The Treatment of Primary Focal Hyperhidrosis.

Primary focal hyperhidrosis is a relatively common disease that has a significant impact on afflicted patient’s quality of life. The pathoge … an also be used for intractable disease. Last-line interventions include local surgical options and sympathectomy, though some patients may prefer permanent treatment. Further investigation of novel treatments as well as ways to optimize existing therapeutic options are needed.

Renal sympathetic denervation for the treatment of refractory hypertension.

Resistant hypertension poses significant health concerns. There are strong demands for new and safe therapies to control resistant hypertension while addressing its common causes, specifically poor compliance to lifelong polypharmacy, lifestyle modifications, and physician inertia. The sympathetic nervous system plays a significant pathophysiological role in hypertension. Surgical sympathectomy for blood pressure reduction is an old but extremely efficacious therapeutic concept, now abandoned with the dawn of a safer contemporary pharmacology era. Recently, clinical studies have revealed promi…

Place of video-thoracoscopy in thoracic surgical practice.

The advent of video-endoscopy revolutionizes the practice of surgery. Within a short span of time, video-assisted thoracic surgery (VATS) has become an acceptable approach to a wide range of thoracic procedures. The use of VATS as a diagnostic modality is now well established. For therapeutic procedures, VATS has also been generally accepted for the treatment of such conditions as primary spontaneous pneumothorax, loculated effusions, thoracodorsal sympathectomy, and resection of simple mediastinal cysts. Its roles in more complex procedures such as thymectomy and anatemic lung resections, how…

Adverse effects of spinal and epidural anaesthesia.

Spinal and epidural blockade are similar in many respects in the anaesthesia they produce, since they both cause major conduction blockade with local anaesthetic agents. Physiologically, the blockade involves the production of a sympathectomy with venous pooling and decreased venous return, causing decreased cardiac output and hypotension. Physiological complications include bradycardia, heart block and, rarely, cardiac arrest. Nonphysiological complications include high or total block from extensive spread of the local anaesthetic agent, and toxic reactions from inadvertent intravenous inject…

Pulmonary artery denervation in pulmonary hypertension: A comprehensive meta-analysis.

INTRODUCTION: Pulmonary hypertension (PH) is a serious condition characterized by increased pulmonary vascular resistance and elevated pulmonary artery pressure, leading to right heart failure and high mortality rates. Conventional treatments primarily include vasodilators, which provide symptomatic relief but do not effectively reverse the underlying vascular pathology. Pulmonary artery denervation (PADN) has emerged as a novel therapeutic approach targeting the sympathetic nervous system's role in PH. OBJECTIVE: This meta-analysis aims to evaluate the impact of PADN on hemodynamic parameter

Renal Denervation in Asia: 2025 Asia Renal Denervation Consortium (ARDeC) Consensus Statement Endorsed by the Hypertension Cardiovascular Outcome Prevention and Evidence in Asia (HOPE Asia) Network.

The second Asia Renal Denervation Consortium consensus conference shared information and developed updated recommendations for renal denervation (RDN). Current evidence confirms that RDN significantly reduces blood pressure across all metrics (office, home, and ambulatory) throughout 24 hours. Modern RDN approaches target the distal main renal artery and branches where nerves more closely approximate the vessel wall. Understanding renal artery anatomy is crucial; the main renal artery typically divides into anterior and posterior divisions as first-order branches, which further subdivide into

Combined percutaneous renal artery denervation and angioplasty: still too early to consider them complementary?

In case of renovascular hypertension, percutaneous renal artery transluminal angioplasty alone is still far from demonstrating predictable clinical results. Assuming a potential synergistic effect, we reported the first case of combined percutaneous renal denervation and angioplasty as intention-to-treat strategy in a very high cardiovascular risk patient.

Effects of renal sympathetic denervation on the course of congestive heart failure combined with chronic kidney disease: Insight from studies with fawn-hooded hypertensive rats with volume overload induced using aorto-caval fistula.

