The impact of implementing a hospital electronic prescribing and administration system on clinical pharmacists' activities - a mixed methods study.

BACKGROUND: The increasing adoption of hospital electronic prescribing and medication administration (ePA) systems has driven a wealth of research around the impact on patient safety. Yet relatively little research has sought to understand the effects on staff, particularly pharmacists. We aimed to investigate the effects of ePA on pharmacists' activities, including interactions with patients and health professionals, and their perceptions of medication safety risks. METHODS: A mixed methods study comprising quantitative direct observations of ward pharmacists before and after implementation

Implementation of cardiovascular disease risk prediction scores in Sub-Saharan Africa: a scoping review.

BACKGROUND: Cardiovascular disease (CVD) risk prediction scores are widely recommended to guide primary prevention, yet their use in routine care across sub-Saharan Africa (SSA) remains limited. As the number of available scores continues to increase, understanding how they are implemented is important in closing the gap between policy and practice. This scoping review aims to map and synthesise the available literature on the implementation of CVD risk prediction scores in SSA. METHODS: This review was conducted in accordance with JBI methodology. MEDLINE, Embase, Global Health (Ovid), CINAH

ECG-based detection of occlusion myocardial infarction: a dedicated deep neural network versus multimodal large language models and physicians - a retrospective diagnostic accuracy study.

BACKGROUND: Accurate electrocardiogram (ECG) interpretation for acute coronary occlusion is a critical, time-sensitive task in emergency care. The field is increasingly shifting from the ST-segment elevation myocardial infarction paradigm to the broader concept of occlusion myocardial infarction (OMI). This study compared the diagnostic accuracy of a dedicated occlusion-detection deep neural network, Queen of Hearts (QoH), two general-purpose multimodal large language models (LLMs), and emergency physicians for ECG-based OMI detection. Response consistency and confidence calibration of the LLM

Altered Decision-Making Across Acute and Chronic Pain States: A Bias Towards Context-Dependent Valuations.

Chronic pain has been linked to changes in learning and decision-making, but the underlying cognitive mechanisms remain unclear. In this study, 239 participants with either chronic, acute, or no pain completed a reinforcement learning task designed to dissociate choices based on global expected value from those driven by context-specific reinforcement history. While learning performance was comparable across groups, individuals with chronic pain relied more on recent reinforcement history and less on integrated value representations during decision-making. Computational modeling confirmed that

Transformer-Based Multi-Task Learning with a Task-Aware Weighted Ensemble for Simultaneous Age Estimation and Gender Classification from Facial Images.

Accurate age estimation and gender classification from facial images remain challenging tasks owing to substantial variations in facial appearance caused by pose, illumination, expression, occlusion, image quality, and age-related morphological changes. Although transformer-based architectures have recently demonstrated remarkable performance in facial analysis, effectively exploiting their complementary representation capabilities within a unified multi-task framework remains an open research problem. To address this limitation, this study proposes a transformer-based Multi-task Learning (MTL

Effects of Ih and TASK-like shunting current on dendritic impedance in layer 5 pyramidal-tract neurons.

Pyramidal neurons in neocortex have complex input-output relationships that depend on their morphologies, ion channel distributions, and the nature of their inputs, but which cannot be replicated by simple integrate-and-fire models. The impedance properties of their dendritic arbors, such as resonance and phase shift, shape neuronal responses to synaptic inputs and provide intraneuronal functional maps reflecting their intrinsic dynamics and excitability. Experimental studies of dendritic impedance have shown that neocortical pyramidal tract neurons exhibit distance-dependent changes in resona

Functional reorganization of the reading network in the course of foreign language acquisition.

During foreign language acquisition neural representations of native language and foreign language assimilate. In the reading network, this assimilation leads to a shift from effortful processing to automated reading. Longitudinal studies can track this transition and reveal dynamics that might not become apparent in behavior. Here, we report results from a longitudinal functional magnetic resonance imaging (fMRI) study, which tracked functional changes in the reading network of beginning learners of Greek over one year. We deliberately chose Greek as foreign language that would have similar o

Effect of beta-adrenoceptor antagonists on autonomic control of ciliary smooth muscle.

PURPOSE: Pharmacological intervention with peripheral sympathetic transmission at ciliary smooth muscle neuro-receptor junctions has been used against a background of controlled parasympathetic activity to investigate the characteristics of autonomic control of ocular accommodation. METHODS: A continuously recording infra-red optometer was used to measure accommodation on a group of five visually normal emmetropic subjects under open- and closed-loop conditions. A double-blind protocol between saline, timolol and betaxolol was used to differentiate between the localised action on ciliary smoo

Task-dependent cross-frequency neural coupling during postural perturbation: insights from EEG-based assessment in elite freestyle aerial skiers.

BACKGROUND: Cross-frequency coupling (CFC), particularly phase-amplitude coupling (PAC), reflects hierarchical interactions between neural oscillations and plays a critical role in sensorimotor integration. However, its functional relevance during balance control under sensory perturbations remains insufficiently understood. OBJECTIVE: This study aimed to investigate PAC characteristics during postural control tasks of varying difficulty in elite freestyle aerial skiers versus non-athlete controls. METHODS: EEG signals and center of pressure (COP) data were recorded from participants perform

The Effects of Long Duration Spaceflight on Sensorimotor Control and Cognition.

