. We assessed peripheral sudomotor nerve fibre and sweat gland function to determine if this symptom was due to peripheral denervation hypersensitivity. METHODS: In 14 FD patients and 11 healthy controls, direct and axon reflex mediated sweat responses were determined by measuring transepidermal water loss (TEWL) after application of acetylcholine via a microdialysis membrane, a novel method to evaluate sudomot…

Quantifying hydration-related measures and their association with wearable-derived ergonomic risk in warehouse workers.

INTRODUCTION: Warehouse workers may experience prolonged heat exposure and sweat loss, which can influence fatigue and movement patterns related to ergonomic risk. However, evidence in this occupational group remains limited. Therefore, we aimed to quantify acute (pre- and post-shift) and across-week (Days 1-4) changes in urine specific gravity (USG) and explore the associations between hydration-related measures (Δ urine color, sweat rate, and total fluid intake), wearable-derived ergonomic risk metrics, and self-reported fatigue. METHODS: A total of 25 employees at a Florida beverage bottli…

Autonomic thermoregulatory dysfunction in neurofibromatosis type 1.

OBJECTIVE: Neurofibromatosis type 1 (NF1) causes neural and cutaneous disorders and reduced exercise capacity. Exercise/heat exposure increasing internal temperature must be compensated by eccrine sweat function and warmed skin vasodilation. We suspected NF1 could adversely affect eccrine sweat function and/or vascular thermoregulatory responses (VTR). METHODS: The eccrine sweat function and VTR of 25 NF1 volunteers (14 males, 11 females; 16-57 years old) were compared with 23 non-NF1 controls matched by sex, age, height and weight (CG). Sweating was induced by 1) pilocarpine 1% iontophoresis…

Does extensive genotyping and nasal potential difference testing clarify the diagnosis of cystic fibrosis among patients with single-organ manifestations of cystic fibrosis?

BACKGROUND: The phenotypic spectrum of cystic fibrosis (CF) has expanded to include patients affected by single-organ diseases. Extensive genotyping and nasal potential difference (NPD) testing have been proposed to assist in the diagnosis of CF when sweat testing is inconclusive. However, the diagnostic yield of extensive genotyping and NPD and the concordance between NPD and the sweat test have not been carefully evaluated. METHODS: We evaluated the diagnostic outcomes of genotyping (with 122 mutations included as disease causing), sweat testing and NPD in a prospectively ascertained cohort…

Diagnosis of palmar hyperhidrosis via questionnaire without physical examination.

OBJECTIVES: In order to determine the reliability of a self-administered instrument to diagnose excessive sweating conditions, including palmar hyperhidrosis (PH), we designed two successive questionnaires and compared responses with physical examination and sweat measurement in normal volunteers and a cohort of patients with documented PH. The reliable diagnosis of PH via questionnaire would enable molecular epidemiological studies without the need for physical examination or direct sweat measurement. METHODS: Subjects self identified as either normal or affected by PH. Each completed one or…

Cytochemistry of the skin of patients with mucopolysaccharidoses.

The distribution of complex carbohydrates has been investigated at the light and electron microscope levels in sweat glands of normal subjects and patients with Hurler's or Hunter's disease. Normal sweat glands examined with a battery of light microscopic histochemical methods revealed sulphated complex carbohydrate in secretory granules of the dark cells. These granules lacked affinity for dialysed iron (DI) at the light and electron microscope levels. The DI method demonstrated acid complex carbohydrates ultrastructurally on the surface of the intercellular canaliculi and central lumen in no…

Familial generalized anhidrosis.

A young Caucasian male, otherwise healthy, had had generalized anhidrosis since early childhood. During heat stress tests (40 degrees C, 40% relative humidity), he was found to be heat intolerant since sweat was not apparent. His reaction to muscarinic stimulation of sweat glands was 10% of normal. On biopsy, the sweat glands were morphologically intact, and function of his cardiovascular autonomic responses was normal. The patient's mother reported reduced sweating and her response to muscarinic stimulation was 50% of normal, but his father and both sisters sweated normally. The data suggest …

Acquired idiopathic generalized anhidrosis: a distinctive clinical syndrome.

The present study concerns a 28-year-old Japanese man with acquired generalized anhidrosis. The patient's ability to perspire was investigated in an artificial climate room maintained at 40 degrees C and 40% humidity. Although the body temperature rose to 38 degrees C, the patient did not sweat. Neither did sweating occur when the patient was given an intradermal injection of pilocarpine or nicotine. The serum IgE level was elevated. Atrophy and degeneration of the sweat glands, as well as infiltration by lymphocytes and mast cells around the sweat glands, were observed in skin biopsies. Anhid…

Miliaria crystallina induced by idarubicin and all-trans-retinoic acid: Two case reports.

Miliaria crystallina is a benign, self-limiting disorder of the eccrine sweat glands characterized by the obstruction of the sweat ducts, which leads to secondary sweat retention into stratum corneum. We present two patients with MC during treatment with idarubicin and all-trans-retinoic acid (ATRA) for acute promyelocytic leukaemia (APL). Anthracyclines can be excreted through sweat and induce MC through exfoliation. The use of idarubicin in combination with ATRA would favour the process of producing a peeling effect. Reports of MC associated with idarubicin and ATRA are scarce. Recognizing t…

Focal hypohidrosis in lesional skin in a probable case of confluent and reticulated papillomatosis: A case report with insight into the pathomechanism of recurrence.

Confluent and reticulated papillomatosis (CRP) is a rare skin disorder that develops in young adults and presents as persistent brown papules and plaques predominantly affecting the intertriginous areas, however, its etiopathogenesis remains elusive. Herein, we report a probable case of CRP with lesional hypohidrosis as detected by sweat test and provide insight into the pathomechanism. A 23-year-old man presented with nine-months history of painful sensation on his trunk without any skin change. The result of sweat test was compatible with acquired idiopathic generalized anhidrosis. Topical h…