Background: The coincidence of congestive heart failure (CHF) and chronic kidney disease (CKD) results in poor survival rate. The aim of the study was to examine if renal denervation (RDN) would improve the survival rate in CHF induced by creation of aorto-caval fistula (ACF).Methods: Fawn-hooded hypertensive rats (FHH), a genetic model of spontaneous hypertension associated with CKD development, were used. Fawn-hooded low-pressure rats (FHL), without CKD, served as controls. RDN was performed 4 weeks after creation of ACF and the follow-up period was 10 weeks.Results: We found that intact (no

Renal denervation: basic and clinical evidence.

Renal nerves have critical roles in regulating blood pressure and fluid volume, and their dysfunction is closely related with cardiovascular diseases. Renal nerves are composed of sympathetic efferent and sensory afferent nerves. Activation of the efferent renal sympathetic nerves induces renin secretion, sodium absorption, and increased renal vascular resistance, which lead to increased blood pressure and fluid retention. Afferent renal sensory nerves, which are densely innervated in the renal pelvic wall, project to the hypothalamic paraventricular nucleus in the brain to modulate sympatheti

[Renal denervation for the treatment of refractory hypertension: where are we?].

Renal denervation (RDN) is a device-based procedure historically studied in the treatment of resistant hypertension. Studies conducted over the years have produced conflicting results and its application in routine antihypertensive treatment is still debated. Recent trials using different devices have shown promising results in the treatment of patients with hypertension at low-moderate cardiovascular risk. Although long-term results are still needed to confirm the efficacy and safety of the procedure, there is reason at the moment to consider an application of RDN in the treatment of selected

A systematic review and meta-analysis of T2, T3 or T4, to evaluate the best denervation level for palmar hyperhidrosis.

We systematically reviewed and compared the clinical outcomes of thoracoscopic sympathectomy (TS) at different denervation levels for palmar hyperhidrosis. We searched PubMed, Ovid MEDLINE, EMBASE, Web of Science, ScienceDirect, the Cochrane Library, Scopus and Google Scholar for relevant studies published during 1990-2016. Symptom resolution, patient satisfaction, compensatory sweating (CS), recurrence, dry hands and gustatory sweating were assessed. We selected 13 studies from 2228 for the final analysis. A comparison of T2 vs. T3 TS revealed that T3 TS reduced the risk of CS (95% confidence…

Palmar hyperhidrosis: clinical, pathophysiological, diagnostic and therapeutic aspects.

Palmar hyperhidrosis affects up to 3% of the population and inflict significant impact on quality of life. It is characterized by chronic excessive sweating, not related to the necessity of heat loss. It evolves from a localized hyperactivity of the sympathetic autonomic system and can be triggered by stressful events. In this study, the authors discuss clinical findings, pathophysiological, diagnostic and therapeutic issues (clinical and surgical) related to palmar hyperhidrosis.

An update on renal artery denervation and its clinical impact on hypertensive disease.

Hypertension is a globally prevalent condition, with a heavy clinical and economic burden. It is the predominant risk factor for premature cardiovascular and cerebrovascular disease, and is associated with a variety of clinical disorders including stroke, congestive cardiac failure, ischaemic heart disease, chronic renal failure, and peripheral arterial disease. A significant subset of hypertensive patients have resistant hypertensive disease. In this group of patients, catheter-based renal artery denervation has emerged as a potential therapy, with favourable clinical efficacy and safety in e

A Case Report of Renal Sympathetic Denervation for the Treatment of Polymorphic Ventricular Premature Complexes: Expanding Horizons.

Premature ventricular complexes are very common, appearing most frequently in patients with hypertension, obesity, sleep apnea, and structural heart disease. Sympathetic hyperactivity plays a critical role in the development, maintenance, and aggravation of ventricular arrhythmias. Recently, Armaganijan et al reported the relevance of sympathetic activation in patients with ventricular arrhythmias and suggested a potential role for catheter-based renal sympathetic denervation in reducing the arrhythmic burden. In this report, we describe a 32-year-old hypertensive male patient presenting with