Astronauts returning from spaceflight typically show transient declines in mobility and balance. Other sensorimotor behaviors and cognitive function have not been investigated as much. Here, we tested whether spaceflight affects performance on various sensorimotor and cognitive tasks during and after missions to the International Space Station (ISS). We obtained mobility (Functional Mobility Test), balance (Sensory Organization Test-5), bimanual coordination (bimanual Purdue Pegboard), cognitive-motor dual-tasking and various other cognitive measures (Digit Symbol Substitution Test, Cube Rotat

Relationship between anxiety and executive functioning among youth with Down syndrome.

Down syndrome (DS) is commonly associated with various cognitive and behavioral challenges including difficulty with executive functioning (EF) and elevated levels of anxiety symptoms. In the general population, high levels of anxiety are associated with difficulties with EF tasks like initiation and cognitive shifting, with some bidirectional findings between these two constructs. Given the high overlapping behavioral presentation of symptoms of anxiety and challenges in EF, the present study examined the relationship between anxiety symptoms and EF in a sample of 139 individuals with DS ages

Spatial attention modulation of the brain network involved in mental time travel.

OBJECTIVES: The ability to mental time travel (MTT) consists in moving along a cognitive and spatially oriented representation of time, that is, an ideal mental time line, where past and future events are, respectively, located on the left and on the right portion of such a line. A shift of spatial attention by prismatic adaptation (PA) influences this spatial coding of time, thus affecting MTT. Here, we investigated the neural correlates of such a spatial modulation on MTT in a functional Magnetic Resonance Imaging protocol. METHOD: To study MTT ability, participants were asked to indicate i

Cognitive flexibility assessment in youth with Down syndrome: Reliability, practice effects, and validity.

BACKGROUND: Cognitive flexibility refers to the ability to switch between different mental sets, tasks, or strategies and is challenging for some individuals with Down syndrome (DS). The lack of reliable and valid cognitive flexibility measures for individuals with DS is a major barrier to clinical trials and intervention studies designed to address cognitive challenges specific to DS. To avoid measurement limitations that could confound interpretations of performance in clinical trials in children with DS, it is critical to use phenotype-sensitive and psychometrically sound measures of cognit

Pain and executive functions: a unique relationship between Stroop task and experimentally induced pain.

There is a growing body of evidence that a higher level of cognitive inhibition is associated with lower experimental pain sensitivity. However, a systematic examination of the association between executive functions, which include not only inhibition but also updating and shifting, and experimental pain sensitivity is lacking. This study aimed to overcome this limitation by exploring the relationship between a range of executive functions and different measures of experimentally induced cold pain in healthy participants. In a group of 54 healthy participants (age 21-24 years), executive funct

Subjective somatosensory vertical during dynamic tilt is dependent on task, inertial condition, and multisensory concordance.

To investigate how visual and vestibular cues are integrated for the perception of gravity during passive self-motion, we measured the ability to maintain a handheld object vertical relative to gravity without visual feedback during sinusoidal roll-tilt stimulation. Visual input, either concordant or discordant with actual dynamic roll-tilt, was delivered by a head-mounted display showing the laboratory. The four visual conditions were darkness, visual-vestibular concordance, stationary visual scene, and a visual scene 180 degrees phase-shifted relative to actual tilt. Tilt-indication performa

Occipital-parietal interactions during shifts of exogenous visuospatial attention: trial-dependent changes of effective connectivity.

We studied neural interactions between brain areas involved in exogenous (stimulus-driven) control of visuospatial attention. With event-related functional magnetic resonance imaging (fMRI), we investigated changes of connectivity during shifts of spatial attention from an attended location to a previously unattended target location. Using a 3-T scanner, fMRI data were acquired from three healthy volunteers. According to a central visual cue, participants directed endogenous spatial attention to the left or the right visual hemifield for blocks of 56 s. Peripheral visual targets were presented

Visuospatial attention in dementia of the Alzheimer type.

Cue-directed shifts of spatial attention were examined for a letter-discrimination task in 15 patients with mild to moderate dementia of the Alzheimer type (DAT) and 15 healthy, age-matched controls. Spatial cues were valid, invalid or neutral in indicating probable target location and were presented either centrally at fixation or peripherally 6.7 degrees to the left or right of fixation. Stimulus-onset asynchrony (SOA) between cue and target was varied between 200 ms and 2000 ms. Reaction time (RT) benefits conferred by valid cues did not differ between the DAT group and the controls. Howeve

Multimodal imaging of intracameral foreign body.

BACKGROUND: Retinopathy of Prematurity (ROP) is a vasoproliferative retinal disorder and a major cause of childhood blindness worldwide. Clinical ROP management involves three correlated tasks: Plus disease detection, stage classification, and treatment decision-making. Although deep learning (DL) has shown promise for automated ROP management, several challenges remain: (1) retinal vascular morphology is central to Plus disease assessment, but DL-based vascular feature modeling is not always explicitly integrated into unified end-to-end multi-task frameworks; (2) Plus disease detection, stage

Estimating Musculoskeletal Torque from Ultrasound Images with Regression and Sensor Alignment.

Predicting a userâs joint torque is beneficial for improving the design and control of gait-assist robotic devices, ultimately aiding in restoring motor function for individuals with neurological disorders. Joint torque has been shown to correlate with muscle structural characteristics, and brightness-mode (B-mode) ultrasound (US), as a non-invasive, low-cost, and high-sampling-rate technique, has been widely utilized to capture muscle morphology. However, conventional ultrasound-based methods are prone to noise and errors arising from algorithmic limitations and sensor-shift during or across
